L ATKINS, Jefferson County Environmental Health Specialist The property owner is responsible for the accurate location of all property lines.Any removal of or major disturbance of soil in the primary or reserve drainfield area may create site conditions that are unacceptable for the installation of a sewage disposal system.Any change in drainfield or tank location may invalidate this permit unless prior approval is obtained from the Jefferson County Environmental Health Division. If during excavation or development of the site an area of potential archeological significance is uncovered, all activity in the immediate area shall be halted, and the UDC Administrator shall be notified at once. HEALTH DEPARTMENT MUST BE CALLED FOR FINAL INSPECTION. A minimum of 12' must be provided between edges of the primary laterals for placement of the reserve area between the laterals. 2.) Approval/issuance of a sewage disposal permit does not guarantee the approval of other development or a building permit on this site. Compliance with other Jefferson County and Washington State Codes is required. 3.) AS PER WAC 246-272 AND JEFFERSON COUNTY CODE 8.15 ALL SYSTEMS REQUIRE MONITORING. THIS MONITORING IS PROVIDED BY AGREEMENT BETWEEN THE JEFFERSON COUNTY ENVIRONMENTAL HEALTH DIVISION AND THE JEFFERSON COUNTY PUBLIC UTILITY DISTRICT#1. THIS SEWAGE DISPOSAL SYSTEM WILL REQUIRE AN ACTIVE MONITORING CONTRACT WITH THE PUD#1 PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL SYSTEM. While the County strongly recommends that the property owner allow inspectors to enter their property to inspect the system in order to insure that the system is functioning correctly, the property owner always has the right under the Fourth Amendment and our State Constitution, to refuse the inspectors access to their property. The property owner never loses the right to refuse access and can do so despite never having refused access before. 4.) As per Jefferson County Code 8.15 all onsite sewage systems require monitoring. Monitoring shall be provided by an approved entity. THIS SYSTEM IS REQUIRED TO BE INSPECTED AT 6 MONTHS, AT ONE YEAR AND ANNUALLY THEREAFTER. 5.) Health Dept. required to observe pressure test when system fully installed/complete, 48 hours notice to be provided for scheduling. 6.) An asbuilt drawing and certification of completion by the designer is required prior to final approval. 7.) 10'separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components. 8.) MAXIMUM TRENCH DEPTH 12 INCHES 9.) All components of the septic system are to be completely protected from vehicular traffic or mechanical disturbance. 10.) Permanent barriers are required along/around primary and reserve drainfield areas to protect from parking, driving, and other land disturbing activities prior to final. 11.) Divert all sources of drainage, including roof drains away from septic tank and drainfield area. 12.) Approval of this sewage disposal permit does not preclude the permit holder from complying with the Unified Development Code for other/future development on the site. 13.) The application was reviewed by the Jefferson County staff for the potential presence of Environmentally Sensitive Areas (ESAs) under the provisions of the Unified Development Code (UDC). Sewage Pump: The pumps shall be fully submersible effluent style ejector pumps. Effluent style ejector pumps shall be capable of passing 0.75" diameter solids and have a 1.5"minimum discharge. High head well style pumps may be used. Well style pumps shall have a one-inch minimum discharge. Any pump shall be UL listed for use as a sewage pump. Submittal of manufacturer's specification sheet including pump curve is required. 10. Electrical Controls: Verify electrical requirements for splicing, ventilation, etc. with local electrical inspector prior to ordering parts and installation. Components shall be mounted in a NEMA 4X rated weather tight enclosure that meets NEC code standards and the requirements of the Washington State Department of Labor and Industries (L&I), and be listed by Underwriters Laboratories. System is designed to function with a programmable timer to regulate the length of the dose and the interval between doses to the sand filter. Timers shall be capable of a repeatable cycle from 10 seconds to 10 hours with separate variable controls for the"on" and"off' times. The float system to pump to the drainfield is float switch activated, and no timer is required. In addition to the timer,provide the following features/options for the pump control panel: 1. Manual ON/Off and Automatic modes (HOA), with timer actuating ON/OFF float. 2. Audible and visual high water alarm and low water alarm/redundant off shall be provided for the pump chamber and sandfilter pumping well. Alarm circuit shall be wired separately from the pump circuit. 3. Dose counters and run time meters on both pumps. 4. Float switches must be mounted independent of the pump and fitting to facilitate replacement and/or adjustments without removing the pump. 5. Pump override which shuts off the pump in the tank if a high water alarm occurs in the sand filter basin. Discharge assembly shall contain check and ball valves and union to facilitate removal of the pump unit from the pump tank. Flexible coupling shall be required where recommended by the manufacturer to negate torque effects. Septic Tank Riser/Lid: Concrete riser rings or leveling brick together with locking ring and cover or fiberglass lid may be an acceptable alternate (Engineer approval required)to the'Orenco'riser and bolt down lid. Provide 24" diameter access risers to grade over the inlet and outlet of septic tank. Water and gas tight seals are required. 