If during excavation or development of the site an area of potential archeological significance is uncovered, all activity in the immediate area shall be halted, and the UDC Administrator shall be notified at once. Permit issued to CONSTRUCT, ALTER, REPAIR OR MODIFY AN INDIVIDUAL SEWAGE DISPOSAL SYSTEM IN JEFFERSON COUNTY, WASHINGTON This permit is issued for a period of three years (unless otherwise stated above) in accordance with Jefferson County Rules and Regulations for On-Site Sewage Systems, codified in JCC 8.15 as amended. This permit may not be renewed. Jefferson County Environmental Health Specialist This permit with conditions must be onsite during all phases of construction HEALTH DEPARTMENT MUST BE CONTACTED FOR FINAL INSPECTION. SPECIAL CONDITIONS APPLY - SEE ADDITIONAL PAGES CONDITIONS OF APPROVAL - PERMIT NO.: SEP13-00011 1.) The permit must be completed within 90 days of the date of issuance. 2.) SEVERAL WELLS NEAR BY- 100' required from drainfield and reserve areas. 50' required from tanks and effluent transport lines. 3.) This system as designed and approved is sized for only one single family residence. It is not sized for an Accessory Dwelling Unit (ADU). The minimum daily design flow per residence is 240 GPD. 4.) H - AS PER WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 ALL ONSITE SEWAGE SYSTEMS REQUIRE THAT A RESTRICTIVE COVENANT REGARDING THE MONITORING OF THE ONSITE SEPTIC SYSTEM BE RECORDED TO THE PROPERTY TITLE. THE PROPERTY OWNER SHALL ASSURE THAT MONITORING IS PROVIDED BY AN APPROVED ENTITY AT THE FREQUENCY DEFINED PER STATE WAC 246-272A AND JEFFERSON COUNTY CODE 8.15 AS ADOPTED OR AMENDED. H - An asbuilt drawing and certification of completion by the designer is required prior to final approval. 10.) Before final approval is given, the designer shall provide an operations and maintenance manual to the property owner and the Health Department. The manual must instruct the owner of the on site sewage system on the ways to properly operate and maintain all components of the system. 11.) 10' separation required between a Water line and all portions of the onsite sewage system; effluent transport line, tanks, treatment and disposal components. 12.) Approval/issuance of a sewage disposal permit does not guarantee the approval of other development or a building permit on this site. Compliance with other Jefferson County and Washington State Codes is required. 13.) All construction and development activities must comply with all permit conditions, Washington State and Jefferson County Codes and Recommended Standards and Guidance documents in effect when the permit is issued. 14.) Any portion of transport line under a driven way is to be sleeved/cased or equivalent. 15.) Approval of this sewage disposal permit does not preclude the permit holder from complying with the Unified Development Code for other/future development on the site. 16.) The project shall adhere to the Best Management Practices (BMPs) to control stormwater, erosion and sediment during construction. (Streamline) is pleased to submit this septic system design and permit application for the referenced property, owned Neva Arlene Johnson. Ms. Johnson is currently remodeling the existing dwelling on the property, and wishes to permit and install a replacement septic system. The proposed septic system, depicted in attached drawings, includes three M31 Glendon Biofilter pods. EXISTING APPROVED SEWAGE DISPOSAL PERMIT SEP13-11 Jefferson County Environmental Health issued Sewage Disposal Permit SEP13-11 on October 4, 2014. The permit was given a "limited permit life" due to concerns about the existing gravity- flo septic system, recently decommissioned December 1, 2014. In order to persuade 6L� I ironmental Health to extend this permit, the applicant has vacated the subject premises and Ii'�turned off the water supply to the dwelling, pending the installation of a new, permitted septic f system. ii et SYSTEM RE-DESIGN The three-bedroom septic design approved for Sewage Disposal Permit SEP13-11 included a proposed 200 linear ft of sand-lined trenches equipped with pressurized distribution. Soils at the north end of the subject parcel would support this type of system. However, in an effort to Page 2 minimize the need to remove large trees to accommodate a new drainfield, the 4ppfrct s ` proposed a three-pod Glendon Biofilter system. Site soils are well suited for a Gl- ` ten PENDING BOUNDARY LINE ADJUSTMENT SUB13-18 ye0/th The applicant is in the process of annexing an area adjacent to the subject parcel to the north. This area (outlined in green on the attached Site Vicinity) currently includes an access easement with a gravel driveway serving properties to the west of the subject parcel. The proposed septic system design does not incorporate any of this area proposed for annexation. The only change that will be caused by the boundary line adjustment will be an increase in the subject parcel area, from about 1.3 acres to about 2.2 acres. PROPOSED DESIGN Capacity: The applicant proposes to permit and install a three-bedroom-rated septic system, with a peak discharge capacity of 360 gallon per day, and an average discharge capacity of 270 gpd or less. The design is intended to accommodate the existing one-bedroom house and satellite (detached)bedroom. Parcel Size: 1.296 acres (56,463 SF). Drinking Water Supply: The parcel is currently served by a private on-site water well. A newly-installed water meter connected to the PUD water system, remains un-connected to the subject parcel,though it may