2128-0, EPA Grant Agreement No. 02J97501
Statement of Issue: Change Order No. 4 with Earthwork Solutions LLC of Arlington,
Washington.
Analysis/Strategic Goals/Pro's Et Con's: This change order is for the following
changes: additional septic pumping, not included in the original bid; and overruns for
two existing bid items (Bid Item No. 10 Pothole Existing Utilities and Bid Item No. 33
Connection of On-site Grinder Pump System to the Existing Building Drain).
Fiscal Impact/Cost Benefit Analysis: The amount for Change Order No. 4 is
$26,319.59. The contract amount will increase from $1,069,445.55 to $1,095,765.15.
NOTE, the cost includes 9.2% sales tax. The project is funded by the Department of
Ecology combined funding package (Centennial grant, State Revolving Fund (SRF) loan
and SRF forgivable loan) and the FY 2022 and 2023 EPA Community Grant Funds.
Recommendation: Public Works recommends that the Board authorize Change Order
No. 4 with Earthwork Solutions LLC and return one original to Public Works for further
processing.
