Statement of Issue: Public Works staff requests a modification of the scope of work and
budget in the basic agreement with Tetra Tech, Inc. to include additional services for Alder
Creek Tributary Culvert Replacement and Spruce Creek Culvert Replacement.
Analysis/Strategic Goals/Pro's 8: Con's: This supplement to the existing scope of work
between Jefferson County and Tetra Tech is for construction assistance services for Alder
Creek Tributary Culvert Replacement and Spruce Creek Culvert Replacement. These services
will include performing a bridge load rating for Alder Creek, and assisting the County with
project closeout on Spruce Creek.
Fiscal Impact/Cost Benefit Analysis: The Tetra Tech agreement covers a supplement of
$10,117.00 for the Alder Creek Tributary Culvert Replacement, and $10,338.00 for the Spruce
Creek Culvert Replacement. The maximum amount payable for the Tetra Tech agreement
wilt increase from $504,254.51 to $524,709.51. This work is funded at 86.5% by the Federal
Highway Administration and the remaining 13.5% through a combination of Rural Arterial
Program (RAP) and County Road funds.

Recommendation: The Board is requested to execute all three originals of the Supplemental
Agreement with Tetra Tech, Inc., and return two originals to Public Works.

Replacements
The County of Jefferson, State of Washington, desires to supplement the agreement entered into
with Tetra Tech, Inc. and executed on January 19, 2010 and identified as:
Tetra Tech -Upper Hoh Road
All provisions in the basic agreement remain in effect except as expressly modified by this
supplement.
The changes to the agreement are described as follows:

Section II, SCOPE OF WORK, shall be amended by adding work items to Alder Creek -- Task
No. 8, Construction Administration Assistance. The additional items of work that will be done
under Construction Administration Assistance are set forth in the attached Exhibit A, and by this
reference made a part of this supplement.
Section II, SCOPE OF WORK, shall also be amended by adding work items to Spruce Creek --
Task No. 8, Construction Administration Assistance. The additional items of work that will be
done under Construction Administration Assistance are set forth in the attached Exhibit A, and
by this reference made a part of this supplement.
There will be no change to the scope of work for Dismal Creek.

Section V, PAYMENT, shall be amended as follows:
For Alder Creek, Task No. 8, Construction Administration Assistance, will have a new
maximum amount payable of $31,626.00. The new, total maximum amount payable for Alder
Creek will be $192,876.00, as shown below.
For Spruce Creek, Task No. 8, Construction Administration Assistance, will have a new
maximum amount payable of $25,911.00. The new, total maximum amount payable for Spruce
Creek will be $183,888.00, as shown below.
There will be no change to the maximum amount payable for Dismal Creek.

EXHIBIT A
SUPPLEMENT N0.6 -- SCOPE OF WORK
ALDER CREEK TRIBUTARY CULVERT REPLACEMENT -- LOAD RATING
This supplement provides for additional professional services necessary to perform a load rating
on the Alder Creek Tributary as described below. The analyses will be based upon the
Contractor-provided culvert fabrication submittal and the design drawings.
1.5 Preliminary Load Rating Analysis
Following receipt and review of the culvert submittal from the Contractor, a load rating
will be performed and submitted to the County within 21 days of the receipt of the
submittal. The load rating will be done in accordance with the AASHTO Manual for
Bridge Evaluation, Second Edition 2011, and in accordance with Chapter 14 of the
WSDOT Bridge Design Manual. The Consultant shall provide a stamped load rating
report summarizing the controlling points for the Legal Vehicles and Overload Vehicles.
The load rating will check that the culvert structure has sufficient reserve capacity to rate
at least 1.00 for the HL-93 design vehicle and for the Legal Vehicles (Type 3, Type 3-52,
Type 3-3). In addition, the Rating values for the Notional Rating Load and WSDOT
Overload Vehicles (Overload 1 and Overload 2) will be checked for a load rating greater
than 1.00. The load rating report will include the following:
a. Rating summary cover sheet
b. LFR analysis for NBI Operating and Inventory
c. LRFR analysis for all legal and overload vehicles
1.6 Final Load Rating Confirmation
After project construction, the load rating will be updated to reflect changes made during
the construction that would alter the load rating. An updated load rating report will be
prepared and submitted to the County that incorporates comments received from the
preliminary load rating performed in Task 1.5.

rating analysis of the Spruce Creek Culvert.
