The property owner is responsible for the accurate location of all property lines.Any removal of or major disturbance of soil in the primary or reserve drainfield area may create site conditions that are unacceptable for the installation of a sewage disposal system. Any change in drainfield or tank location may invalidate this permit unless prior approval is obtained from the Jefferson County Environmental Health Division. If during excavation or development of the site an area of potential archeological significance is uncovered,all activity in the immediate area shall be halted, and the UDC Administrator shall be notified at once. Permit issued to CONSTRUCT, ALTER, REPAIR OR MODIFY AN INDIVIDUAL SEWAGE DISPOSAL SYSTEM IN JEFFERSON COUNTY, WASHINGTON This permit is issued for a period of three years (unless otherwise stated above) in accordance with Jefferson County Rules and Regulations for On-Site Sewage Systems, codified in JCC 8.15 as amended. This permit may not be renewed. '11°0 Jefferson County Environmental Health Specialist This permit with conditions must be onsite during all phases of construction HEALTH DEPARTMENT MUST BE CONTACTED FOR FINAL INSPECTION. SPECIAL CONDITIONS APPLY - SEE ADDITIONAL PAGES CONDITIONS OF APPROVAL - PERMIT NO.: SEP14-00096 1.) This permit was issued to correct a violation of WAC 246-272A for a failed drainfield. The permit must be completed within 90 days of the date of issuance. Weather conditions may cause a one time extension. 2.) New water line to be installed along property line and cross sewage transport line once at home. 10' separation to any septic component to be maintained all other areas. 3.) H - The new transport line must be pressure tested for 24hr with no pressure loss. Doccumentation must be a part of the record drawing. 4.) Two pods are installed in 0.6 gal sq/f soils one pod at 0.4. 5.) Health Dept. required to observe pressure test with system designer when system fully installed/complete, 48 hours notice to be provided for scheduling. 6.) This system must be constructed by an installer certified by the Jefferson County Public Health Department per Jefferson County Code 8.15.120 (9) and state code WAC246-272A-0250, (proximity to shoreline) 7.) Designer must be contacted prior to start of construction and for inspections during installation. DESIGNER IS REQUIRED TO DO A PRECOVER INSPECTION ON ALL TYPES OF SYSTEMS. 8.) Notification of the start of construction shall be faxed or emailed to Jefferson County Public Health ONE WORKING DAY prior to start. 9.) H - Existing tankS shall be properly abandoned. They shall be pumped and filled with clean fill. Documentation to be provided to Health Dept. prior to final. 10.) H - An asbuilt drawing and certification of completion by the designer is required prior to final approval. 11.) H - AS PER WAC 246-272AAND JEFFERSON COUNTY CODE 8.15 ALL ONSITE SEWAGE SYSTEMS REQUIRE THAT A RESTRICTIVE COVENANT REGARDING THE MONITORING OF THE ONSITE SEPTIC SYSTEM BE RECORDED TO THE PROPERTY TITLE. THE PROPERTY OWNER SHALL ASSURE THAT MONITORING IS PROVIDED BY AN APPROVED ENTITY AT THE FREQUENCY DEFINED PER STATE WAC 246-272AAND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. A COPY OF THE RECORDED OPERATIONS AND MONITORING AGREEMENT IS REQUIRED PRIOR TO FINAL APPROVAL OF THE SEWAGE DISPOSAL PERMIT 12.) Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system. 13.) Dry season installation required. 14.) Contact designer prior to installation for staking of drainfield area. 15.) Divert all sources of drainage, including roof drains away from septic tank and drainfield area. 16.) Approval of this sewage disposal permit does not preclude the permit holder from complying with the Unified Development Code for other/future development on the site. 17.) This system as designed and approved is sized for only one single family residence. It is not sized for an Accessory Dwelling Unit (ADU). The minimum daily design flow per residence is 240 GPD. 18.) The project shall adhere to the Best Management Practices (BMPs) to control stormwater, erosion and sediment during construction. BMPs shall address permanent measures to stabilize soil exposed during construction, and in the design and operation of stormwater and drainage control systems. 20.) Approval/issuance of a sewage disposal permit does not guarantee the approval of other development or a building permit on this site. Compliance with other Jefferson County and Washington State Codes is required. 