The road approach permit(RAP11-00005) must be inspected and finaled by the Jefferson County Department of Public Works prior to the final inspection/certificate of occupancy of this residence. Contact Terry Duff, Department of Public Works at 360-385-9159 for the final road approach inspection. 2.) 2005 DOE STORMWATER MANUAL DRYWELL DESIGN CRITERIA ALL INFILTRATION SYSTEMS SHOULD BE AT LEAST 10 FEET FROM ANY STRUCTURE, PROPERTY LINE, OR SENSITIVE AREA. FOR SITE WITH SEPTIC SYSTEMS, INFILTRATION SYSTEMS MUST BE DOWNGRADIENT OF THE DRAINFIELD UNLESS THE SITE TOPOGRAPHY CLEARLY PROHIBITS SUBSURFACE FLOWS FROM INTERSECTING THE DRAINFIELD. DRYWELL BOTTOMS MUST BE A MINIMUM OF 1 FOOT ABOVE SEASONAL HIGH GROUNDWATER LEVEL OR IMPERMEABLE SOIL LAYERS. VOLUNTARY MEASURES OF COASTAL &AT RISK SIPZ: Water conservation measures: 1. Roof and other intercepted precipitation shall be routed to on-site detention ponds and/or other approved means and allowed to be released to the soil slowly. 2. Water collected from Storm water and roof catchments may be used for watering lawns and gardens. Unless catchment water has been treated to meet drinking water standards, there shall be no cross connections allowed between the potable supply and impounded water. 3. Water withdrawn from wells on each property shall not be used for watering of lawns and/or gardens. 4. Ground water withdrawn from each property shall be restricted to a rate of three (3) gallons per minute. 5. Installation of water conserving fixtures such as low flow toilets, faucets and shower restrictors and other water saving plumbing fixtures. 6. Landscaping plan (xeriscaping, native vegetation with minimal amounts of irrigation). Please NOTE that the above listed measures are not intended to be exhaustive, but rather is intended to be illustrative of the types of water conservation measures. 5.) MANDATORY MEASURES FOR SSTAL SIPZ: • 1. For proof of potable water on a building permit application, applicant must utilize DOH-approved public water system if available. 2. If public water is unavailable, a qualifying alternative system may be used as proof of potable water or an individual well may be used as proof of potable water subject to the following requirement: a. Chloride concentration of a laboratory-certified well water sample submitted with building permit application. 3. If public water is unavailable, a qualifying alternative system may be used as proof of potable water. 6.) Critical Aquifer Recharge Areas may require special protection measures to mitigate water quality degradation. The submitted proposal does not require additional aquifer protection measures. However, during construction the project shall follow the Best Management Practices (BMPs) and facility design standards as identified and defined in the Stormwater Management Manual for the Puget Sound Basin. 7.) The project shall adhere to the Best Management Practices (BMPs) to control stormwater, erosion and sediment during construction. BMPs shall address permanent measures to stabilize soil exposed during construction, and in the design and operation of stormwater and drainage control systems. 8.) A Stormwater Plan has been submitted and approved by the Department of Community Development. Once the subject permit has been issued the applicant shall fully implement the provisions of the submitted plan and contact the Jefferson County Department of Community Development to arrange a schedule to inspect the property for plan compliance. A Certificate of Occupancy will not be issued until the Department verifies plan compliance. No clearing for roadways or utilities shall occur on the project site until clearing necessary for the installation of temporary sedimentation and erosion control measures have been completed. 9.) Outdoor residential storage shall be maintained in an orderly manner and shall create no fire, safety, health or sanitary hazard. 10.) Not more than 2 unlicensed vehicles shall be stored on any lot unless totally screened from view of neighboring dwellings and rights-of-way. Such screening shall meet all applicable performance and development standards specific to the district in which the storage is kept, and shall be in keeping with the character of the area. Screening shall meet the requirements of Chapter 18.30 JCC. Outdoor storage of 3 or more junk motor vehicles is prohibited except in those districts where specified as an automobile wrecking yard or junk (or salvage)yard and allowed as a permitted use in Table 3-1 or Chapter 18.18 JCC, and such storage shall meet the requirements of JCC 18.20.100, Automobile wrecking yards and junk (or salvage) yards. In no case, shall any such junk motor vehicles be stored in a critical area. 11.) A minimum of two (2) on-site parking spaces shall be provided for the single family residence. 12.) Maximum lot coverage is not to exceed 25%. Lot coverage is defined as amount of impervious surface which includes roof tops, driveways, concrete, etc. 13.) The building height is not to exceed 35 feet. 14.) Setback from Shine Road right-of-way is no less than 20 ft., setback from SR 104 right-of-way is no less than 8 ft.,side and rear setbacks are no less than 5 ft. If an environmentally sensitive area (ESA) is present, then the more restrictive setback shall apply. 15.) Exterior lighting for residential uses shall not exceed twenty feet(20') in height from the finished grade, excepting when such lighting is an integral part of a building or structure. Ground level lighting is encouraged. 