stATEMENT OF ISSUE: Amendment No.1 to Inter10cal Agreement for Professional
services - Jefferson County Library District. This extends the agreement from January 1,

AMENDMENT NO.1
TO
INTERLOCAL AGREEMENT FOR PROFESSIONAL SERVICES
The Interlocal Agreement for Professional Services by and between Jefferson County
("COUNTY") and the Jefferson County Library District ("PUBLIC ENTITY") is amended as
follows:
TERM: AMENDMENT
Services performed under this contract is amended to read, "shall begin on January 1,2011 and
shall continue through December 31, 2011 for the issuance of warrants; and through February
28,2012, for the submission of required annual reports to employees, the Internal Revenue
Service, the Social Security Administration, the Department of Retirement, the Labor and
Industries Department, the Employment Security Department and PUBLIC ENTITY."
COMPENSATION AND METHOD OF PAYMENT
Fees for services is amended to read, "shall be in accordance with the fee schedule attached as
Exhibit B and shall not exceed $5,000 annually."

$ 25.89/hr.
$ 35.62/hr.
