The County of Jefferson, State of Washington, desires to supplement the agreement entered into
with Shearer Design LLC and executed on October 27, 2014 and identified as:
14-1021P
All provisions in the basic agreement remain in effect except as expressly modified by this
supplement.
The changes to the agreement are described as follows:
II
Section II, SCOPE OF WORK, shall be amended by supplementing Task No. 2 — PLANS,
SPECIFICATIONS, AND ESTIMATE (PS&E), Task No. 4 — HYDRAULIC ANALYSIS, and
Task No. 6 — LOAD RATING. The revised scope of work under Task Numbers 2, 4, and 6 are
set forth in the attached Exhibit A, and by this reference made a part of this supplement.
IV
Section IV, TIME FOR BEGINNING AND COMPLETION, shall be amended to establish a
new contract completion date. The previous contract completion date was 12/31/16. The new
contract completion date is 12/31/18.

Section V, PAYMENT, shall be amended as follows:
Task No. 2, PLANS, SPECIFICATIONS, AND ESTIMATE will have a new maximum amount
payable of $48,909.
Task No. 4, HYDRAULIC ANALYSIS will have a new maximum amount payable of $20,847.
Task No. 6, LOAD RATING will have a new maximum amount payable of $12,799.
The new, total maximum amount payable for the Agreement will be $102,980, as shown below.

Description of Work:
Amend the current PS&E per the RCO technical review panel comments so as to accommodate a 24 ft natural
ankfull width. Supplement also establishes a new contract completion date due to a delay attributable to the
agency.

EXHIBIT A
SUPPLEMENT NO. 1 -- SCOPE OF WORK
Salmon Creek Bridge
West Uncas Road, Milepost 0.804
The objective of this Agreement is to provide professional services necessary for the preparation
of reports, plans, specifications, and estimates (PS&E) for the construction of replacement of the
culvert at West Uncas Road milepost 0.804, in unincorporated Jefferson County, Washington.
The current PS&E set will be modified via this supplement to redesign the channel bed width
under the bridge to a 24 ft natural bankfull width for permit submission and implementation.
TASK 2 - PLANS, SPECIFICATIONS AND ESTIMATE (PS&E)
The focus of this task will be modification of the current PS&E to provide a 24 foot natural
bankfull stream width.
Several options will be evaluated to arrive at the best solution in regards to project overall cost,
impact to adjacent properties, constructability and habitat impacts. A summary letter stating the
preferred option will be provided to the County.
2.1 Structural Design
This scope of work is based on the CONSULTANT performing the structural engineering
analysis necessary to provide road and structure plans for the modified stream width.
Deliverables:
A. Revised PS&E for the West Uncas Road, Salmon Creek Bridge
B. Preferred option summary letter.
TASK 4 - HYDRAULIC ANALYSIS
The general focus of this task will be to modify and reissue the existing hydraulic report for the
project to reflect comments provided by the RCO Salmon Recovery Funding Board.
Deliverables:
A. Revised Hydraulic Report
TASK 6 — LOAD RATING
Replenish funds for this task, used as previously documented to cover coordination with the
RCO Salmon Recovery Funding Board review.

Statement of Issue:
Public Works requests a modification of the scope of work, budget, and time for completion in the
agreement with Shearer Design, LLC.
Analysis/Strategic Goals/Pro's & Con's:
The culvert at West Uncas Road MP 0.804 has been a chronic fish passage problem requiring yearly
maintenance for over 5 years. Should this culvert fail it will significantly impact the substantial
restoration work that has been completed to the north where Salmon Creek joins South Discovery Bay.
This supplement to the agreement with Shearer Design LLC is for design modifications required by the
Washington State Dept. of Fish and Wildlife.
Fiscal Impact/Cost Benefit Analysis:
The maximum amount payable for this supplement is $26,739. The maximum amount payable for the
Shearer Design agreement will increase from $76,241 to $102,980. This project is funded at 85% by a
Recreation and Conservation Office (RCO) Salmon Recovery Funding Board grant. Remaining funding
will be through a combination of a U.S. Fish and Wildlife Service National Fish Passage Program grant,
and local funds.
Recommendation:
The Board is asked to execute the three (3) originals of the Supplemental Agreement with Shearer
Design, and return (2) signed originals to Public Works.
