Statement of Issue:
Execution of the Change Order #1, to the contract for construction for the Hoh Shop
Roof Retrofit Project, with PHC Construction LLC of Bainbridge Island.
This change order provides additional construction services for the above referenced
project.
Analysis/Strategic Goals/Pro's B: Con's:
The contract for construction with PHC Construction LLC, was executed on September
19, 2001. The additional work included in the Change Order #1 was necessary to
complete the project in a practical and responsible manner
Fiscal Impact/Cost Benefit Analysis:
The original contract amount with PHC Construction LLC, is $56,337.74 including
WSST. This Change Order will authorize an additionaL$1, 035.96 including WSST in
payment. This project is funded through local funds.
Recommendation:
We recommend that this Change order to the Contract for Construction be signed by
the BoCC. Please sign and return the (3) originals to Public Works.

PHC Cons1ruction LLC proposes the following changes to the above referenced project in
accordance with the contract and specifications as requested.
1. To relocate the main electric feed to the building 30 inches above new roofwith metal
conduit and to supply the extra wire to the extended metal pipe. This change is based as
a result of the precons1ruction meeting where it was decided that the conduit would
terminate in the horizontal position, 12 inches out from the building.
The price for the above-described changes is: $ 262.52 and includes WSST

Change Order Description: Photo Cell Pipe Relocation
PHC Construction LLC proposes the following changes to the above referenced project in
accordance with the contract and specifications as requested.
1. To relocate the existing photo cell that currently goes through the roof and install it
through the side of the building. PHC will have it installed out past the gutters and then
turned up to extend past the top of the gutters. This will include the extra wire and all
noct"Ssary fittings and pipe to complete relocating.
The price for the above-described changes is: $545.00 this includes WSST

Change Order Description: Replacement of Guide Wires on existing antenna
PHC Construction LLC proposes the following changes to the above referenced project in
accordance with the contract and specifications as requested.
To replace three guide wires on existing antenna with three new wires of 3/16 diameter braided
cable and install three turn buckles to the S-S-E clamps for tightening of cables. The price below
includes 8 percent PHC markup and WSST at 9 percent.
· 6S-feet ofJ/16 galvanized cable 49 cents per foot
. 6- Galvanized cable clamps 79 cents per clamp
. 3- Galvanized turn buckles 2.49 per buckle
. Kiwi Steels Price of$150.00 to install items above
