This work, conducted within the DIAGNOSE CTE Study based in Boston, examined CSF biomarkers in a deeply phenotyped cohort of former professional and collegiate football players and age
matched unexposed controls. We evaluated Alzheimer’s disease-related CSF measures - including Abeta40/42 and multiple tau species - to determine whether repetitive head impacts (RHI)
or a clinical diagnosis of traumatic encephalopathy syndrome (TES) were associated with biomarker changes.

We found that CSF Abeta40 and Abeta42 levels were both reduced in individuals with football exposure under age 60, and in those with cognitive impairment. Unexpectedly, total tau, pTau181,
and pTau217 were not associated with football exposure, RHI metrics, or TES clinical features. Although CSF Abeta42 reductions are characteristic of Alzheimer’s disease, the parallel
decreases in both Abeta40 and Abeta42 - and the absence of changes in CSF pTau - suggest that this biomarker pattern differs from typical AD. These findings indicate that RHI, TES,
and CTE - widely regarded as a tauopathy - may have a distinct CSF biomarker profile, including potentially different pTau signatures, from that of AD. Novel CSF biomarkers that better
capture brain injury–related neurodegeneration, and that distinguish RHI driven pathology from age related comorbidities, may be needed for early detection of individuals at risk for
CTE.

Most notably, staff at the NW MIRECC pioneered the use of the blood pressure medication prazosin to treat trauma-related nightmares, one of the most common symptoms of military PTSD.
Due in part to our educational outreach efforts, this medication is now prescribed to over 100,000 Veterans and active-duty Servicemembers, and we continue to investigate its usefulness
