JEFFERSON COUNTY PUBLIC HOSPITAL DISTRICT NO.2 BOARD OF COMMISSIONERS

IN THE MATTER OF THE COMMUNITY
HEALTH IMPROVEMENT PLAN } Jefferson County RESOLUTION NO. 44-16
AND FINAL ADOPTION }.Jefferson County Public Hospital District No.2
RESOLUTION NO. 2016-20
WHEREAS, with the support of each of their respective Boards, Jefferson County Public Health and Jefferson
County Public Hospital District No.2 jointly conducted a community -wide health assessment and;
WHEREAS, Jefferson County Public Health and Jefferson County Public Hospital District No.2 brought together a
large, representative group of stakeholders to review the assessment data and;
WHEREAS, the stakeholders identified four top priority health issues facing Jefferson County residents. The
identified health priorities are: access to healthcare, chronic disease prevention, immunizations, and mental health
and chemical dependency and;
WHEREAS, Jefferson County Public Health and Jefferson County Public Hospital District No. 2, as primary
sponsors of the work, facilitated the creation and the work of four committees comprised of residents, leaders, and
organizations who developed goals, objectives, and specific strategies for each priority and;
WHEREAS, Jefferson County Public Health and Jefferson County Public Hospital District No. 2 leaders took the
significant amount of community work that was done and completed the strategic frameworks for each priority;
and
WHEREAS, Jefferson County Public Health and Jefferson County Public Hospital District No.2 leaders took the
tremendous and collaborative work created by our community and crafted a Community Health Improvement Plan
capturing that work and;
WHEREAS, the end goal of the work and the Community Health Improvement Plan is one of improving health —
not just by reducing illness, but by enhancing physical, mental and social well-being; and
WHEREAS, the Community Health Improvement Plan is a roadmap, a guide to help the Jefferson County
community impact the issues and barriers to better health and;
WHEREAS, the Community Health Improvement Plan is community -driven, data focused, and defines a vision for
health in Jefferson County and should guide community leaders and residents in making decisions about where to
invest time and resources to improve health and well-being and;
WHEREAS, the overarching goal of the Community Health Improvement Plan is to facilitate alignment of efforts
within the community, utilizing collective impact in order to make measurable differences in the health and well-
being of Jefferson County people, families, and communities;
NOW THEREFORE BE IT RESOLVED that the Jefferson County Board of Health and the Jefferson County
Public Hospital District No. 2 Board of Commissioners do hereby approve the Jefferson County Community
Health Improvement Plan and direct that Jefferson County Public Health and Jefferson County Public Hospital
District No. 2 continue to work with community organizations and individuals to increase awareness of the Plan,
encourage organizations to use the Plan and the priority Strategic Framework to guide decision-making that
impacts health and;
BE IT FURTHER RESOLVED that Jefferson County Public Health and Jefferson County Public Hospital
District No. 2 are directed to initiate and facilitate implementing the Community Health Improvement Plan
Strategic Framework including, but not limited to, serving as lead agency for strategies aligned with their
respective missions and seek lead agencies or individuals for the remaining strategies and;
BE IT FURTHER RESOLVED that Jefferson County Public Health and Jefferson County Public Hospital
District No. 2 will jointly develop an on-going monitoring process that collects data and measures progress on
meeting agreed upon metrics included in the Strategic Framework and report on progress to the Jefferson County
Board of Health, the Jefferson County Public Hospital District No. 2 Board of Commissioners, and the community
at large; and
BE IT FURTHER RESOLVED that the Jefferson County Board of Health and the Jefferson County Public
Hospital District No. 2 Board of Commissioners recognize that community assessment and community health
improvement planning are on-going and that improved health and well-being depends upon continued commitment
to that goal.
