Statement of Issue:
Execution of a contract change order with Anderson Environmental Contracting, LLC
for the Dismal Creek Culvert Replacement project, County Project No. X01736.
Analysis/Strategic Goals/Pro's & Con's:
Emergency repairs were completed at Upper Hoh Road M.P. 4.0 in March, 2007, with
construction of 400 lineal feet of armored bank protection. Remaining work consists of
mitigation as required by the Washington State Dept. of Fish & Wildlife (WDFW). The mitigation
will be satisfied by replacing the fish-passage barrier culvert at Dismal Creek, Upper Hoh Road
M.P. 9.19, with a three-sided concrete box culvert.
This contract change order will provide for two changes: Part A: the work required for a change
in grade for two culvert wingwall footings, Part B: Six additional contract working days to
complete the three-sided concrete box culvert.
FiscallmpactlCost Benefit Analysis:
The cost of the contract change order is Part A: $1,077.33 and no contract days, Part B: $0.00
and six contract days. This will be funded at 86.5% by the Federal Highway Administration and
13.5% by the Jefferson County roads fund. The original contract amount was $258,327.31
Recommendation:
The attached contract change order is recommended for your execution. Tl:tis action is
recommended, as it will allow for the completion of this project. The Board is asked to sign the
contract change order where indicated, and return two signed originals to Public Works (attn.
John Wayand).

This contract is revised as follows: PART A: This change order adds the new pay item "Item 30, Grade Change for NE
and NW Wlngwall Footings, Lump Sum". This additional item is necessary as it will correct an unforeseen gap at the
vertical joint between the precast Reinf. Cone. Three Sided Structure leg and the corresponding wingwall, for the NE
and NW wingwalls. No addtional contract time is granted for this part of the change order. PART B: In accordance
with Section 1-08.8, this change order adds 6 Contract Days due to the necessity of replacing the third section of Item
15, Precast Reinf. Cone. Three Sided Structure, which was damaged beyond repair during transport by the supplier. The
Engineer has determined that the Contractor had no control over the cause of this delay. No additional payment Is
granted for this part of the change order.