16.) Lighting fixtures shall be designed. hooded to prevent the light source from g directly visible from outside the boundaries of the property. Project Overview 1. Describe the proposed developed conditions of the site. Indicate position and relative size of proposed improvements on the site map. Construct 1 story 2,469 SF single family residence with 2 on grade patios and 644 sf detached garage and 1 story 2,054 SF detached Garage/ Shop in existing cleared area of the site approximately 299' from OHWM of Squamish Harbor adjacent to 601 Shine Rd. Existing Conditions Summary 2. Describe the existing topography. Indicate contours on the site map. 2.63 acre site divided by State Highway 104 slopes from N to S at a consistent rate rising approximately 42'from top of bank to the North property line of the portion lying South of State Route 104. 3. Describe the existing vegetation. Indicate native vegetation areas on the site map. The portion of the property South of State Route 104 is mostly cleared and in pasture. The portion North of the cemetery is partially treed with doug fir. hemlock, alder and maple with little undergrowth. North of State Route 104 is heavily wooded. 4. Describe the existing soils. Indicate soil type on the site map. Soil is consistent throughout the site. Site soils consist of a 0"-12" layer of dark brown loam over light brown medium sand with scattered gravels and cobbles to a minimum depth of 5' . 5. Describe the existing site hydrology(i.e., drainage; behavior of water on the site—above, below, and on the ground). Indicate existing stormwater drainage to and from the site on the site map. Depict separate drainage basins on the site map, if applicable,and indicate acreage of each. Lot is consistently dry and dries out rapidly after rainfall. An existing culvert crossing State Route 104 above the cemetery occasionally carries a small amount of water that dissipates before it reaches Shine Road. 6. Portion of property South of State Route 104 slopes consistently from N to S with an elevation change of 42' in 458' (9.2%)to the top of a 10' -12' high bank on the North shoreline of Squamish Harbor. 8. Describe the general vicinity of the site, including adjacent land uses and structures, utilities, roads, and sensitive/critical areas (streams,wetlands, lakes, steep slopes,etc.). Property is on the North shore of Squamish Harbor. Adjacent to the site are single family homes and the Shine Cemetery. Access to the site is via existing paved Jefferson County road, Shine Road. Connection to power, Water, telephone and cable are all existing on the property. There are no known crtical or sensitive areas on nr• adjacent to the site. Permanent Stormwater Control Plan This portion of the Stormwater Site Plan consists of the selection and installation of the appropriate stormwater control BMPs and facilities to remain in place after construction of the project is completed. "Medium" size projects are required to have the totals calculated of all impervious surfaces, pollution-generating impervious surfaces, and pollution-generating pervious surfaces to verify that the thresholds for treatment facilities and flow control facilities are not exceeded. g. Describe the developed site hydrology, as proposed. Indicate whether stormwater will be fully dispersed (i.e., per BMP T5.30 in the Manual) or, if not, what types of stormwater flow control will be utilized for the site or specific threshold discharge areas within the site. Locate these facilities on the site plan and differentiate proposed facilities from existing facilities. Roof drains and foundation drains will be connected to gravel drywells to collect and disperse stormwater. 10. If the project requires the use of stormwater treatment facilities, describe the types of stormwater treatment facilities proposed for use on the site. Locate these facilities on the site plan and differentiate proposed facilities from existing facilities. [This is normally for"large"projects or projects that involve the potential for dispersion of contaminants.] N/A 11. Describe the performance goals and standards applicable to the project. Minimal impact to the site. Developed and disturbed areas of the site is 19.255 of site area with new impervious area at 12% of site area. All disturbed areas of the site will be reestablished to pasture, its current condition. 12. Describe the flow control system. Flow control system will be 7ea 4'x4'x5' gravel drywells (1:1,000 SF of roof area) lined with geotextile fabric and constructed per 2005 Stormwater Management in Western Washington, Volume III. Wood chips or straw will be used to cover exposed ground during construction as required during wet weather. 14. Describe the conveyance system analysis and design. Stormwater will be directed via downspouts and tight lines to drywells ass located on the Site Plan, 3ea 4'x4 'x5' at the Garage/Shop and 4ea 4'x4'x5' for the new residence Number of drywells based upon 1:1, 000 SF of roof area. 15. Describe the source of fill material, physical characteristics of fill material, and deposition of excess material. Fill material will be native material excavated for construction from the site. Gravel and sand as required per construction details will be per the Structural Notes. It is anticipated to be a balanced site for excavation. If any excess materials are generated, they will be disposed of at an approved fill site. 16. Proposed methods of placement and compaction consistent with the applicable standards on Appendix Chapter 33 of the Uniform Building Code. Clean naive fill material will be placed in no more than 12" lifts and compacted with vibratory compaction equipment. Footings will bear on undisturbed soil. 17. Describe the proposed surfacing material. New drives and parking areas will be 3/4" minus crushed basalt placed by truck and compacted with a vibratory roller. 