This proposal requires a public hearing with a decision by the Jefferson County Hearing Examiner per JCC 18.42.080(3) Table 13-1. A copy of the application and all the reports are available on Jefferson County's Customer Self -Service Portal (see below). Location: The 6.12 acre parcel is zoned Rural Residential 1 dwelling per 5 acres (RR1:5), and is located north of Swansonville Road and Fleet Drive, and west of Jefferson Avenue in the Port Ludlow Area. The parcel is in Section: 8, Township: 28N, Range: 1 E, W.M. (APN 821081015). Description of Proposal: Horizon Tower in conjunction with Verizon Wireless is proposing a new WCF. The proposal is to construct a new 150' monopole tower with attached antennas within a 50'x50' fenced area along with ground mounted equipment and generator. The tower will improve coverage and network reliability in rural Jefferson County. Horizon Tower has obtained an agreement with the property owner that will allow for construction and maintenance of this facility, necessary utility connections, and provisions for access to the site. Reports Submitted: The applicant has submitted documentation in their application regarding their Radio Frequency propagation maps and how co -locating the proposed antennas is not feasible on existing towers within their desired service area. They also provided a photo simulations of the view of the proposed monopole from key viewing area. A copy of all the studies and reports are available at the DCD Office or on the DCD customer self-service portal. Permits required: The implementation of this proposal would require building and address permit from Jefferson County DCD. The lease of the 50 by 50 foot area for the proposed WCF facility is exempt from the land division requirement per RCW 58.17.040(8). This proposal would also require approval from Federal Communication Commission (FCC) and clearance from the Federal Aviation Administration (FAA). SEPA Threshold Determination: Jefferson County Department of Community Development (DCD) is the lead agency for this proposal and issued a preliminary threshold determination of a Determination of Non -Significance (DNS) on November 5, 2025 per WAC 197-11-355. The comment period ended on November 19, 2025. DCD received comments from agencies and the public regarding this proposal. The SEPA record included DCD SEPA Memo dated May 12, 2026, application, the design of this proposal, the studies/reports submitted, comments from agencies and the public, and the existing Jefferson County development regulations. DCD has determined that this proposal will not result in probable significant adverse impacts to the environment. Based on the above, DCD has issued a final threshold determination of a DNS on May 13, 2026. There is no additional SEPA comment for this proposal. PROPOSAL: Horizon Tower in conjunction with Verizon Wireless is proposing a new Wireless Communication Facility (WCF). This proposal is referred by the applicant as the Mats Mats Bay site. The proposal is to construct a new 150' monopole tower with attached antennas within a 50'x50' fenced area along with ground mounted equipment and generator. The tower will improve coverage and network reliability in rural Jefferson County. Horizon Tower has obtained an agreement with the property owner that will allow for construction and maintenance of this facility, necessary utility connections, and provisions for access to the site. In addition to Verizon's antennas at the top of the monopole, the proposal could co -locate three other wireless communication antennas on the monopole. This project requires a TYPE III Zoning Conditional Use Permit (ZON2025-00011), with a notice of application and public hearing and an open record public hearing before the Jefferson County Hearing Examiner (H.E.) per JCC 18.42.080(3) Table 13-1. STAFF AMENDMENTS TO THE ENVIRONMENTAL CHECKLIST The following sections correspond with related categories of the environmental checklist submitted for the proposal, and clarify, amend, or add to that document. CHECKLIST ITEMS EARTH The proposed site is predominantly flat with a knoll located in the northern section of the parcel where the cell tower would be located. The proposal would be located within a 50 by 50 foot compound area. From the knoll there is minimal slope to the north, there is a 7.5% slope to the western property line (PL), a 10% slope to the eastern PL, and a 4% slope to the southern property line. The site contains trees and native shrubs. The compound areas would be served by a 30 foot wide access and utility easement. Grading and clearing would be anticipated with a 12 foot wide by approximately 400 long gravel road from the southern edge of the parcel to the proposed compound area. This proposal should not entail the importing or exporting of significant quantities of cut or fill material. SEPA Memo Horizon 150 foot high Monopole ZON2025-00001 Page 2 2. AIR The checklist adequately addresses the issues of this section. 3. WATER Surface Water: There are no streams or wetlands on this 6.12 acre parcel or within 200 feet of the boundary of this proposal. Ground Water: This is an unmanned site so no water or sewage disposal is proposed. Water Runoff: The retention of native vegetation around the compound areas should assist in fully dispersing stormwater from the site. The proposal will entail approximately 12 by 400 sq foot access road would entail 4,800 sq ft of impervious surface. The 50 by 50 foot compound area would equate to 2,500 sq ft of impervious surface. This would equate to 7,300 sq ft of impervious surface. All stormwater for non-residential projects is reviewed by the Jefferson County Public Works Department. The engineered stormwater plan would be required to meet the Washington State Department of Ecology's 2024 Stormwater Management Manual of Western Washington. 4. PLANTS Except for the gravel for the access and compound area, this proposal would retain most of the native trees and shrubs for screening of the proposal. The retention of existing trees and vegetation meet the landscaping requirements per Section 18.30.130 JCC. If the vegetation removed the applicant would be required to submit a landscaping plan to meet the landscaping requirements. 5. ANIMALS The checklist adequately addresses the issues of this section. 6. ENERGY AND NATURAL RESOURCES The checklist adequately addresses the issues of this section. Electricity will be provided by the Jefferson County PUD. The site would also have telephone cable install within the 30 foot wide access and utility easement. 