someday be connected. Topography: The site gently slopes downward to the south at about 1 to 5%. Surface Water and Drainage: No surface waters exist on the subject parcel, nor are there any indications of focused stormwater surface flow. Mystery Bay, a branch of Kilisut Harbor, borders the subject parcel to the south. Soil Evaluation: Seven soil evaluation holes were excavated to examine site soils in the locations of the proposed Glendon pods and reserve drainfield (See attached Site Detail with Proposed Septic System Layout). Beneath about 6 to 8 inches of topsoil, light brown sandy loams of varying coarseness generally show no indications of historical prolonged saturation above about 36" to 48" below ground surface (bgs). Compacted restrictive material ("hardpan") was encountered at depths of 36" to 48" bgs in some holes, and not others, suggesting soil heterogeneities in the study area. The proposed reserve area, in the location of a former driveway, has an additional —8" of compacted, imported gravel road bed. Beneath this fill material comprising the former road bed, native soils appear un-disturbed and suitable for shallow pressurized distribution. Drain Field Sizing: Using an application rate of 0.6 gals per day per square foot, consistent with Soil Type 4, each of the three proposed M31 Glendon pods would require 200 square feet of absorption area, to accommodate a peak flow of 120 gal/day/pod: 120 gal/day_0.6 (gal/day)/ft2 =200 ft2 Glendon Biofilter Positioning: The three Glendon Biofilter pods would be positioned approximately as shown in the attached Site Detail with Proposed Septic System Layout. Exact positions of the pods would be determined in the field upon system layout, taking into account existing large trees. Each pod will be surriounded by the 200 SF primary absorption area, covered with appropriately-graded sand. A 30-ft protection buffer downhill of the absorption area will be protected for the life of the system. Glendon Biofilter Installation: The Glendon pods must be installed by a Glendon-certified installer. The certified installer will position the M31 basin with an average rim elevation, above pre-disturbance grade, in accordance with proprietary design, as prescribed for Soil Type 4 and typical slopes of 1 to 3%. System Dosing Frequency and Volume: Glendon Biofilters require frequent, small doses of pump tank effluent delivered throughout the day. Dose frequency and dose volume will be programmed by the designer, upon system start-up, in accordance with proprietary design, as prescribed for the standard M31 basin. Septic System Operations & Maintenance The property owner will record an active monitoring agreement to the property title, prior to permit finalization, in accordance with County requirements. State law requires this alternative septic system to be inspected every year, with an evaluation completed by qualified personnel and recorded with the Jefferson County Environmental Health Department. More frequent inspections of the system are recommended, but do not need to be recorded. Prior to permit finalization, Streamline will provide the property owner an Operations & Maintenance Manual that specifically describes this system and the best practices to be followed for maximum service lifetime. Elevations of Glendon Pod Rims must vary less than +/-Y4-inch. • /Y E, Septic Tank and Pump Tank Placement: The pump tank excavation must have a horizontal bottom prior to adding bedding material. Six inches (6") of pea gravel bedding must be provided to properly support tank in order for tank warranty to remain effective. If shallow groundwater is encountered during tank installation, or if shallow groundwater is known to occur, a newly-installed pump tank should be filled with water to prevent floating, in case groundwater rises around it. Tank must be covered with a 1-ft minimum thickness compacted soil, or structural fill, to a maximum cover of 3-ft, unless a heavy load-rated tank is used. Risers: Access risers should be installed above pump tank openings, and valve splitter assembly. Risers and lids should extend 2 to 6 inches above grade to prevent debris from falling into risers upon opening. Tank access risers shall be bonded to integral rings built into tank openings, using 3MTM Marine 5200 Adhesive Sealant, or equal, or better. Piping and conduit entering/leaving the pump tank shall enter the access riser above such integral rings. Piping and conduit openings shall be grouted and/or sealed, or equivalent, in order to prevent the transport of water or gases. Electric Supply: Two separate circuit breakers must be provided, in order to maintain a separate circuit for the alarm and control panel, and a separate circuit solely for the pump. All electrical equipment installed within the pump tank must be of non-corrosive material. Any wire splices installed in pump tank must be suitable for wet locations. All electrical conduit material must be non-corrosive and must be installed to prevent the transport of gases or vapors (sealed). Monitoring Ports: Future septic system inspections will be simplified with properly-installed monitoring ports at key points. In the Glendon Pods, it is useful to have 4-inch PVC monitoring ports extending to different elevations within the media. Monitoring ports must be installed during installation, and prior to covering. They must be anchored by any of several means, in order to maintain their stability for several decades. Using wood or other biodegradable materials is not recommended for anchoring/stabilizing systems.
