HomeMy WebLinkAboutDevelopmental Disabilities Person to Person Program (PDF)1/31/2012 Jefferson County Public Health – Planned Performance Measures 2012 Person to Person Program MISSION: The mission of the Developmental Disabilities Person to Person Program is to assure that citizens with Developmental Disabilities in Jefferson County have the choice, opportunity and support to achieve inclusive and productive participation in community life. SERVICE: These services are a part of an individual’s pathway to individual employment. These time limited services and supports may be needed to assist people with: Initial Planning: System overview, development of a person centered employment plan, and/ or preparing an individualized budget, Technical Assistance Services: These services are provided by professionals specialized in an area that would benefit a client to move further on their pathway to Individual Employment. An example is assistive technology or a behavior specialist. Discovery: Job preparation, exploration and/or volunteering in the community to achieve integration and employment. GOALS FOR FY 2012: 1. If the person is between the ages of 21-62, a Person Centered Employment Plan/Pathway to Employment Plan and Action Step Plan must be completed for each participant to assist them along their “Pathway to Employment” to competitive gainful employment. 2. The “Pathways to Employment” plan shall support the individual to experience vocational assessments at community sites. Participation in community employment assessment sites will assist staff to evaluate person's vocational skills, and give clients the opportunity to make informed choices about future employment options with the ultimate goal to reach competitive employment in the community. OBJECTIVES FOR FY 2012: 1. That a Person Centered Plan/Pathway to Employment Plan is completed for each participant to include an Action Step Plan with specific activities that support the completion of the plan goals and are supported by the financial resources available. 2. That each individual will be supported to pursue his or her own unique path to work, a career, or his or her contribution to participation in community life while on a Pathway to Employment Plan within the allotted time frame. PERFORMANCE INDICATORS: 2010 Actual 2011 Projection 2012 Projection Number of persons in Person-to-Person that have been served, are currently being served and/or we are planning to serve 18 People 6-15 People 6-15 People SUMMARY OF KEY FUNDING/SERVICE ISSUES: An ongoing issue in Jefferson County for people with Developmental Disabilities is the need for more integrated employment opportunities in order to bring up their economic reach and inclusion into the community. The opportunity for people with disabilities to obtain gainful employment continues to be an essential priority for a number of reasons: The Division of Developmental Disabilities “Working Age Adult” Policy mandates that all people with a developmental disability between the ages of 21-62 must have a written Pathway to Employment which reflects the goals needed to pursue or maintain gainful employment in the community. Employment brings benefits to people in terms of opportunities to build relationships and to be seen as a contributing member of society. Society grants status to those with "spending power", along with the fact that expendable income allows additional personal choice & quality of life.
