Statement of Issue:
The issue is requesting execution of the Supplemental Consultant Agreement #3 with
Integrity Structural Engineering, PLLC of Seattle, WA. for providing continuing
construction engineering support services for the Queets Bridge Painting, CR1881 project.
Analysis /Strategic Goals /Pro's l3 Con's:
This project consists of repainting the steel bridge girders and bracing members of the
Queets Bridge. The work includes minor girder web bracing, abrasive blasting, treatment
of pack rust, containment of wastes, and repainting with a primer and top coat paint
system. Additional construction engineering support services are necessary due to an
extended project close out phase.
This supplemental agreement will provide additional construction engineering support
services by Integrity Structural Engineering, PLLC, through the end of the construction
phase. The additional required engineering support work for this project consists of
Engineer of Record and technical Paint Expert services.
Fiscal Impact /Cost Benefit Analysis:
The maximum amount payable for consultant services under this Supplemental Consultant
Agreement #3 is $0.00. These consultant services will be eligible for reimbursement from
the Federal Highway Administration BHOS funding at the rate of 100 %. The Date of
Completion will be extended to December 31, 2014.

Recommendation:
The Board is asked to execute the three (3) originals of the Supplemental Consultant
Agreement #3 with Integrity Structural Engineering, PLLC, and return two (2) signed
originals to Public Works (attn. John Wayand).

The Local Agency of Jefferson County, Washington desires to supplement the agreement entered into
with IntegritV Structural Engineering, PPLC and executed on April 8, 2013 and identified as Agreement
No. Queets 2013 -2.
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:
Section 11, SCOPE OF WORK, is hereby amended to read:
No Chance.
II
Section IV, TIME FOR BEGINNING AND COMPLETION, is amended to change the number of
calendar days for completion of the work to read:
December 3 1 2014
III
Section V, PAYMENT, shall be amended as follows:
Original Agreement: $185,143.68 (April 8, 2013)
Supplemental Agreement #l: $146,817.65 (October 28, 2013)
Supplemental Agreement #2: $12,901.62 (February 10, 2014)
Supplemental AKreement 43: $0.00 (Reauested)
New Agreement Maximum Amount Payable: $344,862.95
