Dear Mr. Wold:
Let this letter suffice as "NOTICE OF PHYSICAL COMPLETION"for the above
referenced permitted projects and the sites are ready for final inspection by the
Department of Community Development.
Per our phone conversation on May 11, 2001, contaminated soil has been removed and
stockpiled on the Nylund Property site, Demolition Permit No. BDL01-00129 and this
soil is anticipated to be removed and disposed of sometime this month. Upon removal of
the contaminated soil the stockpile site and roadway access to the stockpile site shall be
seeded and mulched.

Dear Ms. Atkins:
Let this letter suffice as "NOTICE OF PHYSICAL COMPLETION"for the above
referenced permitted projects.
The well water casing at address site 484 E. Quilcene Rd. has been decommissioned and
backfilled with concrete per WAC 173-160-381 (b). Attached is a copy of the well water
report, Environmental Health Division Permit Receipt No. 27852, and an invoice for concrete
to fill the well casing hole.
Septic tanks at address sites, 482 E. Quilcene Rd. and 484 E. Quilcene Rd., have been
pumped, removed, and the void filled with pit run graved per your letter dated 12/5/01 and
WAC 246-272-18501. The tank located at 484 E. Quilcene Rd. was concrete and the tank
located at 482 E. Quilcene Rd. consisted of a round, open top, metal container approximately 8
ft. diameter and 3 ft. depth. Attached are copies of invoices for septic pumping and disposal
of the concrete tank. The metal tank was disposed of with other demolished materials
