Request for Approval of a Type III Wireless Communication Facility (WCF)
with SEPA Determination
On behalf of Horizon Tower, in conjunction with Verizon Wireless, we are submitting a Type III
Conditional Use Permit application for a new Wireless Communication Facility (WCF) located
on an undeveloped parcel in Jefferson County, Port Ludlow, WA 98365 (APN: 821081015). The
proposed facility consists of a 150’ monopole tower with Verizon Wireless antennas and ground-
mounted equipment within a 50' x 50' fenced lease area.
