This funding continues to support the Proctor Home Project and is included in my

This Contract between the State of Washington Department of Social and Health Services (DSHS) and the
Contractor is hereby amended as follows:
The purpose of this amendment is to award the Federal Fiscal Year (FFY) 2009 JABG Grant.
1. In Exhibit A - Statement of Work, #4. Contract Project Dates, is amended as follows:
The project start date is July 1, 2010, with an estimated duration of 12 months, ending on June 30, 2011.
For the new grant period of July 1, 2010 through June 30, 2011, the maximum consideration is onlv
$11,200 with a match amount of $1,244.44.
No unspent funds from the previous grant period of July 1, 2009 through June 30, 2010 may be carried
forward.
The amended total maximum consideration for this contract that includes the past grant periods is
$S1,200.
2. Due to an increase in JABG funding for FFY09, the Contractor shall complete the JRA provided "FFY
2009 - JABG County Budget", incorporated herein by reference. The Contractor shall submit the
completed Budget to the JRA Program Administrator identified on page one of this Agreement prior to
their first billing under the FFY 2009 JABG grant.
3. All references to "Exhibit C - Approved Budget Summary" and "Exhibit 0 - Approved Budget Detail" are
deleted and replaced with "FFY 2009 - JABG County Budget".
4. In Section #6. Acknowledgement of Assistance, delete "Award No. 2008-JB-FX-0038" and replace with
"Award No. 2009-S0S89-WA-JB".
5. In Section #20. Reports, add:
"The Contractor shall complete the JRA provided "FFY09 - JABG Annual Data Report", Incorporated
herein by reference, and submit to the JABG Program Administrator identified on page one of this
Agreement no later than May 1S, 2011."
All other terms and conditions of this Contract remain in full force and effect.
