THE DEPARTMENT WILL NOT GIVE FINAL APPROVAL FOR THE PROJECT UNTIL THE AGREEMENT HAS BEEN SIGNED BY THE PROPONENT AND FILED WITH THE JEFFERSON COUNTY AUDITOR. 5.) THE DEPARTMENT OF PUBLIC WORK'S FEE FOR REVIEWING STORMWATER SITE PLANS AND CONSTRUCTION STORMWATER POLLUTION PREVENTION PLANS AND INSPECTING IMPLEMENTATION OF BEST MANAGEMENT PRACTICES IS $62 PER HOUR. FEES MUST BE PAID PRIOR TO FINAL PROJECT APPROVAL. 6.) TO IMPLEMENT THE INTENT OF LNG 19.0 OF THE COMPREHENSIVE PLAN TO PROTECT THE FOREST CORRIDOR AND TREE CANOPY IN THE GLEN COVE AREA, THE SETBACK FROM THE RIGHT-OF-WAY OF SR 20 SHALL BE 50 FEET ON EACH SIDE OF THE HIGHWAY(COMPRISED OF A 30-FOOT BUFFER AND A 20-FOOT SETBACK FROM THE BUFFER), FOR NEW DEVELOPMENT, FROM THE INTERSECTION OF OLD FORT TOWNSEND ROAD AND SR 20 TO THE INCORPORATED BOUNDARY OF THE CITY OF PORT TOWNSEND. 7.) 10' SEPARATION REQUIRED BETWEEN A WATER LINE AND ALL PORTIONS OF THE ONSITE SEWAGE SYSTEM; EFFLUENT TRANSPORT LINE, TANKS, TREATMENT AND DISPOSAL COMPONENTS. 8.) Maximum lot coverage is not to exceed 55%. Lot coverage is defined as amount of impervious surface which includes roof tops, driveways, concrete, etc. 9.) Minimum setback from North Otto Street right-of-way shall be 20 feet. Minimum side yard setbacks shall be 10 feet. Minimum setback from SR 20 right-of-way shall be 50 feet. 10.) Loading and unloading must occur on site (off-street). 11.) Exterior lighting for commercial and industrial uses shall not exceed thirty feet(30') in height from the finished grade. Exterior lighting for residential uses shall not exceed twenty feet(20') in height from the finished grade, excepting when such lighting is an integral part of a building or structure. Ground level lighting is encouraged. 12.) Lighting fixtures shall be designed and hooded to prevent the light source from being directly visible from outside the boundaries of the property. The intensity or brightness of all lighting, during construction and after project completion shall not adversely affect the use of surrounding properties or adjoining rights-of-way. 13.) The building height is not to exceed 35 feet. 14.) The applicant and/or representative shall contact the Development Review Division of the Permit Center (360-379-4450) regarding a final inspection for compliance of zoning permit conditions prior to scheduling a final building inspection. 15.) Commercial and industrial developments located in all zoning districts hall screen from areas frequented by the general public, all materials stored outside buildings or structures which are not for display or immediate sale. Areas frequented by the general public shall be defined as public streets and rights-of-way, and other areas normally used by the general public located adjacent to the subject property. 16.) The storage and handling of inflammable liquids, liquified petroleum, gases and explosives shall comply with rules and regulations falling under the jurisdiction of the fire marshal, the laws of the state and other local ordinances. Bulk storage of inflammable liquids below ground shall be located no closer to the property line than the greatest dimension (diameter, length, or height) of the tank. 17.) All noise emitting machinery that cannot in any other way operate below the maximum permitted noise level shall be housed in a sound-attenuating structure. 18.) The intensity of sound emitted by any commercial or industrial activity shall not exceed levels estabished by the Washington State Department of Ecology under WAC 173-60, and by Jefferson County under Resolution No. 67-85, "Establishment of Environmental Designations for Noise Abatement Areas for Jefferson County." 19.) Signs shall be reviewed administratively by the Department of Development Review. An administrative determination to approve a sign application shall be made when the proposal is consistent with the goals, policies, and provisions of the Comprehensive Plan, and/or applicable community development plan, and this ordinance. 20.) Exterior flashing, moving, or blinking lighting is prohibited, except as required by local, state, or federal standards. 