STATEMENT OF ISSUE: Approval and execution of Change Order Request (COR) No. 15 in the amount
of$8,212.00 for BlIchan"1'l General Contracting Company (Contmctor).
ANAL~: On January 25, 2010. the BoardofCQunty CommiAAioners approved and executed a contract
with Buchanan General Contracting Company, P.O. Box 40069, Bellevue" W A 98015 for the construction of
the Jet'fCom 90.1-1 Radio System Ln1"ul/ement Project. Construction started at the South (Coyle) and Maynw:d
Peak sites on February 8. Work began at the Green Mountain site the first week of March. The following COR
is essential for the completion of construction at the let'fCom 911 fucllity site. Change Order Request No. 15 is
attaclted for your approval and execution.
There is a radio tower adjacent to the Port Hadlock JefiComIDepartme of Emergency Management fucllity
with numerous cables coming from the tower, through an existing 'port' and into the facility. That port is
entirely full and Additionll1 radio cables need to be installed as part of the project. COR No. 15 provides for the
Contmctorto excavate adjacent to the easterly wall of the facility, core drill 3 -6" holes through the concrete
wall into the mechaniad eqW,jnn.mt room, and install 2 ~ 4" conduits (one each per 6" hole) and 3 - 2"
conduits (in one of the 6" holes) from the radio tower into the <1juipment room for the current project and
future needs. Change Order No. IS was approved by the Jet'fCom A,dm1n1mauve Board on August 26, 2010.
FISCAL IMPACT: Funds will be provided by the JeffCom Enbam:ement Fund 121 which was funded by a
2007 bond i$ue. The total amount of the original contract with sales tax is $364,728.08. Change Order
Requests No. 1 through 14 increased the current approved cost to $453,697.08. The addition oCCOR No. 15 in
the amoun1of$8,212.oo brings the new total to $461,697.08. The total ofCORs 1 through 15 is $97,181.00 or
24.65% of the original contract. Sufficient funds are available.
RECOMMENDATION: Move to approve and execute Change Order Request No. 15 to the Jet'fCom 911
Radio System Improvement Col11rlwt with Buchanan Geneml CQl11rlwting Company.
