MISSION: The mission of the Developmental Disabilities Community Information & Education
Program is to assure that citizens with Developmental Disabilities in Jefferson County have the choice,
opportunity and support to achieve inclusive and productive participation in community life.
GOALS FOR FY 2012:
1. Costs incurred by the program for activities to inform and/or educate the general public about
developmental disabilities related services.
2. To promote activities aimed at promoting public awareness and involvement, community
consultation, capacity building, and organizational activities.
3. To provide information, education, emotional support and referrals to families of children and
professionals who work with these families.
4. To promote parent and professional partnerships and relationships in Jefferson County.
OBJECTIVES FOR FY 2012:
1. DD County Coordinator will support and maintain strong working relationships and open lines
of communication with County Providers.
2. Provide opportunities for broader community input and involvement in DD services by educating
and informing the community with publicity about issues through the media, local newsletters, a
community forum, workshops/trainings and a dedicated spot/time for public comment at every
Board meeting.
3. Support the local People First Chapter for Adults to: Broaden membership through outreach
efforts aimed at future members provide training and/or individualized mentoring in self-
determination, self-advocacy, and other skills as requested by the Chapters, provide outreach, &
education at a Jefferson County Event.
4. The County DD Program will continue to support the Jefferson County Transition Network
Group to meet 7-9 times a year. The Transition Network will review gaps, needs and services in
order to build goals and objectives that will create a successful Jefferson County off-site
Transition Program, while establishing and maintaining successful communication and working
relationships between Schools Districts, Parents, DDD, Employment Vendors, Residential
Providers, the County DD Program, DVR, Work Source and any other interested
Agency/Organization.
5. Continue positive networking relationships with the Port Townsend & Chimacum Special
Education Directors, by supporting effective High School Transition Programs, i.e. Project
SEARCH & the new Project TRANSITION Program.
6. DD County Coordinator will create and maintain a Transition Guide, a Transition Portfolio
Workbook and How to Create a Portfolio Workbook.
7. DD County Coordinator will work with the Transition Network to set up informational training
sessions or “Flight Teams” for school personnel, in order to teach them about the Working Age
Adult Policy/Pathway to Employment, Residential, Employment, Transition options and Adult
Service Systems.
8. DD County Coordinator will create and maintain an updated DD Resource Guide, and a Guide to
The Working Age Adult Policy and Pathway to Employment.
9. The County DD Program will continue to support a local Parent-to-Parent, (P2P) Program. The
P2P Program will to bring emotional support and education to parents of children/adult children
and/or individuals with Developmental Disabilities in Jefferson County; by gathering and/or
prepare, distribute materials about health and education related planning/services, policy
development, legislative activity, conferences, and workshops/trainings through emails updates
& monthly newsletters.
10. DD County Coordinator will conduct comprehensive onsite Program Evaluations; i.e.: Contractual
Outcome Attainment Issues, Program Planning and Evaluation Issues, Health Safety and
Regulatory Oversight Compliance Issues, Individual Rights Issues. Other impressions and
systems may be reviewed in an adjunct manner. Results of these Program Evaluations will be
available in a summary and/or data format.
11. DD County Coordinator will network and participate with local agencies, groups on Community
Information and Educational projects and inclusion strategies for Jefferson County: State
Supported Employment, Transition Services, Transportation, Housing, Community Health Care,
Prevention, Mental Health, Emergency Preparedness, Transition Issues, Birth to Three Services
& Community Building.
12. DD County Coordinator will support a Community Planning process and an Advisory Board
Retreat every four years when appropriate.
13. DD County Coordinator will update DD Advisory Board Manual and offer a copy of the Manual
to every DD Board Member. DD Coordinator will provide an overview of the Manual &
orientation of the DD Program to new DD Board Members and/or any Board Member who is
interested.
14. DD County Coordinator will maintain, educate and support the DD Advisory Board. The Board
will include, whenever possible, People with Disabilities, a Port Townsend City Council
Member, Jefferson County Commissioner, Parents, Residential Agency, Spiritual
Representative, DASH Member, Employers and varied Community representation as
participating members of the DD Advisory Board.
15. DD Coordinator will develop presentations to the Jefferson County Community, DD Board, and
Parent Groups on the subject of: Working Age Adult Policy, Transition Services, Service
Trends/Innovations in the System, the DD County System and the County/Board's perceived
roles in the future when appropriate.
16. DD County Coordinator (when financially feasible), will provide workshops/trainings for local
employment vendors, community providers, families, people with disabilities and school
personnel. Some of the subjects depending on the requests: Job Development, Marketing,
Training, Coaching Skills & Service Trends, Community Asset Building, Discovery Process
and/or Pathways to Employment.
17. The DD County Coordinator shall participate in local and statewide meetings that relate to
developmental disabilities, Jefferson County DD Board meetings, JICC meetings, Jefferson
County People First Group, Jefferson County Transition Network Group, the Emergency
Preparedness Group for People w/Disabilities, Association of County Coordinator (ACHS), and
various committees as appropriate. The DD Coordinator shall network with other groups through
out the County and State as appropriate.
18. The County Coordinator with local DD Providers will develop and implement a
community/education awareness campaign during National Disability Employment Month in
October. The campaign will value and appreciate the diversity that results from full participation
of people with disabilities in community life through employment.
19. The DD County Coordinator will work with the Prevention Team on the Poster/Coaster
Campaign by dispersing educational materials on the Number One Preventable Developmental
Disabilities: abstaining from alcohol when pregnant
SUMMARY OF KEY FUNDING/SERVICE ISSUES:
The Developmental Disabilities Community Information & Education Program improve the quality of
life for citizens with Developmental Disabilities, by delivering programs through alternative ways, that will
improve services within available resources and by ensuring those citizens have access to integrated services to
improve the quality of their lives.
It is in the best interest of Jefferson County, and society in general, that all people deserve to have the
choice, opportunity and support to achieve inclusive and productive participation in community life.