21.) Off street parking shall be provided in accordance with the minimum standards set forth in Tables 6-2 &6-3 of the Jefferson County Code or one (1) space per employee and one (1) space per 300 square feet of any associated retail sales area. No retail sales is proposed for the project. (The submitted site plan identifies two (2) parking spaces and one (1) handicap parking space). No less than 2% of the total number of parking spaces or a minimum or one (1) space, whichever is greater, shall be designated handicap parking and be no less than 12' 6"wide. 22.) All parking areas shall be provided with an all-weather surface. 23.) Parking requirements are based on a formula of one space per employee and one space per 300 square feet of associated retail. Additional parking to meet physically handicapped needs shall be provided at a rate of not less than 2% of the total number of parking spaces, or a minimum of one parking space. 24.) Wheel stops, striping, or similar measures are required where a parked vehicle would encroach on adjacent property, pedestrian access or circulation areas, rights-of-way, or landscaped areas. 25.) Parking for physically handicapped needs shall be provided consistent with state standards at a rate of not less than two percent of the total number of parking spaces, or a minimum of one parking space, whichever greater. Parking spaces for physically handicapped needs shall comply with ANSI 117.1-2003 (502.2 and 502.4.2). Such spaces shall be not less than 12 feet, six inches wide. 26.) Owners of two or more adjoining uses, structures or lots may utilize jointly the same parking or loading area when the hours of operation do not overlap. In the event that owners of one or more adjoining uses, structures or lots desire to utilize jointly the same parking concurrently, the total requirement for parking spaces shall be the sum of the requirements for each individual use, unless the applicants can demonstrate to the Administrator that a lower number of parking spaces are justified through implementation of transportation demand management strategies, off-peak use, availability and use of public transit or alternative modes of travel or other measures. 27.) An average width of ten (10) feet of Screen-B landscaping shall be provided along N Otto St. Screen-B landscaping is a "filtered screen" that functions as a visual separator. This landscaping is a mix of evergreen and deciduous trees and shrubs generally interspersed to create a filtered screen that grows to at least eight feet in height within two growing seasons. 28.) All landscaping and necessary support systems shall be maintained for the life of the project. All landscape materials shall be pruned and trimmed as necessary to maintain a healthy growing condition. Landscape areas shall be kept free of trash. 29.) The required landscaping shall be installed prior to project occupancy. However, a temporary certificate of occupancy may be issued prior to landscape installation at the written request of the owner that identifies a time line for plant installation. FINDINGS: 1.) The Administrator finds that this application complies with applicable provisions of the Unified Development Code,all other applicable ordinances and regulations, and is consistent with the Jefferson County Comprehensive Plan and Land Use map. 2.) The application was reviewed by the Jefferson County Department of Community Development staff on September 10, 2008 for the potential presence of Environmentally Sensitive Areas (ESAs) under the provisions of the Unified Development Code (UDC). After an initial Geographic Information Systems mapping review and an investigative site inspection, the following ESAs were confirmed to be present on the subject property: SARPA& SUSC aquifer recharge area. 3.) Aquifer Recharge Areas in Jefferson County are characterized by porous geological formations that allow percolation of the surface water into the soils and the underlying zone of saturation. Aquifers are geologic formations that contain sufficient saturated permeable material to yield significant quantities of water to wells and springs. Aquifers serve as the source of drinking water within most of the rural portions of Jefferson County. 4.) Susceptible Aquifer Recharge Areas are those with geologic and hydrologic conditions that promote rapid infiltration of recharge waters to groundwater aquifers. 