Dear AMERCO REAL ESTATE COMPANY:
We have received the above referenced application for review. We are in need of additional
information and/or items as listed below to continue our review of the proposal:
Septic Issues, need to address: 1) ID on building plot plan the building that has
the proposed bathroom, 2) show on the building plot plan the proposed waterline,
3) show sewage transport line on the building plot plan, 4) contact aseptic system
designer and apply for a modification permit to add transport line to existing septic
system.
If a Monitoring Inspection of the onsite sewage system is required please contact a certified
O&M Specialist or Licensed Designer to schedule the inspection. A list of these providers
can be found at our website www.ieffemoncountvoubllchealth.ora Please notify this office
when the Inspection has been completed.
Please submit the necessary items in order for us to complete the plan review process of this permit.

David, I got your message this morning regarding the well decommissioning question and Frank's comments to you. The
USR case is accurate and was updated on 9/6/13 the same day of the inspection for decommissioning the well, so I don't
know the problem you referenced in your message. Maybe Frank can clarify for me.
I also signed off the potable water portion of BLD13-20 based on the completed well decommissioning on the same day,
9/6/13. As far as I am concerned the tenant improvement permit, BLD33-20, has complied with the drinking water
portion of the case.
The other two cases are pending my review and may need to be forwarded to DOH for, their review based on my
comments during the pre -app regarding the lack of sanitary control within 100' of the active well. Unless your proposed
new storage units and roadways are 100' away from the active well, DOH review will be required.
Let me know if anyone needs further clarification.

Hi Susan,
The building applicant will need to submit an new Sanitary Control Variance request to us regarding the new
contaminants within the SCA. The SCA variance needs to include a well log, justification of reduction and stamped by a
Civil Engineer.

POTABLE WATER ISSUES
1/15/14 tc with DOH, no information submitted to them for variance of 100' protective radius to date.
10/31/13 await confirmation from DOH that the 100' protective radius around the single active well is
approved with buildings and interior roads.
7/3/13 met in office with Cary, archetct for the project and explained what has been necessary for
potable water since the first pre-app on the site and prior to the first building permit. DCD took in the
application with inadequate informtion for potable water review.
