DISCLAIMER-This approval is for an on-site sewage system that meets the state and county standards in effect on the
date of application. This approval for an on-site sewage system DOES NOT assure you of any other County approvals. For
example,it DOES NOT GUARANTEE that you will later obtain permission to build a permanent residence or other structure
on this parcel. Any future application will be separately judged by the rules and laws in effect at that time. All construction
and development activities must comply with all permit conditions,state and local codes, and Recommended Standards and
Guidance documents in effect when the permit is issued.
The property owner is responsible for the accurate location of all property lines.Any removal of or major disturbance of soil in the
primary or reserve drainfield area may create site conditions that are unacceptable for the installation of a sewage disposal system.
Any change in drainfield or tank location may invalidate this permit unless prior approval is obtained from the Jefferson County
Environmental Health Division. If during excavation or development of the site an area of potential archeological significance is
uncovered, all activity in the immediate area shall be halted,and the UDC Administrator shall be notified at once.

SYSTEM IN JEFFERSON COUNTY, WASHINGTON
This permit is issued for a period of three years (unless otherwise stated above) in accordance with
Jefferson County Rules and Regulations for On-Site Sewage Systems, codified in JCC 8.15 as amended.
This permit may not be renewed.

1.) This system was installed w/o a permit. System was fully exposed and evaluated by a septic
system designer and inspected by county health staff. After review of adjacent soil logs the system
was found to be in compliance w/the current on-site waste water code.
This "permit" documents and as-builts the system that was installed
2.) Jefferson County has determined that the use of real property for agriculture and forestry
operations is a high priority and favored use in the county. The county will not consider to be a
nuisance those inconveniences or discomforts arising from such operations, if such operations are
consistent with commonly accepted best management practices in compliance with local, state,and
federal laws. If your real property includes or is within five hundred (500) feet of real property
designated as Rural Residential 1:10 or 1:20, Rural Industrial, Rural Commercial, Agriculture, or
Forestry, you may be subject to inconveniences or discomforts arising from such farming and
forestry operations, including but not limited to noise, tree removal, odors, flies, fumes, dust,
smoke, the operation of farm and forestry machinery during any 24-hour period, the storage and
disposal or manure, and the application of permitted fertilizers and permitted pesticides. One or
more of these inconveniences may occur as a result of agricultural and forestry operations which
are in conformance with existing laws and regulations.

Note: Original un-permitted installation c. 2006 by Mr. Don Benshoof, homeowner. On-site
consultation with Randy Marx on October 14, 2013, to evaluate soils and inspect installation. Soil
evaluation holes subsequently re-filled, distribution trenches restored (horizontal excavation into
undisturbed native soils, drain rock at least 6 inches thick, with horizontal 4-inch slotted PVC
distribution pipes, distribution trench covered with Class C un-woven geotextile filter fabric and 16
to 24 inches of soil and cover; two 4-inch monitoriong ports per lateral, installed down to
infiltrative surface).
Note: Plumbing removed from old cabin, reported by Don to Everett o 1 10, 2014. Cabin
removal occurring now.

Dear Mr. Marx,
Streamline Environmental, Inc. (Streamline) is pleased to submit this septic system design and
permit application for the referenced property, owned by Donald and Lisa Benshoof.
SUMMARY
The applicants wish to properly permit the conventional gravity-drain septic system installed at
the referenced property, by the applicants, without a permit. Upon thorough evaluation of the
site and installed septic system, Streamline found the system, as installed, designed in
conformance with current Washington Department of Health guidance. Design drawings
presented in this submittal are As-Builts.
SEPTIC SYSTEM DETAILS
Capacity Peak discharge rate: 240 gallon per day (gpd); average discharge capacity: 160 gpd
or less.
Parcel Land Area The parcel is over 219,000 square feet(SF) in area, about 5.03 acres.
Drinking Water Supply Private water well.

Nearby Water Wells No known wells are within 100 ft of the subject parcel.
Topography The site is relatively level in the developed portion, dropping off to moderate
slopes with grades typically ranging from 10 to 15 percent.
Surface Water and Drainage There are no perennial surface waters on the site, or within 100 ft
of it. There is a non-fish habitat stream with headwaters close to the southern boundary of the
subject parcel, flowing southward, away from the subject parcel.
Soil Evaluation Four soil evaluation holes (labeled S 1-S4 on site plan) excavated in August
2013 revealed shallow topsoil to <8 inches below ground surface, followed by coarse sand with
intermittent layers of medium sands, and gravel comprising less than 20%, to the full depth of
excavation, ranging down to 96 inches bgs in S1 and S2, shown on the attached Site Plan.
Groundwater was not encountered in any of the holes, and no indications of prolonged
saturation, or confining layers, were observed.
Environmentally-Sensitive Areas Jefferson County's online parcel mapping tool did not
indicate any environmentally-sensitive areas covering the subject parcel. An area with 'slight
landslide hazard' is located northeast of the subject parcel.
Vegetation The site is well vegetated with forest, shrubs, lawn and gardens. The area
surrounding the parcel is occupied by forest.
Drainfield Sizing With Soil Type 2 an application rate of 1.0 gal/day/ft2 (gal per day per square
ft) is allowed. With a peak flow of 240 gal/day,240 square ft of infiltrative surface is required.
Reserve Drainfield Area A proposed 1,300 SF reserve area is shown on the included Site Plan.
This area provides ample space for a 100%replacement drainfield.
Septic System Operations & Maintenance The applicant will record an active monitoring
agreement to the property title, prior to final approval of the system, in accordance with County
requirements. State law requires that this gravity-drain system will be inspected every three
years, with an evaluation completed by qualified personnel and recorded with the Jefferson
County Environmental Health Department. More frequent inspections of the system are
recommended, but do not need to be recorded. Also prior to final approval, Streamline
Environmental will provide the applicant an Operations & Maintenance Manual that specifically
describes this system and the best practices to be followed for maximum service lifetime.