12. Pump Tank Riser/Lid: Concrete riser rings or leveling brick together with locking ring and cover or fiberglass lid may be an acceptable alternate (Engineer approval required) to the 'Orenco' riser and bolt down lid specified in the details. Splice box suitable to L&I shall be attached firmly to the riser for connection of the pumps and control leads. OTHER UTILITIES: In addition to utilities shown on the plans, other utilities may exist on site. Contractor shall be responsible for any damage to utilities,whether shown or not. OPERATION AND MAINTENANCE: The operation and maintenance of the system will be an ongoing function of the owner. System principals and maintenance procedures are outlined in the "Users Manual" provided with the project. WHERE SEPTIC SYSTEM IS LOCATED: Parcel#: 931300007 LEGAL DESCRIPTION(S): Section: 2 Township 29N Range 1W ANDERSON COURT LONG PLAT LOT 7 ADDRESS OF RESIDENCE SERVED(if assigned) 52 MEGS WAY and, WHEREAS,a sewage collection,treatment and disposal system,as described in case#SEP05-00008 has been installed thereon,and WHEREAS,the residence or facility at the above described property,including all parcels associated with said case, utilizes an onsite sewage system which requires regular maintenance and monitoring for the life of the system. Monitoring is required to be performed by a person authorized by the Public Health Department,and WHEREAS,the Owner is required by Jefferson County Public Health Department,as per WAC 246-272A through Jefferson County Code 8.15 to implement a program for periodic monitoring to assure the continuous and satisfactory operation and maintenance of the sewage system,and NOW THEREFORE,in consideration of the premises and the mutual promises and covenants hereafter set forth,the parties hereto agree as follows: 1. Construction: The Owner has constructed and installed a sewage collection,treatment and disposal system together with appurtenant facilities whose capacity and design are satisfactory to the Jefferson County Public Health Department. The plans and specifications for this construction have been approved by the Jefferson County Public Health Department prior to construction. 2. Owners responsibilities: a. Maintain all components as accessible and unobstructed to the surface.Non-accessible or obstructed components will not be serviced and may result in a violation of permit requirements. b. Utilize the system consistent with producing residential waste strength effluent. c. Operate and maintain the system in accordance with conditions of the permit and Jefferson County Code 8.15. Schedule: The Owner shall have their sewage system inspected by a person authorized by Jefferson County Public Health in accordance with the schedule set forth in Jefferson County Code as adopted and hereinafter amended. For some proprietary products registered with the State Department of Health,the inspector must also be authorized by the device manufacturer. 5. Reminder: The Jefferson County Public Health Department shall provide a reminder of required monitoring. The lack of receipt of said reminder does not relieve the property owner from compliance with the monitoring schedule pursuant to Jefferson County Code. 6. Records: A report of each inspection conducted shall be submitted to the Jefferson County Public Health Department within 30 days of completion of the inspection. Such records shall be open to the inspection of the Owner. 7. Liability: The Grantor agrees to relieve Grantee,its agencies,divisions,officers, agents,and employees of any liability,damages,judgments,claims or other expenses that might arise as a result of the inspections required by this agreement. 8. Fees: A fee as set forth in the Jefferson County Fee schedule and adopted by the Board of County Commissioners shall be paid upon submittal of each inspection report. 9. Terms: This agreement shall remain in full force and effect until the date of notification to the Owner by the Jefferson County Public Health Department that the Owner's Sewer System has been intercepted and connected to a general sewer system. 10. Amendments: This agreement shall not be modified except with the express written approval of the Environmental Health Department of Jefferson County. 11. Penalties: Non-compliance with monitoring and inspection requirements is a violation of Jefferson County Code and shall be subject to enforcement as described in said codes. The County shall have the right to enforce and compel compliance with this covenant, including all remedies available at law and equity. The County shall have the right in any such proceeding to recover its attorney fees and costs. 12. Venues: Venue for any action hereunder shall be in Jefferson County,Washington. If any term or provision of this agreement is in whole or part held invalid or unenforceable by any court of competent jurisdiction,the remainder of this agreement shall continue in full force and effect. 13.