21.) All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued. 22.) The site plan as submitted with the Septic Permit application on July 14, 2014 has been reviewed for consistency under the UDC, and has been approved by Jefferson County Department of Community Development. Any modifications, changes, and/or additions to the stamped, approved site plan dated September 30, 2014 shall be resubmitted for review and approval by Jefferson County Department of Community Development. 23.) This approval is for a Septic System Repair only. Any future permits on this site are subject to review for consistency with applicable codes and ordinances and does not preclude review and conditions which may be placed on future permits. 24.) The parcel is located within a coastal SIPZ (seawater intrusion protection zone) according to the County GIS map. There are voluntary and mandatory measures identified in the Jefferson County Seawater Intrusion Policy (Resolution 61-02, effective September 23, 2002) that apply to well drilling proposals and building permit applications on existing lots of record. 25.) The construction notes assume that the designer is not a licensed surveyor. 1. This site map does not represent a survey nor does it purport to show all easements or encroachments, if any. If easements are not shown, it is the responsibility of the owner and the owner's liability. 2. The location of this drainfield is based on the owner or the owner's agent locating property lines and corners. If those boundaries are not accurate,the designer is not liable for mistakes in location of primary or reserve drainfields on the design. Easements not shown on design or on the plat map included with the design are the sole responsibility of the owner.However,the designer must make a good faith effort to accurately locate drainfield areas according to these instructions and,as circumstances permit,the preferences of the owner. 3.All surface water must be diverted away from drainfield area. Insure that final grading of drainfield area directs water away from the drainfield. 4.Prepare the site and install the units during dry conditions so equipment does not damage the structure of the soil in the absorption area. Use of tracked machinery is strongly recommended on Glendon®sites. 5. This system is not designed for use with a garbage disposal installed in the building(s)served. Use of a garbage disposal in a Glendon®system will require a larger than normal septic tank and additional expense in pumping fees due to the markedly increased quantity of solids in the septic tank. 6. The property owner is responsible to fill soil logs after final approval of the design. 7.No burning on drainfield area. 8.No vehicular traffic on drainfield area allowed. 9. This system requires a proper degree of maintenance by a Glendon®licensed technician. Certification, design and installation does not necessarily guarantee trouble-free operation. 10.No septic additives are to be used. 11. The property owner is responsible for damage done by landscapers. 12. Curtain drains may be required at time of installation even though not shown on design. 13. The owner/grantor acknowledges having read the above construction notes and disclaimers and agrees to abide by the content. Code of Good Practice for Glendon®Designers: 1. When considering locations for Glendon®units,try to locate them so they are not too noticeable. 2. Staking out the Glendon®is a good practice so the Health Department can see the actual location,and the installer knows exactly where they are to be installed. 3. It is important to see where surface runoff from the adjoining property may be coming,and,if necessary, install a curtain drain to protect the area with the Glendon®units. Avoid installing Glendon®BioFilter units in swales. 4. On tight soils(low permeability), surface runoff can be a major factor, and a swale or berm can be effective to divert surface water from flooding the area. 5. When designing curtain drains at property lines,check for roots for mature trees on the other side of the property which must be avoided. 6. Curtain drains should not be installed against the slope so they become too deep.Also,sometimes you can be in the shallow aquifer. Extreme Cold Weather Climates Excluded Application of Glendon®BioFilter Models M3 and M31 is limited to areas with climates exhibiting an annual accumulation of 6000 heating degree days or less. Glendon®BioFilter Technologies has research sites in planning that will extend this range of applicability.
