HomeMy WebLinkAboutDevelopmental Disabilities Community Access Programs (PDF)8/12/2011 Jefferson County Public Health – Planned Performance Measures 2012 Community Access Programs MISSION: The mission of the Developmental Disabilities Community Access Program is to assure that citizens with Developmental Disabilities in Jefferson County have the choice, opportunity and support to achieve inclusive and productive participation in community life. SERVICE: Community Access services are individualized & provided for individuals in retirement, in typical integrated community settings. Services will promote the persons’ competence, integration, physical or mental abilities. Services assist individuals to participate in activities, events and organizations in the community in ways similar to others of similar age. These services may also be available for working age individuals for whom an Exception to Rule has been approved. GOALS FOR FY 2012 1. To provide services that assist people with disabilities to participate in community activities, events and organizations in ways similar to others of retirement age. 2. Community Access will base instruction on the goals established in a Person Centered Future planning/profile delineating individual skills to provide training and support in social, communication and volunteer/job skills that are essential to the person’s quality of life. OBJECTIVES FOR FY 2012 1. Increase the numbers of opportunities to contribute and participate in the community activities, events and organizations through voting, civic groups, and other clubs etc. Progress will be documented in the client’s files, i.e. Person Centered Future planning/profile. 2. To increase opportunities for community contribution through volunteer experiences, with the staff person working in the background as a consultant, in order for community relationships to develop. Staff will assess client’s interest in volunteer experiences on a monthly basis. PERFORMANCE INDICATORS: 2010 Actual 2011 Projection 2012 Projection Number of persons in Com Acc that have been served, are currently being served and/or we are planning to serve 3 People 3-4 People 4 -5 People Average Outings of Service Per Month 4-10 per client 4-10 per client 4-8 per client Average Hours of Services Per Month 20 hours per client 20 hours per month 20 hours per client Total Number of Outings 229 280 360 SUMMARY OF KEY FUNDING/SERVICE ISSUES: With the advent of the Working Age Adult Policy, the Developmental Disabilities Community Access Program changed to service for people with developmental disabilities over 62 who are retired or for individuals under age 62 with a rare exception. The legislative session of 2011 found many parents upset with the Working Age Adult Policy & so Community Access may indeed become a program option for clients who are of working age, (21-62 years of age). The Community Access Program is about Community Participation, Skill Development & Coordination. It showcases a discovery component that looks at identifying people and places within the local community where an individual’s interest, skills, culture, talent, gifts can be contributed and shared with others with similar interests. The Program focuses on developing a balance of supports to promote independence, personal growth and/or engagement in the following areas: socialization, education, recreation and personal development. Community Access staff roles continue to evolve to one of consultant or guide to the individual rather than only as accompanied staff. It is in the best interest of Jefferson County, and society in general, that all people deserve to have the choice, opportunity and support to achieve an inclusive and productive participation in community life.
