Subject: BLD13-321
As chair of the Quilcene Museum and also the Worthington Mansion's rehabilitation, I am requesting that a
"foundation only" permit be granted under the IBC. Inclement weather will soon be upon us and the situation
will turn very serious if delays continue. I learned from Mr. Benskin via messages, that he had concluded
review and the permit should have been granted last Thursday. I received an email from Randy Marx stating
that he would not release the permit until an old septic design is final-ed. Apparently,there was a septic upgrade
back in the 80's that was never completed. I can understand the necessity of making sure the existing septic is in
safe working order so I did contract with Suzanne Martin,septic designer, and Reeve's Excavation, licensed
installer,to locate the items on Mr. Marx's' list. What I don't understand is why these items couldn't have been
"conditioned" so work could continue. The house is empty and moved from its old foundation and it will remain
unoccupied until next spring so no septic usage will occur. Thanks for any help and I plan on being at the
DCD's office when it opens tomorrow. Mari Phillips

Your application is missing the approval letter from ORCAA. Please submit this item so that the
application can be properly routed to the appropriate departments for review.
Pursuant to JCC18.40.110(3)(b), if an applicant refuses to submit additional information, does not
request additional time to submit the required information within the ninety (90) calendar day period,
or does not appeal the decision, the application will be considered abandoned and therefore
withdrawn and the applicant shall forfeit the application fee. The Department of Community

So nice to meet you! I have chatted with several folks in your group over the past years about a variety of
items... mainly National Register listing for the building. I would love to come and see the house. Turns out I
need to go to Port Townsend in the next couple of weeks and I was thinking that might be a good opportunity
to meet, look at the building,discuss issues, etc... I'm unclear as to what our specific regulatory review might
be for any of the work on site at this point since the building is not listed, nor or you receiving any grant
funding from the state or feds. We are certainly here for help and can answer questions..but I don't think we
have any official review power at this point.

Subject:Hamilton/Worthington House Quilcene
Hello Mr.Houser,Please allow me to introduce myself. My name is Mari Phillips and I serve as the
chair for Worthington Park and also for the Quilcene Historical Museum. As I believe you now know,
our Museum has recently purchased the Worthington property including the 121 year old house.
Sadly, it is in need of a new foundation and we are arranging all the details towards that process. We
are planning no restoration or renovations at this time due to money constraints. We have a very tight
budget until next year but do have enough to do the foundation and the relocation of the electrical
service now. The permit center in Jefferson County tells me that we must notify your agency and
submit a letter from your agency to them before the demolition permit can be obtained. We are a
volunteer organization and much of this first phase is being done by volunteers so the timing is tricky.
Please inform me as to what the policy is that we must fullfill. Thanks for your help in this matter.

ARTICLE 1 _ `

The name of this non-profit organization shall be the QUILCENE HISTORICAL MUSEUM,
located at 151 Columbia Street; P. O. Box 574; Quilcene WA 98376.
ARTICLE II
Purposes of Quilcene Historical Museum
Section 1. To bring together interested persons who want to preserve, display and explain the
history of this community in a museum setting.
Section 2. To promote the significance of historical preservation, awareness and understanding
for the benefit of present and future generations.
Section 3. To collect, preserve and make available to interested persons items of historical
content—with special emphasis on exhibits and information concerning past and present
families, businesses, events, organizations, schools and structures of the Quilcene area.
ARTICLE III
Membership and Dues
Section 1. Membership. General membership shall be open to anyone upon payment of dues.
Section 2. Dues. Anneal dues shall be for the following categories: individual member,
household family, service group and business. The amount of dues will be determined by the
Museum Board.
Section 3. General Member. Each general member is entitled to Museum publications, to work
in and for the Museum and to be considered for Board membership when a vacancy should
occur.

