Chair David Sullivan called the February, 2016 meeting of the Jefferson County Board of Health
to order at 2:30 p.m. A quorum was present.

APPROVAL OF AGENDA
Jean Baldwin, Director, Jefferson County Public Health, informed the Board that additional
information was distributed to be added to packet materials under the Community Health fees
Policy update agenda item.
Chair Sullivan asked for approval of the February 18, 2016 Agenda.
Member Kler motioned to approve the agenda as amended; the motion was seconded by
Member Westerman. No further discussion. The motion passed unanimously.
APPROVAL OF MINUTES
Chair Sullivan asked for approval of the minutes of the January 21, 2016 meeting of the
Jefferson County Board of Health.
Vice -chair Catharine Robinson, Port Townsend City Council, noted an inconsistency in the
minutes regarding the terms used to indicate motions and approvals, and asked that this
inconsistency be corrected. It was decided the phrasing would be "asked for approval,"
"motioned to approve," and "the motion was seconded."
Chair Sullivan asked for approval of the minutes of the January 21, 2016 meeting of the
Jefferson County Board of Health as amended.
Member Kler motioned to approve the minutes as amended; the motion was seconded by
Member Austin. No further discussion. The motion passed unanimously.

OLD BUSINESS AND INFORMATIONAL ITEMS
1. Deputy Mayor Catharine Robinson Appointed to the Board of Health
The Board congratulated and welcomed Ms. Robinson.
2. Jefferson County Board of Health Roster
The roster was presented to the Board. There was no comment.
3. January 28, 2016 CHIP Vaccine Hesitancy Training with Dr. Ed. Marcuse
Ms. Baldwin reviewed a recent training attended by health professionals in the community as
part of the Jefferson County Community Health Plan's immunization initiative. The speaker was
Dr. Ed Marcuse, MD, MPH, Professor of Pediatrics and Adjunct Professor of Epidemiology at
the University of Washington and Associate Medical Director, Quality Improvement at Seattle
Children's Hospital. His special interest area is pediatric immunization. The focus of his
presentation was research on factors that drive parental resistance to vaccination, a phenomenon
known as "vaccine hesitancy". Dr. Marcuse advised the group on how to help parents overcome
their hesitancies. Effective communication by a trusted health advisor, unified messaging by the
entire health delivery team, and ease of availability were noted as effective tools. Fear tactics
were dismissed as ineffective. Focusing on groups that don't harbor deep philosophical
resistance to vaccination was emphasized as the best strategy to increase community
immunization levels — a key goal in the control of vaccine preventable disease outbreaks.
4. Yearly Public Records Request Summary
Denise Banker, Health Assistant, reported on the number of public record requests and their
associated costs for the year 2015. There were fewer requests as compared to 2014 and this is
due to continuing efforts to make records available to the public via on-line access.
5. Nurse -Family Partnership (NFP) Update
Ms. Baldwin reported to the Board on the importance of the Nurse Family Partnership, a
maternal and early childhood health program that links at -risk, first-time parents to specially
trained maternal and child health nurses. The program documented long-term success for first-
time mothers, their babies, and society. Dr. David Olds, the program's founder, recently
contacted a former Jefferson County Public Health employee, Quen Zorrah, to discuss her
foundational research into Adverse Childhood Experiences (ACEs), and her work on the
NEAR@Home Toolkit developed by Health Resources and Services Administration Region X.