5.) Critical Aquifer Recharge Areas are defined as Seawater Intrusion Areas, and Special Aquifer Recharge Protection Areas and the following land uses located within Susceptible Aquifer Recharge Areas (1)All Industrial Land Uses (2)All Commercial Uses (3)All Rural Residential Land Uses requiring a Discretionary Use or Conditional Use Permit(4)All Rural Residential Land Uses with nonconforming uses that would otherwise require a Discretionary Use or Conditional Use Permit (5) Unsewered Planned Rural Residential Developments (6) Unsewered residential development with gross densities greater than one unit per acre. All of the above listed uses shall be subject to the applicable requirements and standards of the Jefferson County Unified Development Code. 6.) The applicant is proposing to create or add 4959 square feet of impervious surface and 4959 square feet of land disturbing activities. 7.) Jefferson County determined that this proposal is categorically exempt from review under the State Environmental Policy Act(SEPA) pursuant to WAC 197-11-800(1)(c)(iii). 8.) The revised site plan dated August 29, 2008 has been reviewed for consistency under the UDC, and has been approved by Jefferson County Department of Community Development. Any modifications, changes, and/or additions to the stamped, approved site plan dated September 10, 2008 shall be resubmitted for review and approval by Jefferson County Department of Community Development. 9.) This approval is for a 4959 sqare foot warehouse and parking and stormwater facilities only. Any future permits on this site are subject to review for consistency with applicable codes and ordinances and does not preclude review and conditions which may be placed on future permits. 10.) The parcel has been designated as LI/C under the Jefferson County Comprehensive Land Use Map effective August 28, 1998. 11.) A Declaration of Restrictive Covenant has been recorded on June 24, 2007 under AFN 523790 for the purpose of consolidating the lots to meet minimum land area requirements for on-site septic and to allow zero lot line setbacks. The Covenant is strictly for building and septic purposes only. 12.) A stormwater permit subject to SEPA(BLD07-0065) to disturb 152,460 square feet of land was approved on June 1, 2007. A pre-application conference (PRE07-0020) was held on June 11, 2007 for construction of a 10,000 square foot warehouse. A road approach permit(RAP07-0079)was finaled on December 11, 2007. A septic permit(SEP07-0159) was finaled on December 19, 2007. A building permit (BLD07-0310) for a 10,000 square foot warehouse was finaled on March 25, 2008. NOTICE: This permit does not excuse the proponent from complying with other local,state, and federal ordinances, regulations,or statutes applicable to the proposed development. Development pursuant to this permit shall be undertaken subject to the applicable development and performance standards of the Jefferson County Unified Development Code. If during excavation or development of the site an area of potential archaeological significance is uncovered, all activity in the immediate area shall be halted, and the Administrator shall be notified at once. The Federal Endangered Species Act rules to protect threatened Chinook and Summer-run Chum salmon became effective on January 8, 2001. Bull trout have been listed as threatened since early 2000. Under the ESA,any person may bring lawsuit against any individual or agency that"takes"listed species (defined as causing harm,harassing,or damaging habitat for the listed species). In addition,the National Marine Fisheries Service can levy penalties. All areas in Jefferson County are induded as"critical habitat"for a listed species. Development of property along any marine shoreline,freshwater shoreline,or floodplains could harm habitat if protective measures are not taken. To minimize the potential to damage habitat,all property owners developing adjacent to marine shoreline,freshwater shoreline, or floodplains are advised to do the following: -Set back buildings,utilities and roads as far as possible from surface waters (streams, rivers,lakes, marine waters),or at least 150 feet from the edge of the water -All development activities should avoid unstable slopes,wetlands,and forested areas near surface waters -Remove minimal vegetation for site development,especially large trees -Allow trees that have fallen into surface waters to remain there -Infiltrate stormwater from buildings and driveways onsite through drywells rather than discharging directly into surface waters or roadside ditches Any individual, group,or agency can bring suit for a listed species"taking,"even if you are in compliance with Jefferson County development codes. The applicant is proposing to create or add 4959 square feet of impervious surface and 4959 square feet of land disturbing activities. 