Section 4. Life Memberships. Life memberships without further payment of dues may be
purchased. A life member shall have full membership privileges.
Section 5. Honorary Members. Honorary memberships maybe bestowed by 2/3 vote of the
Board

ARTICLE IV
Board of Directors, Officers, Museum Manager and Duties
Section 1. Board Number, Officers and Vacancies. The governance ofthe Museum shall be by
a board of up to twenty-five members, which includes four(4) elected officers: chairman, vice-
chairman, secretary and treasurer. Vacancies on the Board will be filled by a majority vote of the
Board at a regular meeting.
Section 2. Removal. Any Board member or officer who misses 3 consecutive meetings shall be
notified in writing by the Board of their removal from said Board or office after a majority vote
at the fourth meeting.
Section 3. Chairman. The chairman shall preside at regular and special board meetings and at
meetings of the Executive Board. The chairman will conduct Museum business with other
organizations and governmental agencies or appoint a Board member to represent the Museum.
The chairman shall sign all deeds, leases, conveyances and other legal instruments executed by
the Museum, except in those cases where the signing is delegated by the Board to some other
officer or agent.
Section 4. Vice-Chairman. The vice-chairman shall assist the chairman and assume the duties
of that office in his/her absence, or in the event of his or her inability to refusal to act. The vice-
chairman will be responsible for those needed duties as determined by the Board.
Section 5. Secretary. The secretary shall record all proceedings of the Museum Board and
Executive Board meetings. The secretary shall be responsible for correspondence of the Museum
Board and the custodian of its records; and, in general,perform such duties as may be assigned
by the chairman of the Board.
Section 5. Treasurer. The treasurer shall be responsible for all funds received by the Museum
and given to him/her. The treasurer shall keep an accurate account of all receipts and
disbursements and report thereon at all regular board meetings. An in-house accounting by the
Executive Board will be held at each year's end.
Section 7. Board Duties. The Board shall control, regulate and conduct the affairs of the
Museum in all matters and at all times; provide for housing of artifacts and materials, provide
equipment, facilities and operation of the Museum; hire a manager and define his or her duties
and salary; fix dues amounts; authorize all expenditures; remove any officer, Board member or
manager for disability, incompetence or misconduct with a 2/3 Board vote at a regular meeting.
Section 8. Compensation.No Board member shall receive directly or indirectly any salary. The
Museum shall be authorized, with a 2/3 vote, to pay a Board member reasonable compensation
for services rendered or material provided to the Museum.

Section 9. Indemnification. No Board member or employee of the Museum shall be personally
liable for the debts, liabilities or obligations of the Museum. Any and all creditors of the Museum
shall look only to the assets of the Museum for payment.
Section 10. Museum Manager. The museum manager shall be in charge of opening the
Museum for the benefit of the public; scheduling his/her salaried time as well as that of
volunteers; record the accession of material received; arrange for the display of artifact; increase
membership; offer tourist information; increase Museum's awareness with community, school
and other organizations; conduct correspondence for the Museum and maintain a correspondence
log; conclude sales of items made available by the Museum and approved by the Board. The
manager shall perform such additional duties as from time to time may be required by the Board.
A specific list of jobs will be provided to the manager at the time of hiring.
ARTICLE V
Election of Officers
Section 1. Nominations. The nomination of officers (chairman, vice-chairman, secretary and
treasurer) shall be held in September of even-numbered years. Nominees shall be members of the
Museum Board and nominated by said Board.
Section 2. Elections. The election of officers shall be at the scheduled meeting in October with
elected officers assuming their duties in November.
Section 3. Term of Office. The term of office shall be two years, with re-election permitted.
Section 4. Special Elections. In extenuating circumstances where an officer may not be able to
continue to serve, a special election may beheld. The nomination will be made at a meeting of
the Board,with election of the officer at the following meeting.
ARTICLE VI
Meetings
Section 1.Meetings. The Museum Board shall meet monthly unless postponed by the chairman.
Special meetings of the Board may be called by the chairman, or at the request of three board
members.
Section 2. Executive. Executive meetings may also be called by the chairman. Action by the
Executive Board (chairman, vice-chairman, secretary and treasurer) is subject to review by the
whole board.
Section 3. Quorum. A simple majority of the board members attending a regular or special
meeting constitutes a quorum and may conduct business of the Museum

Committees
Section 1. Committees. The chairman or the Board may create such committees as either may
deem necessary.

Custodial Responsibilities
The Board shall make provision for the custody of all material of historic value received by the
Museum, limited only by a signed agreement with the donor.

been duly adopted by the Quilcene Historical Museum this second day of May 2011.
