Replacements
The County of Jefferson, State of Washington, desires to supplement the agreement entered into
with Tetra Tech, Inc. and executed on January 19, 2010 and identified as:
Tetra Tech - Upper Hoh Road
All provisions in the basic agreement remain in effect except as expressly modified. by this
supplement.
The changes to the agreement are described as follows:

Section II, SCOPE OF WORK, shall be amended by creating a new task for Alder Creek -- Task
No. 8, Construction Administration Assistance. The items of work that will be done under
Construction Administration Assistance are set forth in the attached Exhibit A - Alder Creek
Culvert, and by this reference made a part of this supplement.
There will be no change to the scope of work for Dismal Creek and Spruce Creek.

Section IV, TIME FOR BEGINNING AND COMPLETION, is amended to change the date for
completion of the work to read:
This contract will expire on January 30, 2014.

Section V, PAYMENT, shall be amended as follows:
For Alder Creek, Task No. 8, Construction Administration Assistance, will have a maximum
amount payable of $21,509.00. The new, total maximum amount payable for Spruce Creek will
be $182,759.00, as shown below. There will be no change to the maximum amount payable for
Dismal Creek and Spruce Creek.

EXHIBIT A
SUPPLEMENT NO.5 -- SCOPE OF WORK
ALDER CREEK TRIBUTARY CULVERT REPLACEMENT
INTRODUCTION
This supplement provides for additional professional services necessary to provide support
during construction for Jefferson County. The additional effort is described per major task
category below. Jefferson County will provide all construction inspection. One site visit by the
project's geotechnical engineer is budgeted to observe footing conditions as described below.
1. Assist County with final revisions to Contract Documents: The Consultant will assist
the county in performing minor revisions to the contract documents prior to bid
advertisement.
2. Review Shop Drawings and Culvert Calculations: The Consultant will perform
review of shop drawing and calculations for the precast concrete box culvert and footings.
The Special Provision reference is 6-02.3 Construction Requirements, Shop Drawings. A
limited time allocation is established to respond to questions.
3. Respond to Questions during Construction: The Consultant will assist the County in
answering questions that may arise during construction. It is anticipated that the majority
of these requests will be responded to by the County. A limited time allocation is
established for the Consultant to respond to questions when requested by the County.
4. Geotechnical Construction Support: Time is allocated for one site visit by the team's
geotechnical engineer, AMEC Environmental & Infrastructure, Inc., to observe footing
conditions and to provide recommendations for the placement of the precast concrete
footings.
