Amendment No.2 to Interlocal Agreement for Professional
Services - Jefferson Connty Library District
STATEMENT OF ISSUE: Amendment No.2 to Interlocal Agreement for Professional
Services - Jefferson County Library. This extends the agreement from January 1,2012
through December 31, 2014.
ANALYSIS: Extends Contract whereby Jefferson County Auditor issues warrants and annual
reports to employees, IRS, Social Security Administration, State Dept of Retirement, L & !,
Employment Security and the Jefferson County Library.
FISCAL IMPACT: Fees pay for services, not to exceed $5,000 for each year.
RECOMMENDATION: Please sign three (3) originals.

AMENDMENT NO.2
TO
INTERLOCAL AGREEMENT FOR PROFESSIONAL SERVICES
The Interloca1 Agreement for Professional Services by and between Jefferson County
("COUNTY") and Jefferson County.Library District (''PUBLIC ENTITY'')is amended
as follows:
TERM:
AMENDMENT
Services performed under this Agreement is amended to read "shall begin on January 1,
2012 and shall continue through December 31, 2014 for the issuance of warrants; and
through February 28, 2015 for the submission ofrequired annual reports to employees,
the Internal Revenue Service, the Social. Security Administration, the Department of
Retirement Services, the Labor and. Industries Department, the Employment Security
Department and to the PUBLIC ENTITY."
All other terms, conditions and obligations imposed on and agreed upon by the COUNTY
and the PUBLIC ENTITY as memorialized in the Interloca1 Agreement (AFN 551286)
and Amendment No. 1 to said Interloca1 (AFN 556049) remain valid and in full force and
effect as against the COUNTY and the PUBLIC ENTITY.
