This permit was issued to correct a violation of WAC 246-272A for a drainfield installed without a permit and a leaking septic tank. The permit must be completed within 90 days of the date of issuance. 2.) H - Existing tank shall be properly abandoned. It shall be pumped and filled with clean fill. Documentation to be provided to Health Dept. prior to final. 3.) Contact designer prior to installation for staking of drainfield area. 4.) Health Dept. required to observe pressure test with system designer when system fully installed/complete, 48 hours notice to be provided for scheduling. 5.) Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS. 6.) A measuring tape was used to be sure that the well setback has been maintained during layout of the lower portion of the drainfield. All the permit conditions have been maintained. Orifices are at 3'spacing, decided during installation that the closer orifices would be better due to the trench . width being 4.5' instead of the standard 3'wide trenches. There was a water line that ran from the lower well to through the drainfield area this water line is now decommissioned and capped more than 10'from the laterals. Excavation of the septic tank showed a Lot Area :67872sf high effluent level,a corroded and plugged iron tightline to Area soils: Cassolary Sandy Loam,0-15%slopes the d-box. The d-box had corroded. The drainfield excavation revealed thin wall,cracked pvc pipe,variable depth of 30-36". The gravel around the pipe is limited and ,...._N mixed with soil. Since the pvc is not consistent with the time period of the home, it is assumed that it was installed without SEWAGE a permit,does not meet code and must be replaced.
