Health/Arst Steps team. As a Behavioral Health Spedallst, the contractor will provide counseling
Interventions and education to Arst Steps clients based on Identlt1ed needs and In keeping with the
Identified core services. The Contractor will utilize the client's Identified priorities, Information from the'
screening tool, professional observation, and assessment to participate with other members of the JCPH
Family Health/Arst Steps Team In developing a comprehensive Plan for Care for the maternity cycle.
FISCAL IMPACT /COST ~ENEFIT ANALYSIS:
This contract Is funded by Maternity Support billing and is a contract that Is required by the State. Mental
Health consultation Is a mandated service as part of the Maternal Health program.

CONTRACT AGREEMENT
Between
Jefferson Healthcare
and
Jefferson County Public Health
Section 1: PURPOSE:
THIS AGREEMENT for Professional Services is entered into between Jefferson County herein
referred to as the "County" and Jefferson Healthcare (JHC), herein referred to as the
"Contractor" to provide Behavioral Health Specialist services at Jefferson County Public Health
(JCPH).
Section 2: TERMS:
This Agreement shall commence on March 1,2012 and continue through February 28, 2017
unless terminated as provided herein.
Section 3: SCOPE OF AGREEMENT:
Contractor will provide consultation for First Steps clients' case conferences with the JCPH
Family HealthlFirst Steps Team.
Section 4: COMPENSATION:
The total amount payable under this Contract by County to Contractor shall not exceed
$3,000.00 annnally in completion of this project without express written amendment signed by
both parties to this Agreement Contractor shall be paid at the rate of $50.00 per hour to provide
2 - 4 hours of service per month.
A. Contractor shall be paid for services as outlined in Exhibit A, Statement of Work.
B. Contractor shall submit invoices to JCPH, 615 Sheridan St., Port Townsend, WA 98368,
Attn: Finance Department, once a quarter for payment of work actually completed to
date.
C. The Contractor may submit invoices. The County will review such invoices, and upon
approval thereof, payment will be made to the Contractor in the amount approved.
D. The County will make final payment of any balance due the Contractor promptly upon
determining that i) the Contractor has completed its obligations under this Agreement and
Ii) the County can and does accept the work performed by the Contractor.
E. Contractor records and accounts pertaining to this agreement are to be kept available for
inspection by representatives of the County and state for a period of six (6) years after
final payments. Copies shall be made available upon request.

Section 5: INDEMNIFICATION:
The Contractor shall indemnify, defend and hold hannless the County, its officers, agents and
employees, from and against any and all claims, lawsuits, demands for money damages, losses or
liability, or any portion thereof, including attorney's fees and costs, arising from any injury to
person or persons (including the death or injury of the Contractor or damage to personal property)
if said injury or damage was caused by the negligent acts or omissions of the. Contractor.
Section 6: INSURANCE:
The Contractor shall obtain and keep in force during the terms of this Agreement, or as otherwise
required:
A. Commercial Automobile Liability Insurance providing bodily injury and property
damage liability coverage for all owned and non owned vehicles assigned to
or used in the performance of the work for a combined single limit of not less than
$300,000 each occurrence. Contractor shall provide proof of insurance to the County
clo Contracts Manager at Jefferson County Public Health, 615 Sheridan St. Port
Townsend,W A 98368 prior to commencing employment.
B. Professional Liability Insurance providing $2,000,000 per incident; $4,000,000
aggregate. Contractor shall name Jefferson County Public Health as additional
insured.
C. All employees or subcontractors of the Contractor undertaking the performance of
Social Worker services under this agreement are required to be professionally
licensed or certified by the State of Washington.
D. The Contractor shall participate in the Worker's Compensation and Employer's
Liability Insurance Program as may be required by the State of Washington.
Section 7: CONFIDENTIALITY:
Contractor, its employees, subcontractors and their employees will maintain the confidentiality
of all information provided by JCPH or acquired in performance of this Agreement as required
by HIP AA and other privacy laws. lIDs Co~tract, once executed by the parties, is and remains a
Public Record subject to the provision of Ch. 42.56 RCW, the Public Records Act.
Section 8: INDEPENDENCE
The Contractor and the County agree that the Contractor is an independent Contractor with
respect to the services provided pursuant to this agreement. Nothing in this agreement shall be
considered to create the relationship of employer and employee between the parties hereto. The
Contractor shall not be entitled to any benefits accorded County employees by virtue of the
services provided under this agreement. The County shall not be responsible for withholding or
ottrerwise deducting federal income tax or social security or for contributing to the state
industrial insurance program, otherwise assuming the duties of an employer with respect to
employee.
Section 9: ASSIGNMENTS AND SUBCQNTRACTING
The Contractor shall not sublet or assign any of the services covered by this agreement without
the express written consent of the County.