7.) A Declaration of Restrictive Covenant has been recorded on June 24, 2007 under AFN 523790 for the purpose of consolidating the lots to meet minimum land area requirements for on-site septic and to allow zero lot line setbacks. The Covenant is strictly for building and septic purposes only. 12.) A stormwater permit subject to SEPA(BLD07-0065) to disturb 152,460 square feet of land was approved on June 1, 2007. A pre-application conference (PRE07-0020)was held on June 11, 2007 for construction of a 10,000 square foot warehouse. A road approach permit (RAP07-0079)was finaled on December 11, 2007. A septic permit(SEP07-0159)was finaled on December 19, 2007. A building permit(BLD07-0310) for a 10,000 square foot warehouse was finaled on March 25, 2008. NOTICE: This permit does not excuse the proponent from complying with other local,state, and federal ordinances, regulations,or statutes applicable to the proposed development. Development pursuant to this permit shall be undertaken subject to the applicable development and performance standards of the Jefferson County Unified Development Code. If during excavation or development of the site an area of potential archaeological significance is uncovered, all activity in the immediate area shall be halted,and the Administrator shall be notified at once. The Federal Endangered Species Act rules to protect threatened Chinook and Summer-run Chum salmon became effective on January 8, 2001. Bull trout have been listed as threatened since early 2000. There is a shared access constructed with pervious asphalt. Jefferson County Public Works Department staff inspected the infiltration system prior to backfilling. • The Soil Survey for Jefferson County depicts the soils on site as loamy sand. • The proponent has submitted a Jefferson County stormwater management template dated 6/18/2008 that discusses construction stormwater pollution prevention and permanent stormwater management best management practices(BMPs). • The proponent proposes to infiltrate runoff from the warehouse in a 250 square foot infiltration trench with a design identical to the previous development. The design is in conformance with design criteria from the Kitsap County Stormwater Management Manual adapted to the Port Townsend rainfall regime. The application was reviewed by the Jefferson County Department of Community Development staff on March 27, 2007 for the potential presence of Environmentally Sensitive Areas (ESAs) under the provisions of the Unified Development Code(UDC). After an initial Geographic Information Systems mapping review and an investigative site inspection, the following ESAs were confirmed to be present on the subject property: SARPA&SUSC aquifer recharge area. 2.) Aquifer Recharge Areas in Jefferson County are characterized by porous geological formations that allow percolation of the surface water into the soils and the underlying zone of saturation. Aquifers are geologic formations that contain sufficient saturated permeable material to yield significant quantities of water to wells and springs. Aquifers serve as the source of drinking water within most of the rural portions of Jefferson County. 3.) Susceptible Aquifer Recharge Areas are those with geologic and hydrologic conditions that promote rapid infiltration of recharge waters to groundwater aquifers. 4.) Critical Aquifer Recharge Areas are defined as Seawater Intrusion Areas, and Special Aquifer Recharge Protection Areas and the following land uses located within Susceptible Aquifer Recharge Areas(1)All Industrial Land Uses (2) All Commercial Uses (3)All Rural Residential Land Uses requiring a Discretionary Use or Conditional Use Permit(4) All Rural Residential Land Uses with nonconforming uses that would otherwise require a Discretionary Use or Conditional Use Permit(5) Unsewered Planned Rural Residential Developments (6) Unsewered residential development with gross densities greater than one unit per acre. All of the above listed uses shall be subject to the applicable requirements and standards of the Jefferson County Unified Development Code. 5.) Critical Aquifer Recharge Areas may require special protection measures to mitigate water quality degradation. The submitted proposal does not require additional aquifer protection measures. However, during construction the project shall follow the Best Management Practices (BMPs)and facility design standards as identified and defined in the Stormwater Management Manual for the Puget Sound Basin. 