HomeMy WebLinkAboutCommunity Health DirectorJEFFERSON COUNTY JOB DESCRIPTION Position: COMMUNITY HEALTH DIRECTOR FLSA: Exempt - Executive Department: Jefferson County Public Health UNION: Exempt Annual Hours Worked: 2080 Adopted: July, 2015 Exempt Salary System Grade: 20 Approved: 1.0 MAJOR FUNCTION AND PURPOSE 1.1 Persons in this position are responsible for the administration, planning, organizing, directing, and evaluating the Community Health programs, services, and policies of Jefferson County Public Health Department. Work functions are executed in coordination and collaboration with other divisions, agencies, stakeholders, and community partners in Jefferson County. Works with the Health Officer, Board of Health, the County Administrator, and the Public Health Director in the development and management of community health programs and procedures to accomplish local, state, and federal public health goals and policies. 2.0 SUPERVISION RECEIVED 2.1 Persons in this position are given significant discretion, within the scope of policy and regulations, in the routine performance of their duties; supervision and guidance are received from the Health Officer, Public Health Director, and Board of Health. Performance evaluations are conducted by the Public Health Director or the Health Officer. Department performance standards are reviewed by the Board of Health annually. 3.0 SUPERVISION EXERCISED 3.1 Persons in this position supervise professional and administrative staff in public health programs. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; evaluating, rewarding, disciplining, addressing complaints and resolving problems. Uses sound professional and technical judgment, conforms to adopted policies and procedures, solves problems, and achieves results consistent with stated goals and objectives. 3.2 Leadership style that will motivate staff, enhance efficiency, and generate a harmonious work environment through enhanced team building. Jefferson County Job Description: Community Health Director Page: 2 of 5 4.0 DUTIES AND RESPONSIBILITIES 4.1 Administers, prepares, and tracks: grants, deliverables, fees and budget. Oversees staffing and community based services for numerous Public Health community and clinical services. Has responsibility, and accountability for implementation of Community Health programs. Receives medical input from Health Officer. 4.2 Plans for Public Health services with community and Jefferson County Public Health team. Defines the mission, objective, and strategies for each program. Serves as member of community groups and boards that meet to address community health needs or changes to health delivery systems within the state. 4.3 Performs policy development and program design with identified goals. Financial management and resource allocation: responsible for community health budget development, monitoring, tracking, reporting and expenditure approval of the programs. Assure appropriate use of public funds, contractual obligations are met, and the Department and County goals are achieved within available resources. 4.4 Performs strategic planning with the Board of Health to identify local health needs using statistics and local health data. Policy is set by data and quality review process. Community Health Improvement process is ongoing Community Health Director work. Meet with community to improve outcomes in local systems. 4.5 Maintains liaison with Substance Abuse and Mental Health Providers in order to provide collaborative effort with community including, but not limited to: a) Recommending changes to the system, programs and services that reflect community needs, working to collaborate efforts and State mandates. b) Passes policy issues identified to the Board of County Commissioners' for consideration. C) Acts in lieu of Human Service staff as organizational liaison for advisory groups in insuring that issues receive appropriate attention within County and community. 4.6 Maintains active data gathering and monitoring, builds culture of data driven decisions. Maintains and analyzes socio -demographic changes, rates of disease trends, health outcomes, morbidity and mortality, immunization rates, and morbidity and mortality rates. 4.7 Monitors program and community needs within the County. Manages quality assurance process of clinical services and programs. Oversees staffing and community based services for numerous ongoing Public Health programs and clinical services. Jefferson County Job Description: Community Health Director Page: 3 of 5 4.8 Insures that the community is informed and represented in Public Health and resources of the health care delivery system. 4.9 Formulates and influences health policies and social policies which impact the health of the community. 4.10 Collaborates with others including health care providers, community leaders, and organizations to implement plans and evaluate current services in the community and department. 4.11 Participates in continuing education programs and incorporates changes into own practice; updates and schedules all department education programs. 4.12 Prepares proposals for support of research/replication projects. Participates in design of program services. 4.13 Develops local, regional, and statewide plans and policies, supports County Boards, provides leadership and consultation, strategic planning, analysis, and communication. 4.14 Establish annual work programs and identifies team approach to problem solving and process improvements. 4.15 With other management and administrative personnel, prepares budget estimates, statistical and routine reports. 4.16 Performs other tasks as directed. 5.0 REQUIRED KNOWLEDGE, SKILLS AND ABILITIES 5.1 Knowledge and advanced understanding of the principles and practices of Public Health, Nursing, Nursing Administration, and Public Health Policy of all department activities. Knowledge of laws, management and administrative practices, and technical advances related to the delivery of clinical medical services, medical treatments, and intervention. 5.2 Knowledge of health care system, public health system, Behavioral Health service systems locally and in State, and principles and practices associated with public health, healthcare providers, and human services. 5.3 Ability to work effectively with other professionals and community groups. Ability to read, analyze, and interpret scientific and technical journals, policy and regulations, and legal documents. Ability to write grants and reports. Ability to write speeches and articles for publication. Ability to effectively present information to management, public groups, Jefferson County Job Description: Community Health Director Page: 4 of 5 and/or boards. 5.4 Knowledge of principles and practices in the medical fields of family planning, vaccine preventable disease, and communicable disease, including medical treatments and pharmacology. 5.5 Lead agency HIPPA policies and enforcement to maintain confidentiality of health information. Ability to update actions and procedures as needed within Information Technology change: meaningful use, charting security, texting or social media and information sharing. 5.6 Ability to interpret and apply federal, state, and local laws and regulations which may affect situations involving public health services. 5.7 Ability to assure efficient and effective utilization of volunteers and department personnel, funds, materials, facilities, and time. 5.8 Professional grant and report writing skills. 5.9 Demonstrated successful supervisory and managerial experience is preferred. Implement and maintain sound organizational practices. 5.10 Maintain a high standard for accuracy, completeness, and efficiency in the preparation of reports and budgets. Independent and self -motivated to design and follow time lines, meet required deadlines while simultaneously completing other tasks. 5.11 Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. 5.12 Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. 5.13 Ability to use a computer, variety of software including excel, word, and electronic medical record systems, and other office standard software. Adapt to new or modified equipment which may be needed. 6.1 Master's degree in Nursing, Health Administration, Public Health, or related field; or combination of related undergraduate education and experience to equate to a masters Jefferson County Job Description: Community Health Director Page: 5 of 5 level degree; three years progressively responsible and relevant supervisory public health program management experience required, or equivalent combination of education and experience. 6.2 All required licenses must be maintained in an active status without suspension or revocation. 6.3 A valid Washington State Driver's License. Performance of job duties require driving on a regular basis, the use of the incumbent's personal motor vehicle on a regular basis, and proof of appropriate auto insurance. 6.4 Obtain National Incident Command Certifications levels 100, 200. 7.0 WORK ENVIRONMENT 7.1 The noise level in the work environment is usually moderate. 7.2 While preforming the duties of this job, the employee is frequently required to talk or hear. The employee is regularly required to sit. The employee is occasionally required to climb or balance; and stoop, kneel, crouch, or crawl; stand; walk; use hands to finger, handle, or feel objects, tools, or controls; and reach with hands and arms. 7.3 Good health, and good physical condition, appropriate to the position. The employee must occasionally lift and/or move up to 10 pounds.