The acronym NEAR stands for Neuroscience, Epigenetics, Adverse Childhood Experiences, and
Resilience. The NEAR toolkit is a training manual designed, reviewed, and tested that guides
professionals through processes to safely and effectively talk about the trauma of Adverse
Childhood Experiences. Ms. Baldwin emphasized the effectiveness and importance of this
evidence -based program, which is supported by Federal grants to state programs.
Dunia Faulx, Community Health Director, will attend Nurse -Family Partnership training in
Denver, Colorado in March.
6. Board of Health Action Items
Ms. Baldwin presented the Board with a review of the actions it took in 2015.
NEW BUSINESS
1. 2016 Environmental Health Performance Measures
Jared Keefer, Environmental Health Director, and Michael Dawson, Water Quality Manager,
presented end -of -year Environmental Health Performance Measures to the Board. Beginning
with Solid Waste and working their way through Food Safety, Drinking Water, Onsite Sewage,
Septic Operations & Monitoring programs, and Water Quality. Misters Keefer and Dawson
explained to the Board successes achieved and struggles faced by each program within the
division of Environmental Health. Performance Measures are posted to the Jefferson County
Public Health website. It was noted that cuts to funding at the State level have impacted program
effectiveness. Mr. Keefer assured the Board that proposals were before the Legislature to re-
establish funding for Hazardous Waste prevention and clean-up.
2. Proposal: Joint Meeting JCPH Board of Health and JHC Board of Commissioners
Ms. Baldwin suggested to the Board that it schedule a joint meeting with the Jefferson
Healthcare Board of Commissioners to hear the finalized report from the four Community Health
Improvement Plan (CHIP) groups that have been working to develop strategies to address
prioritized community health needs and timelines to implement these strategies. The CHIP
groups seek the respective Boards input on their identified action steps. The Board expressed
support for this meeting. Member Jill Buhler, Hospital Commissioner, assured the group of
Jefferson Healthcare's interest in a joint meeting. Times and locations for the meeting were
touched upon and plans were initiated to further the proposal.
3. Community Health Fees Policy Update
Ms. Baldwin explained to the Board language changes in the Community Health Fees Policy and
asked that the updated Policy be signed.
Chair Sullivan asked for approval of the updated Community Health Fees Policy.
Member Kler motioned to approve. Member Phil Johnson seconded the motion. No further
discussion. The motion passed unanimously.
4. Syringe Exchange Services Adds Naloxone Distribution
Dr. Tom Locke, Jefferson County Public Health Officer, handed out a press release announcing
the availability of Naloxone through the JCPH Syringe Exchange Program. Funding for 25
naloxone kits, each containing two syringes for injection of the drug, has been provided through
donation from the Center for Opioid Safety at the University of Washington. A training session
is required before this injectable form of naloxone can be provided. Dr. Locke emphasized that
naloxone is not a cure for overdose; rather it is a lifesaving antidote that buys the user the time to
get to additional medical treatment.
Vice -chair Robinson noted her relief that an initial fee associated with acquisition had been
dropped. The Substance Abuse Advisory Board, of which she is a member, had been concerned
the fee would present a barrier to most opiate users.
5. Opiate Overdose Reporting — Clallam County Model
Dr. Locke reported to the Board that opiate overdose has been made a notifiable condition in
Jefferson County. JCPH followed Clallam County's model and reporting form.
6. Influenza Update
Dr. Locke reported to the Board that influenza, while late, has arrived in our area. Cases have
been reported in neighboring counties as well. Influenza B, a traditionally less serious strain, has
been the predominate strain detected. There have been a few cases of H1N1 influenza identified
in children; this is being carefully tracked by healthcare professionals.
7. Press Release for Public Health Heroes
Ms. Faulx updated the Board on the status of the Public Health Heroes nomination process.
Wednesday, March 9 is the due date for all nominations. The workgroup will present
nominations for a vote to the Board at its March 17th meeting, and awardees will be invited to the
April Board of Health meeting to receive their certificates.
ACTIVITY UPDATE
Member Kler motioned the Board to acknowledge retiring Director Jean Baldwin's contributions
to Jefferson County Public Health.
The motion was seconded by Jill Buhler.
Member John Austin, Citizen, offered, on behalf of the entire group present, his thanks to
Baldwin, particularly to her dedication and service to Jefferson County and Community Health.
Austin further lauded Baldwin's ability to maintain high quality services in the face of budget
restrictions and funding gaps. The entire group applauded Ms. Baldwin's achievement.

There was no public comment.
AGENDA PLANNING CALENDAR
End -of -year Community Health Performance Measures will be presented at the March 17th
meeting of the Jefferson County Board of Health.
Philip Morley, Jefferson County Administrator, updated the group on the ongoing selection
process regarding the Public Health Director open position. It was decided that Chair Sullivan
would sit on the selection committee in place of Kathleen Kler, whose schedule conflicts with
the desire to expedite the process.
NEXT SCHEDULED MEETING
The next Board of Health meeting will be held on Thursday, March 17th, 2016 from 2:30 — 4:30
p.m. at Jefferson County Public Health, 615 Sheridan Street, Port Townsend, WA.
ADJOURNMENT
Chair Sullivan adjourned the February 18th, 2016 Jefferson County Board of Health
meeting at 4:30 p.m.

Community Health Fees
All of the services/fees listed for Jefferson County Public Health and Community Health
are set as follows:
A comprehensive analysis will be completed, at a minimum, every two (2) years to
review rates for clinical services, including family planning services and immunization
programs.
Fees for medications, antibiotics and treatment methods will be based on cost of
acquisition.
All fees for vaccines not subsidized by the Vaccine for Children or Washington Vaccine
Association will be based on the cost of the vaccine, shipping and handling, and
allowable market reimbursement rates.
Laboratory Services are based on the actual fees of laboratory contractors. Price lists
are available upon request.
Office visit fees are calculated and reviewed annually and are based on cost analysis. In
the event the cost analysis exceeds the contracted private insurance provider rates or
market rates, the lower of the rates will apply.
The Federal Poverty Guidelines is used to establish the sliding fee scale, annually. A
copy of the sliding fee schedule will be available upon request.
The sliding fee schedule will not be offered for privately purchased vaccines
administered for international travel or outside laboratory cost.
Certain service fees may be waived, including but not limited to communicable disease
prevention services, medication, laboratory services, or vaccines to prevent outbreaks
of communicable disease. Services for vaccines subsidized by the Vaccines for
Children or WA Vaccine Association and Family Planning Services will not be denied
due to a client's inability to pay.

Jefferson County Public Health will automatically implement future modifications to fees
for vaccines, medication, and medical supplies when any of the following changes
occur:
There is a change in the actual cost of the service provided by an outside vendor,
the supply, medication, vaccine, laboratory charges, or shipping and handling
costs.
2. The Federal Poverty Level Guidelines change.
3. The maximum allowable Medicaid reimbursement rate changes and is more than
market rate.
