STATEMENT OF ISSUE:
Jefferson County Public Health, Environmental Health Division, requests Board approval of Amendment II
DOE Grant #G1400361, Education Grant; July 1, 2014 —June 30, 2015; [$5,000.00].
ANALYSIS /STRATEGIC GOALS /PRO'S and CON'S:
The purpose of this Amendment is to decrease the state grant share, Department of Ecology, Coordinated
Prevention Grant for education activities by $3,750. Specifically the reduction in funding applies to Task 2,
Moderate Risk Waste (MRW), which is the provision of on -site technical assistance and education to
businesses regarding use of hazardous materials reduction and waste management. The Amendment
reduces overall eligible costs by $5,000, which results in state grant share reduction of $3,750. The state
funds the Grant at 75% with a match requirement of 25 %; the match is reduced by $1,250.00. Grant total
of $148,333.33 is reduced to $143,333.33. Additionally, the Amendment states education and outreach
activities may include additional activities and purchases.
FISCAL IMPACT /COST BENEFIT ANALYSIS:
This is a reduction in funding to the Consolidated Prevention Grant, Education. Reductions are based on
Recipient's ability to perform Task 2 for less money.
RECOMMENDATION:
JCPH management requests approval of Coordinated Prevention Grant (Education) DOE, Amendment II for a
reduction in total funding by [$5,000.00]; July 1, 2014 —June 30, 2015; [$5,000.00].

AMENDMENT NO. 2 TO GRANT AGREEMENT NO. G1400361
BETWEEN THE
STATE OF WASHINGTON DEPARTMENT OF ECOLOGY
AND
JEFFERSON COUNTY PUBLIC HEALTH
Amendment Purpose: To amend the grant agreement between the State of Washington, Department of
Ecology, hereinafter referred to as the "DEPARTMENT" or "ECOLOGY" and Jefferson County Public
Health, hereinafter referred to as the "RECIPIENT ".
WHEREAS, this amendment will decrease the state grant share of Task 2 of this agreement because the
RECIPIENT is able to complete the work for less money.
WHERAS, this amendment will clarify activity in Task 1 of this agreement to allow specific costs for
reimbursement.
IT IS MUTUALLY AGREED that the agreement is amended as follows:
1. The total maximum eligible cost for this agreement decreases by $5,000 from $148,333.33 to $143,333.33.
The state grant share for this agreement decreases by $3,750 from $111,250 to $107,500.
2. The scope of work and expected outcomes 2 remains the same. The scope of work and expected outcomes
for Task 1 can include the following:
RECIPIENT will conduct education and outreach activities through targeted campaigns and school -based
waste reduction and recycling programs that may include reducing organics waste. The RECIPIENT may
include workshops and bin purchases to this task and should estimate the pounds of food waste diverted.

4. The effective date of this amendment is July 1, 2014.
All other terms and conditions of the original Agreement including any Amendments remain in full force and
effect, except as expressly provided by this Amendment.
This Amendment is signed by persons who represent that they have the authority to execute this
Amendment and bind their respective organizations to this Amendment.