7. ENVIRONMENTAL HEALTH The Federal Communication Commission (FCC) has jurisdiction over the maximum permissible exposure for the general public per the Federal Telecommunication Act of 1996. This is addressed in the purpose of the Jefferson County Personal Wireless Service Facilities found in Chapter 18.42 JCC. This is addressed in the Land Use Section below. 8. LAND AND SHORELINE USE Comprehensive Plan: The application addressed compliance with the Jefferson County Comprehensive Plan in stating that the proposal will expanding essential public facilities and service by improving rural telecommunication. In addition, this would protect public safety infrastructure without degrading the rural character. SEPA Memo Horizon 150 foot high Monopole ZON2025-00001 Page 3 Zoning: With the exception of the area to the south located in the Port Ludlow Master Planned resort zone MPR-SF-4, the site and surrounding areas are zoned Rural Residential 1 dwelling per 5 acres (RR1:5). This proposal requires a TYPE III Zoning Conditional Use Permit with a hearing before the Jefferson County Hearing Examiner per Table 13-1 of Section 18.42.080(3) JCC. Zoning CUP Criteria Section 18.40.530 JCC. This proposal is required to meet all of the Zoning CUP criteria in addition to the other Unified Development Code Requirements. This includes the Development Code Standards found in Section 18.30 JCC, which address stormwater, lighting. parking, landscaping, etc. This proposal is also required to meet the Personal Wireless Service Facilities requirements found in Chapter 18.42 JCC. These are the most applicable standards for the review of the construction of this 150 foot high monopole for wireless communication facilities. Chapter 18.42 JCC - Personal Wireless Service Facilities: Section 18.42.010 JCC - Purpose, policy, goals, and interpretation. (1) Purpose. This chapter is designed to provide opportunities for the community to be served by personal wireless service facilities, consistent with the rights of personal wireless service providers as set forth in the Federal Telecommunications Act of 1996 (Pub. L. No. 104-104, 110 State 56, 31-136, codified at 47 U.S.C. 332(c)), hereinafter referred to as "the Telecommunications Act" or "the Act," while at the same time providing for an orderly development of the county and protecting the health, safety, and general welfare of the county's residents and property owners. This chapter is also intended to comply with Section 6409 of the Middle Class Tax Relief and Job Creation Act of 2012 (H.R. 3630, P.L. 112-96), hereinafter referred to as "Section 6409," as implemented by 47 CFR 1.40001, as it exists now or may be amended in the future. (2) Intent and Interpretation. These standards are designed to comply with the Act. The provisions of this chapter shall not be interpreted to prohibit, or to have the effect of prohibiting, telecommunications or personal wireless services, television broadcast signals, multichannel multi -point distribution services or direct broadcast satellite services, or to unreasonably discriminate among providers of functionally equivalent services. The property is not within the jurisdiction of the Shoreline Master Program. The review of this proposal through the Zoning CUP (ZON 2025-00011) will ensure that all of the requirements of the Jefferson County Code are met. 9. HOUSING The checklist is adequate. 10. AESTHETICS This proposal is not in a key viewing area as addressed in Section 18.42.090(2)(a). The applicant has provided photo simulations with the site plan submitted that show the view of the proposed tower from key viewing areas. The design of this proposal locating the monopole on a knoll within the interior of this 6.12 acres parcel and the retention of trees and shrubs outside of the 50 by 50 foot compound area. SEPA Memo Horizon 150 foot high Monopole ZON2025-00001 Page 4 11. LIGHT AND GLARE The only light proposed is a motion activated shielded security light at the entrance of the compound. The proposal is not proposing lighting on the tower. 12. RECREATION This proposal would provide 100 spaces for people to rent for the placement of their own RV and park models. RV's and park models are intended for recreation use. The checklist description is adequate. 13. HISTORICAL AND CULTURAL PRESERVATION This proposal will ensure that historic and cultural resources are protected as part of the federal permitting requirement for the FCC permit. The checklist description is adequate. 14. TRANSPORTATION The subject property is accessed Fleet Drive, a county road. The landowner home is located off an easement to Fleet Drive. The agent will be required to demonstrate that they have legal access the proposal from Fleet Drive. The application indicates that the unmanned facility would be visited only a couple of times a month. 15. PUBLIC SERVICES The objective of this proposal is to provide better telecommunications to the area around the proposal. 16. UTILITIES The proposal would be served by electric and telephone service in the 30 foot access and utility easement. The checklist adequately addresses the issues of this section. CONCLUSIONS AND RECOMMENDATIONS The SEPA record includes this DCD SEPA Memo dated May 12, 2026, application, the design of this proposal, the studies/reports submitted, comments from agencies and the public, and the existing Jefferson County development regulations that are applicable to this proposal. DCD has determined that this proposal will not result in probable significant adverse impacts to the environment. Based on the above, DCD will be issuing a final threshold determination of a DNS on May 13, 2026. There is no additional SEPA comment for this proposal. The SEPA review addresses impacts to the environment, but the proposal is still required to meet all of the applicable sections of the Jefferson County Code as outline above in the Land Use Section of this SEPA Memo. The decision of this proposal will be determined by the Jefferson County Hearing Examiner based on the information in the record and the open record public hearing to be held on June 16, 2026. This information is also helpful to determine if available avoidance, minimization, or compensatory mitigation measures will address the probable significant impacts or if an environmental impact statement will be prepared to further analyze the proposal. Attach any additional information that will help describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain your answers or provide additional information reasonably related to determining if there may be significant adverse impact. Instructions for lead agencies Please adjust the format of this template as needed. Additional information may be necessary to evaluate the existing environment, all interrelated aspects of the proposal and an analysis of adverse impacts.
