Family Planning Access: a. Family planning services are available at Jefferson County Public Health (JCPH) main reproductive clinic as well as at 3 satellite clinics—Quilcene, Chimacum School-Based Health Center, and Port Townsend School-Based Health Center—and serve the reproductive needs of clients up to the age of 65 years. b. Outreach to JCPH clients and community regarding the Affordable Care Act continues to further increase accessibility to Family Planning Services and comprehensive health care. 2.) Teen Family Planning: a. JCPH continues providing confidential, respectful services for all clients, including teens and youth. Family planning services are provided in School-Based Health Centers at the Chimacum and Port Townsend High Schools 2 days per week, at each school. Total numbers in 2014 have decreased slightly from those in 2013. b. Quilcene students have authorization from school administration to access the health clinic across the street for services; the clinic is open once a week for family planning services. 3.) School-based Health Centers (SBHC): a. School-based Health Clinic services in Chimacum and Port Townsend schools began its 7th year in Fall of 2014. Medical clinical services are available 2 days per week in each of the schools for comprehensive medical services, including Family Planning. Advanced Registered Nurse Practitioners (ARNP) also staff Quilcene clinic, which, as mentioned above is across the street from the Quilcene School, once per month for services available to students as well as the community at large. Jefferson Healthcare funds ARNP services at the Chimacum High School. b. 39% of Chimacum students and 29% of Port Townsend students utilized the SBHC in the 2013-14 school year with a combined total of 649 visits made. Chimacum is a rural area without easy access to medical clinics while the Port Townsend High School is located in a town with access to 6 medical clinics within 2 miles of the school. Chimacum SBHC is staffed with medical providers 12 hours per week; Port Townsend is staffed 8 hours per week. (Unduplicated student totals are taken from 2013-14 school year rather than the 2014 calendar year.) c. JCPH contracts with Jefferson Mental Health to provide mental health services to students in the SBHC 2 days per week at both the Chimacum and Port Townsend High Schools. Chimacum and Blue Heron (PT) Middle Schools also have 2 days of mental health provided. Quilcene school district is provide with 1 day of mental health per week. (This is a subcontract made available by the MHCD Jefferson County sales tax.) JCPH continues to provide women’s preventive screenings for women 40-64 years of age through the Breast and Cervical Health Program (BCHP), which includes undocumented residents. b. Educating clients about the Affordable Care Act and assisting with enrollment, has been a focus of JCPH since mid 2013. Currently, a significant reduction in the number of clients using BCHP is noted, due to the increasing number of clients who are now covered by a comprehensive health insurance plan. 5.) Foot Care: a. JCPH has been providing Foot Care Services throughout East Jefferson County at Community Centers, Adult Care Facilities and home visits for over 20 years. b. From Jefferson County’s Community Health Assessment – Jefferson County residents’ Median age is 55.1 in 2012. Median age divides a population into two equal halves so that 50% of the residents in a given place are older, and 50% are younger. Since 2010, Jefferson County’s median age has been the highest in Washington State. In 2012, Jefferson median age is 17 years higher than Washington State. c. The JCPH Foot Care Program helps this population maintain independence and mobility by providing foot maintenance at community centers and in homes. Foot Care services provide what a client can no longer perform for themselves, such as filing calluses, cutting nails, and tending to sores resulting from diabetes. d. Demand for Foot Care fluctuates based on need. Foot Care Staff provide outreach material to Health Care Providers, Community Centers and Adult Care communities.