Section 10: REPORTING
The Contractor will provide a monthly report to the County. The monthly report shall contain a
brief summary of the work performed, relationship to the tasks identified in Exhibit A and the
total hours worked. The monthly report shall be submitted to Jefferson County Public Health in
care of the Jean Baldwin. Director, 615 Sheridan, Port Townsend, and W A 98368 for payment
by the last working day of the month.
Section 11: TERMINATION
The County reserves the right to terminate this contract in whole or in part, with 30 days notice,
in the event that expected or actual funding from any funding source is withdrawn, reduced, or
limited in any way after the effective date of this agreement. In the event of termination under
this clause, the County shall be liable for only payment for services rendered prior to the
effective date of termination.
A. This agreement may also be terminated as provided below:
1. With 30 days notice by the Board of County Commissioners for any
reason, or
2. With 30 days notice by the Board of County Commissioners for non-
performance of the specific joq duties in Exhibit A
3. With 30 days notice by the Contractor by voluntary resignation.
Section 12: MODIFICATION
This professional services agreement may be modified at any time by written agreement of either
party and may be modified if tax revenue decreases
Section 13: DISPUTE RESOLUTION
Any dispute or disagreement concerning the terms and obligations of this Contract shall initially
be brought to the attention of the Public Health Director via a written document. The Public
Health Director shall make her determination with reasonable promptness, i.e., a decision within
ten business days shall be deemed to have been made with reasonable promptness. If the
aggrieved party is not happy with that decision, then the Jefferson County Administrator (or as
may hereafter be amended), will decide the appeal of the Director's decision.
Section 14: INTEGRATED AGREEMENT
This Agreement together with attachments or addenda represents the entire and integrated
agreement between the County and the Contractor and supersedes all prior negotiations,
representations, or agreements written or oral between the parties. This agreement may be
amended only by written instrument signed by both County and Contractor.

Statement of Work

Qualifieations:
The Behavioral Health Specialist must have a current license in the state of Washington as a
Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Psychologist or
registered as a counselor in the state ofW A with a Master's degree in Psychology, Social Work,
Marriage and Family therapy, Counseling or Educational Psychology from an accredited school
with an internship or practicum experience in direct counseling services and one year (40-hour
week, not internships or practicum) supervised experience with direct counseling services to
clients. A Master's prepared person who does not have one year of supervised work experience
requires clinical supervision until they have completed the one year experience requirement or a
registered counselor in the state of W A with a Bachelor's Degree in Psychology, Social Work, or
Educatiomil Psychology from an accredited school plus two years of supervised experience in
direct counseling services to clients. Ongoing clinical supervision is required for Bachelors
prepared staff while in their position.
Description: ,
The Contractor's role on the First Steps Team is to provide brief, counseling interventions,
education and case management for First Steps clients based on identified needs and in keeping
with the identified core services. The Contractor serves as a consultant to the Maternity Support
Services (MSS) team in developing the comprehensive Plan for Care. The Social Worker role is
vital to the First Steps goal of increasing client Self-sufficiency and control over their lives.
Overview:
The MSS Social Worker will utilize the client's identified priorities, information from the screen
tool, professional observation, and assessment to participate with the other members of the MSS
team in developing a comprehensive Plan for Care for the maternity cycle.
The Contractor will provide:
1. Expertise on psychosocial issues, mental health including diagnostics, strategies for
facilitating client change, community reso,urces and improving self-care techniques to
promote emotional well being.
2. An intervention plan developed with the client and other team members based on the
client's identified needs, screening tool, additional evaluation, and assessment of
identified risk areas.
3. Brief treatment [IIldlor transition services to ongoing treatment based on the plan for care.
4. Consultation to the MSS/First Steps Team regarding; the psychosocial needs of pregnant
women and their families and provision of social services and referral support into the
Jefferson Healthcare system.

5. Contractor will review the MSSScreening Tool with client or staff and provide
additional assessment for identified risk factors in a culturally sensitive manner. The
assessment may include, but is not limited to:
a. Current family functioning
b. Individual History: cultural background, health status, substance use/abuse,
violence, legal involvement
c. Review of support systems: formal/informal
d. Basic physical needs (such as food, housing, clothing)
e. Educationlvocation needs .
f. Psychosocial stressors
g. Screening for mental health concerns
h. Family history or genogram
6. Intervention, to include brief counseling and crisis intervention to clients on areas
identified in screening/assessment. Counseling issues may include: crisis intervention,
pregnancy and birth options, family planning methods, grief issUes, stress, parenting,
coping skills, problem solving, abuse/neglect, drug alcohol issues, family violence and
mood disorders.
7. Educate clientS toward informed choices regarding personal, child and family
health/mental health treatment.
8. Assist in empowering clients to advocate for themselves
9. Contractor will maintain records that contain comprehensive behavioral health
assessment information, clear, concise plans for care; treatment/follow up care and
progress toward outcomes. Application of legal mandates for reporting abuse/neglect
must be documented,
10. Participate in team meetings to provide behavioral health consultation and service
planning, aSsist with problem solving, team communication and interdisciplinary
decision-making and monitor activity of CHW related to psychosocial issues and
document.
11. Provide admjnistrators and policy makers with data regarding behavioral health needs of
the MSS client population; develop positive relationships with other provider
organizations serving the population and provide leadership in assuring that all providers
work collaboratively in addressing the needs of the MSS client population.
The Contractor must posses:
. Effective oral and written communication skills
. Ability to form and sustain effective relationships with clients, team members and
community providers
. Understanding of community systems