6.) Summary of Significant Environmental Comments: EARTH: The proposal includes approximately 152,460 square feet of land disturbing activity. The applicant has submitted a preliminary drainage plan. The JCC requires proponents for developments that meet these criteria to implement a Stormwater Site Plan that includes a Large Parcel Erosion and Sediment Control Plan and a Permanent Stormwater Quality Control Plan. Based on the applicant's compliance with an approved Stormwater Site Plan that was reviewed by the Jefferson County Department of Public Works, the Department recommends finding that the proposal is not likely to result in significant adverse impacts related to erosion. WATER: Surface: County GIS maps do not identify any surface waters (wetlands, streams, marine waters)on or near the subject parcel. Groundwater: An Environmentally Sensitive Areas (ESA)check was conducted and the site is identified as a SARPA & SUSC aquifer recharge area. At such time of permit application, the specific use will be reviewed for consistency with JCC Section 18.15.240. Water Run-Off(including stormwater): Washington State Department of Ecology(DOE) commented on the application stating that erosion control measures must be in place prior to any clearing, grading, or construction and must be effective to prevent stormwater runoff from carrying soil and other pollutants into surface water or storm drains that lead to waters of the state. Proper disposal of construction debris must be on land in such a manner that debris cannot enter the natural stormwater drainage system or cause water quality degradation of state waters. The proposal involves clearing and grading on 3.5 acres (152,460 square feet). The site is described as flat to hilly, indicating that there is potential for pre-project offsite discharge of stormwater. If there is potential discharge of stormwater, a Construction Stormwater National Pollution Discharge Elimination System (NPDES) permit will be required from DOE. The permit must be obtained before any clearing or grading or land disturbing activities. The proponent is responsible for contacting DOE should the project fit the above criteria. Based upon the above findings, the SEPA Responsible Official issues a Determination of Non-Significance (DNS). 15.) Appeals of Type I permit decision must be in writing, conform with the procedures for appeal as set forth in UDC Section 8.5.2 and be filed within twenty-one(21)calendar days of the date of the issuance of the land use decision, as defined by the Land Use Petition Act, Chapter 36.70C.040(4), unless another time period is established by superseding state law or local ordinance. All appellants must timely exhaust all administrative remedies prior to filing a judicial appeal. Notice of appeal and any other pleadings required to be filed with the court shall be served by delivery to the County Auditor(see RCW 4.28.080), and all persons identified in RCW 36.70C.040, within the applicable time period. This requirement is jurisdictional. The notice of appeal shall contain a concise statement identifying: i.The decision being appealed and the identification of the application which is the subject of the appeal; ii. Any future permits on this site are subject to review for consistency with applicable codes and ordinances and does not preclude review and conditions which may be placed on future permits. 18.) The application was reviewed by the Jefferson County Department of Community Development staff on June 14, 2007 for the potential presence of Environmentally Sensitive Areas (ESAs) under the provisions of the Unified Development Code (UDC). After an initial Geographic Information Systems mapping review and an investigative site inspection, the following ESAs were confirmed to be present on the subject property: SARPA/SUSC aquifer recharge area. 2.) Aquifer Recharge Areas in Jefferson County are characterized by porous geological formations that allow percolation of the surface water into the soils and the underlying zone of saturation. Aquifers are geologic formations that contain sufficient saturated permeable material to yield significant quantities of water to wells and springs. Aquifers serve as the source of drinking water within most of the rural portions of Jefferson County. 