RE: WSART Consultant — CONTRACT AMENDMENT
STATEMENT OF ISSUE: Original funds for 7/2014 - 6/2015 Contract in the amount of
$14,440.00, to be reduced by $500. The new amount would be $13,940.00.
ANALYSIS:
Shannon Burns, a Juvenile Probation Counselor in Jefferson County is currently serving as one
of five statewide consultants to perform consultant duties to WSART programs. Jefferson
County is reimbursed for Shannon's time pursuant to the WSART Quality Assurance
requirements.
FISCAL IMPACT:
None. These funds reimburse for salary, benefits and travel.
RECOMMENDATION:
That the Board approve the Amended Contract and sign 3 originals. A fully executed original
will be returned to the BOCC office.

This Contract between the State of Washington Department of Social and Health Services (DSHS) and the
Contractor is hereby amended as follows:
1) The maximum consideration payable for FY15 is reduced by $500 from $8,220 to $7,720.
2) The new total consideration payable under this Contract is reduced to a maximum consideration of
$13,940.
All other terms and conditions of this Contract remain in full force and effect.