18. Describe methods for restoration of the site. All areas not covered by new construction will be graded to tie into existing grades and replanted as pasture to match existing. Existing topsoil will be stripped, stockpiled and reused. 19. An Operation and Maintenance Manual is required for each flow control and treatment facility. F 1/)f(r t Ty non # # CONSTRUCTION STORMWATER POLLUTION PREVENTION PLAN(SWPPP) The Construction SWPPP addresses sediment and erosion control during construction. The BMPs indicated by the applicant in the template that follows must be installed on the ground during all construction phases of the project. The proponent is responsible for preventing sediment and erosion impacts to environmentally sensitive areas and off-site areas. Consult the Construction Stormwater Pollution Prevention (SWPP) Best Management Practices (BMPs) Packet for guidance, particularly with rural residential development. SECTION I—CONSTRUCTION SWPPP NARRATIVE 1 Construction Stormwater Pollution Plan Elements. Describe how each of the Construction SWPPP elements has been or will be addressed. Identify the type and location of BMPs used to satisfy the required element. If an element is not applicable to the proposal,justify in writing. Descriptions of the 12 Elements are found at section 2.5.2 of the Manual(beginning on page 2-15). 12 Required Elements—Construction SWPPP 1. Mark Clearing Limits. Existing site is already cleared, no additional trees or native vegetation are planned for removal. The septic system transport line will be routed so as to avoid the drip line of the maple tree as indicated on the Site Plan. 2. Establish Construction Access. Construction access will be via paved county road and new crushed rock driveway. 3. Control Flow Rates. Site drains naturally to the South. Silt fence will be installed at edge of site disturbance as required. 4. Install Sediment Controls. Wood chips or straw will be utilized to cover disturbed soils as required during wet weather. Silt fence along southern edge of soil disturbance as required. 5. Stabilize Soils. Wood chips or straw as required. 6. Protect Slopes. Bank slope is on the opposite side of Shine Road from the construction and is protected by a drainage ditch along the North side of Shine Road. Construction excavation slopes will be covered by 6 mil visqueen until backfill is has been completed. 7. Stabilize Channels and Outlets. None 9. Control Pollutants. Wood chips or straw will aid in pollutant control and can be collected, removed and replace if necessary. The majority of existing vegetation will remain undisturbed below the construction site and will help to filter stormwater. Wash water is available on site and a spill kit will be maintained on site. 10. Control De-Watering(the act of pumping groundwater or stormwater away from an active construction site). If necessary clean stormwater will be pumped uphill into existing vegetation and allowed to filter down the existing pasture. 11. Maintain Best Management Practices(BMPs). Each Subcontractor will be receive instructions regarding proper site management along with their directions to the site. The Owner, acting as General Contractor will make regular site inspections. 12. Manage the Project. The Owner, acting as General Contractor will make regular site inspections and will enforce all aspects of this plan. 2. Adjacent Areas. a. Description of the adjacent areas that may be affected by site disturbance (e.g., streams, lakes, wetlands, residential areas, roads). Single family residences developed East and West of the site will not be disturbed because water does not flow in their direction. Very little construction will take place north of the cemetery adjacent to the site. The septic system, drywells and geothermal field will be installed during dry weather. b. Description of the downstream drainage path leading from the site to the receiving body of water(minimum distance of 400 yards). OHWM of Squamish Harbor is approximately 299' South of proposed house location. Any water leaving the site is intercepted by a grass lined drainage ditch located along the North edge of Shine Road. 3. Environmentally Sensitive Areas. a. The submittal shall include substitute pages that include all proposed changes, revised drawings showing any structural changes,and any other supporting information that explains and supports the reason for the change. Final Corrected Plan Submittal If the project included construction of conveyance systems, treatment facilities, flow control facilities, or structural source control BMPs (not standard on-site stormwater management BMPs), the applicant shall submit a final corrected plan ("as-builts")when the project is completed. These should be engineering drawings (stamped by a licensed civil engineer) that accurately represent the project as constructed.