3.) Susceptible Aquifer Recharge Areas are those with geologic and hydrologic conditions that promote rapid infiltration of recharge waters to groundwater aquifers. 4.) Critical Aquifer Recharge Areas are defined as Seawater Intrusion Areas, and Special Aquifer Recharge Protection Areas and the following land uses located within Susceptible Aquifer Recharge Areas (1)All Industrial Land Uses (2) All Commercial Uses (3)All Rural Residential Land Uses requiring a Discretionary Use or Conditional Use Permit(4) All Rural Residential Land Uses with nonconforming uses that would otherwise require a Discretionary Use or Conditional Use Permit(5) Unsewered Planned Rural Residential Developments (6) Unsewered residential development with gross densities greater than one unit per acre. All of the above listed uses shall be subject to the applicable requirements and standards of the Jefferson County Unified Development Code. 5.) The applicant is proposing to create or add 12,800 square feet of impervious surface and 26,000 square feet of land disturbing activities. 6.) THE PROPONENT SHALL SUBMIT A JEFFERSON COUNTY STORMWATER MANAGEMENT TEMPLATE AND CONSTRUCTION PLANS FOR THE INFILTRATION SYSTEM AND PERVIOUS PAVEMENT TO THE DEPARTMENT OF PUBLIC WORKS FOR REVIEW AND APPROVAL PRIOR TO CONSTRUCTION. 7.) THE DESIGN OF THE INFILTRATION TRENCHES SHALL CONFORM TO THE CRITERIA FROM THE KITSAP COUNTY STORMWATER MANAGEMENT MANUAL ADOPTED TO THE PORT TOWNSEND RAINFALL REGIME. 8.) DESIGN, CONSTRUCTION, AND MAINTENANCE OF THE PERVIOUS PAVEMENT SHALL CONFORM TO THE STANDARDS OF THE LOW IMPACT DEVELOPMENT TECHNICAL GUIDANCE MANUAL (PUGET SOUND ACTION TEAM, 2005), PARTICULARLY SECTION 6.3 PERMEABLE PAVING AND SECTION 7.1 PERMEABLE PAVEMENTS. THE PERVIOUS PAVEMENT SPECIFICATIONS SHALL INCLUDE SUBGRADE PREPARATION, BASE COURSE MATERIALS AND PLACEMENT, PLACEMENT OF GEOTEXTILE FABRIC, AND PAVEMENT MIX DESIGN AND PLACEMENT. THE PROPONENT SHALL EITHER DEMONSTRATE THAT THERE WILL BE NO SIGNIFICANT STORMWATER RUNOFF FROM THE PERVIOUS PAVEMENT OR ADDITIONAL STORMWATER MANAGEMENT FACILITIES SHALL BE REQUIRED. 9.) PRIOR TO CONDUCTING PERVIOUS PAVING OPERATIONS, THE PROPONENT SHALL HOLD A PRE-CONSTRUCTION CONFERENCE WITH THE PAVING CONTRACTOR, PAVEMENT SUPPLIER, AND PUBLIC WORKS DEPARTMENT. 10.) A LICENSED ENGINEER OR QUALIFIED TECHNICIAN SHALL BE ON SITE WHILE PAVING IS BEING CONDUCTED AND SHALL CERTIFY THAT PAVING OPERATIONS WERE CONDUCTED IN CONFORMANCE WITH THE APPROVED PLANS. 11.) THE PAVEMENT SUPPLIER SHALL CERTIFY THAT THE CONCRETE MIX MEETS THE PLAN SPECIFICATIONS. 12.) THE PROPONENT AND SUBSEQUENT OCCUPANTS SHALL BE RESPONSIBLE FOR ENSURING THAT MATERIALS USED AND STORED ON THE SITE DO NOT CONTAMINATE STORMWATER RUNOFF. THIS MAY REQUIRE, AS APPROPRIATE, ADDITIONAL BEST MANAGEMENT PRACTICES SUCH AS COVERED STORAGE, CONTAINMENT STRUCTURES, AND/OR A SPILL RESPONSE AND CLEAN-UP PLAN. 13.) IN ORDER TO ENSURE THAT THE APPROVED STORMWATER MANAGEMENT FACILITIES ARE APPROPRIATELY MAINTAINED FOR THE LIFE OF THE PROJECT, THE PROPONENT SHALL ENTER INTO A STORMWATER MANAGEMENT FACILITY MAINTENANCE AGREEMENT WITH JEFFERSON COUNTY. PRIOR TO FINAL APPROVAL OF THE PROJECT, THE DEPARTMENT OF PUBLIC WORKS WILL SEND A COPY OF THE AGREEMENT THAT HAS BEEN SIGNED BY THE PUBLIC WORKS DIRECTOR TO THE PROPONENT. THE DEPARTMENT WILL NOT GIVE FINAL APPROVAL FOR THE PROJECT UNTIL THE AGREEMENT HAS BEEN SIGNED BY THE PROPONENT AND FILED WITH THE JEFFERSON COUNTY AUDITOR. THE AGREEMENT SHALL INCLUDE BMPs FOR MAINTAINING PERVIOUS PAVEMENT. 14.) THE DEPARTMENT OF PUBLIC WORKS FEE FOR REVIEWING STORMWATER SITE PLANS AND CONSTRUCTION STORMWATER POLLUTION PREVENTION PLANS AND INSPECTING IMPLEMENTATION OF BEST MANAGEMENT PRACTICES IS$60 PER HOUR. Commercial and industrial developments located in all zoning districts shall screen from areas frequented by the general public, all materials stored outside buildings or structures which are not for display or immediate sale. Areas frequented by the general public shall be defined as public streets and rights-of-way, and other areas normally used by the general public located adjacent to the subject property. 