Chair David Sullivan called the July 21, 2016 meeting of the Jefferson County Board of Health
to order at 2:30 p.m. A quorum was present.

APPROVAL OF AGENDA
Chair Sullivan asked for approval of the Agenda of the July 21, 2016.
Member Kathleen Kler motioned to approve the agenda. The motion was seconded by
Member Sheila Westerman. No further discussion. The motion passed unanimously.
APPROVAL OF MINUTES
Chair Sullivan asked for approval of the minutes of the June 16, 2016 meeting of the Jefferson
County Board of Health.
Member Catharine Robinson asked that a transposition on page 3 be corrected, changing the
word "Framers" to "Farmers." Member John Austin asked that the homophones "hurtle and
hurdle" on page 4 be corrected to "hurdle."
Chair Sullivan asked for approval of the minutes of the June 16, 2016 meeting of the Jefferson
County Board of Health as amended.
Member John Austin motioned to approve the minutes as amended; the motion was
seconded by Member Kathleen Kler. No further discussion. The motion passed
unanimously.

There was no public comment.
OLD BUSINESS AND INFORMATIONAL ITEMS
1. World Breastfeeding Tea
Julia Danskin, Public Health Nurse Il, Jefferson County Public Health shared with Board
members the`upcoming picnic to be held at Chetzamoka Park on Wednesday, August 3rd from
11:00 — 12:30 in celebration of World Breastfeeding Week. Danskin said the picnic is part of
JCPH's successful, weekly breastfeeding tea. This is a celebration of sustainable nutrition and
provides support and encouragement to breastfeeding mothers. Danskin also mentioned
breastfeeding policy JCPH has adopted and has encouraged the County as a whole to adopt. As
well, she told the Board under the Affordable Care Act insurance companies have to pay for
breast pumps for nursing mothers. This is information JCPH WIC and nutrition staff share with
clients. The CHIP groups are working, in association with their health lifestyles priority, to
encourage business to support mothers and their nursing babies in the workplace and community.
NEW BUSINESS
1. Food Fee Schedule Revision
Jared Keefer, Environmental Health Director, Jefferson County Public Health summarized for
the Board proposed changes to the Food Fee Schedule structure. This proposal, as its
development continues, has been before the Board for its ongoing consideration. For the Board's
convenience, Mr. Keefer presented the proposed policy edits and presented the proposed fee
schedule alongside the line -in / line-out edits made to the existing fee schedule. Essentially, the
changes will reclassify food establishment fees based on risk factors. There will be three permit
categories, multiple permits in restaurants will be replaced with permit endorsements, inspection
frequency will be more narrowly focused, based on risk category. Letters were sent to food
establishments informing them of proposed changes and post -letters will be sent explaining the
new fee schedule, should it be adopted. In April an informational meeting was held to assist and
solicit suggestion from food establishment proprietors concerning the proposed changes.
Keefer asked that a public hearing be held at the August 181h, 2016 Board of Health meeting
Member Catharine Robinson motioned to propose a call to public hearing for Food Fee
Schedule Revision and Policy adoption. Member John Austin seconded the motion. The
motion passed unanimously.
Chair Sullivan motioned to hold a public hearing for Food Fee Schedule and Policy

adoption at the August 181h, 2016 Jefferson County Board of Health meeting. Member
Austin seconded the motion. No further discussion. The motion passed unanimously.
2. On-site Sewage System Inspection Reminder Notices
Mr. Keefer reviewed the ongoing update of the On-site Sewage Code (OSS), which began in
2011, was adopted in 2012, and has now been implemented. The new code allows for
homeowner inspections, provides training and certifications, and created an on-line system for
recording inspection results. Keefer explained next steps to the Board: letters were sent to
residents with septic systems on July 16 explaining the inspection frequency requirements,
whether or not the county has record of monitoring inspections having been performed,
informing residents inspection is required within 60 days. Mr. Keefer provided the Board with a
timeline for monitoring inspection reminders.
Keefer reminded the Board, as we go forward in implementing the OSS Code, that the annual
flat fee for septic systems would be collected via property taxes. In order to accommodate this,
the Board needs to approve proposed changes to the On-site Sewage Management Fee Schedule
before December, 2016,
3. Opiate Epidemic Response — Federal and Regional Actions
Dr. Tom Locke, Health Officer, Jefferson County Public Health, updated the Board on the
ongoing, rapidly evolving response to the national epidemic of opiate use. Congress recently
enacted the Comprehensive Addiction and Recovery Act (CARA). No Federal funding was
attached to the passage of this Act but have been promised when Congress reconvenes after their
summer recess.
Locke stressed to the Board one of the major steps forward of the Act is that it redefines opiate
addiction to be primarily a public health problem rather than a criminal justice problem.
Understanding Opiate Use Disorder as a physical disease process helps to reduce stigma related
to this particular addiction. Actions are being taken locally to expand the availability of overdose
antidotes. The move to encourage proponents of abstinence only addiction rehabilitation
programs to understand how the recidivism rates reflect the need for more effective treatment
programs takes time. Prescribing and monitoring naltrexone, methadone, and buprenorphine use,
as well as engaging behavioral counseling, are scientifically, medically sound approaches to
opiate addiction. There is Medicaid funding for MAT (Medication -Assisted Treatment)
programs, which are more effective than abstinence -only programs. Locke said there is an urgent
need for more MAT providers, and suggested Jefferson County certify more providers in opioid
treatment programs.
4. Community Health Improvement Plan Update
Vicki Kirkpatrick, Director, Jefferson County Public Health, updated the Board on ongoing
Community Health Improvement Plan (CHIP) milestones. The sub -committees have established
goals, activities, and strategies. The next steps in the process are to finalize the Plan, meet with
key stakeholders, present the Plan to the community, and delegate CHIP implementation. One of
the goals the Community Health Improvement Plan hopes to achieve is communitywide
ownership of the Plan; the Plan needs to become part of the community fabric.
Jill Buhler, Hospital Commissioner, urged Kirkpatrick to set a date for a joint meeting of the
Stakeholders. This meeting will likely take place in September or October.
ACTIVITY UPDATE
Chair Sullivan reported to the Board a vacancy on the OlyCap Board of Directors. Rich
Ciccarone is stepping down as chair. Sullivan urged Board members to help recruit a new
member.
Michael Dawson, Water Quality Manager, Jefferson County Public Health, told Board members
there were two press releases issued over the past week. A sewage spill occurred in Port Ludlow
and Port Townsend Bay was closed due to marine biotoxins.
Jared Keefer said Environmental Health is planning its upcoming budget and expects to add
additional staff. Keefer also mentioned permit applications have increased, and cautioned that
there is much client anxiety surrounding the septic inspection letters that were mailed July 10h.
Member Kler commended Vicki Kirkpatrick on her excellent presentation at the Community
Conversation on Youth Mental Health and Substance Abuse, which was hosted by United Good
Neighbors on July 14th at the Port Townsend Carnegie Library.

AGENDA PLANNING CALENDAR
John Austin will not attend the August 18th, 2016 meeting of the Jefferson County Board of
Health but asked if there might be a presentation on the Olympic Community of Health's
progress. Dr. Locke said Elya Moore, Director, Olympic Community of Health, would be the
ideal person to brief the Board on regional health system reform efforts. It was decided Public
Health would invite Ms. Moore to update the Jefferson County Board of Health.
NEXT SCHEDULED MEETING
The next Board of Health meeting will be held on Thursday, August 18th, 2016 from 2:30 — 4:30
p.m. at Jefferson County Public Health, 615 Sheridan Street, Port Townsend, WA.