28.) The storage and handling of inflammable liquids, liquified petroleum, gases and explosives shall comply with rules and regulations falling under the jurisdiction of the fire marshal, the laws of the state and other local ordinances. Bulk storage of inflammable liquids below ground shall be located no closer to the property line than the greatest dimension (diameter, length, or height)of the tank. 29.) All noise emitting machinery that cannot in any other way operate below the maximum permitted noise level shall be housed in a sound-attenuating structure. 30.) THIS SEWAGE DISPOSAL SYSTEM WILL REQUIRE AN ACTIVE MONITORING CONTRACT WITH THE PUD #1 PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL SYSTEM. While the County strongly recommends that the property owner allow inspectors to enter their property to inspect the system in order to insure that the system is functioning correctly, the property owner always has the right under the Fourth Amendment and our State Constitution, to refuse the inspectors access to their property. The property owner never loses the right to refuse access and can do so despite never having refused access before. 8.) As per Jefferson County Code 8.15 all onsite sewage systems require monitoring. Monitoring shall be provided by an approved entity. THIS SYSTEM IS REQUIRED TO BE INSPECTED EVERY 3 YEARS AT 6 MONTHS, AT ONE YEAR AND EVERY 3 YEARS THEREAFTER. A TIME OF SALE INSPECTION IS REQUIRED SHOULD THIS SCHEDULE HAVE NOT BEEN KEPT CURRENT. 9.) Health Dept. required to observe pressure test with system designer when system fully installed/complete, 48 hours notice to be provided for scheduling. 10.) Designer must be contacted prior to start of construction and for inspections during installation. 11.) Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system. 13.) Setbacks to all wells- 100' required from drainfield and reserve areas. 50' required from tanks and effluent transport lines. 14.) 10'separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components. 15.) Setback to surface waters- 100' required from drainfield and reserve areas. 50' required from tanks. 16.) When/if designated reserve area is utilized an alternative system providing an additional level of treatment may be required. 17.) A 30'setback shall be maintained from the disposal component and reserve area to any downslope footing, curtain or interceptor drain or drainage ditch. 18.) All components of the septic system are to be completely protected from vehicular traffic or mechanical disturbance. 19.) H -Permanent barriers are required along/around primary and reserve drainfield areas to protect from parking, driving, and other land disturbing activities prior to final. 20.) Drainlines are to be installed along the natural contours. 21.) Contact designer prior to installation for staking of drainfield area. 22.) Divert all sources of drainage, including roof drains away from septic tank and drainfield area. 23.) Any portion of transport line under a driven way is to be sleeved/cased or equivalent. 24.) Pump chamber shall be water tight and a minimum capacity of 1,000 gallons. It shall be equipped with a control panel outside within line of sight of the pump chamber. The panel shall have an audio and visual alarm, a cycle counter and a riser to grade. It shall be UL listed and meet L&I requirements. 25.) A screen shall be provided either at the outlet of the septic tank or around the pump or at both locations. A pump shroud is required if only an outlet baffle filter is used. 26.) Dose counters are required in the control panel for all drainfield components. 27.) Risers are required to grade with secured lids over both compartments and over the outlet of the septic tank for inspection and maintenance/monitoring. Minimum 20" riser over inlet. 28.) Distribution boxes shall be fitted with a riser to grade, constructed of durable materials and a secured lid, adequately sized to allow visible inspection of the liquid level in the box. 29.) This site has been granted a waiver from the state onsite sewage regulation 246-272-09501(1): disposal component down from 30 to 15 feet from a down-gradient drainage ditch. Compliance with conditions of the waiver is required. 30.) Approval of this sewage disposal permit does not preclude the permit holder from complying with the Unified Development Code for other/future development on the site. 31.) H -This permit is granted based on the recorded RESTRICTIVE COVENANT combining lots 5-9, and 4-10 of block 19, Phillips Bayview Addition to meet the minimum land area required under WAC 246-272 Onsite Sewage Code.
