STATEMENT OF ISSUE:
Jefferson County Public Health, Community Health, requests Board approval of Amendment No. 1 to the
Agreement with Kitsap Public Health District for Nurse Family Partnership; July 1, 2017— June 30, 2018
ANALYSIS STRATEGIC GOALS/PRO'S-and COWS:
The purpose of this amendment is to ensure that Kitsap County Public Health District (KPHD) can provide
data from the NFP data collection system to authorized staff at DOH in order to meet federal reporting,
program evaluation and planning requirements and objectives.
FISCAL IMPACT/COST BENEFIT ANALYSIS:
There is no change to funding.
RECOMMENDATION:
JCPH management request approval of Amendment No. 1 to the Agreement with Kitsap Public Health District
for Nurse Family Partnership; July 1, 2017— June 30, 2018

This Amendment ("Amendment") to the CONTRACT AGREEMENT for Professional
Services for Nurse Family Partnership staff, training, and supervision through the
Department of Early Learning (DEL) grant, ("CONTRACT") is entered into between
Jefferson County Public Health ("COUNTY") and Kitsap Public Health District
("CONTRACTOR")
RECITALS
WHEREAS, the parties entered into the Agreement effective July 1, 2017; and
WHEREAS, it is necessary to amend the language in the Agreement;
NOW, THEREFORE, THE PARTIES AGREE AS FOLLOWS;
Amendment of the Contract Agreement Section 10. The following paragraph
will be added to the beginning of the section: "CONTRACTOR will enter into a data
sharing agreement with DOH. Contractor will send a release letter to NFP National
Service Office instructing them to release ETO data to DOH.
Other Provisions Unchanged: Further provisions of this CONTRACT
AGREEMENT remain unchanged.
Effective Date: "This Amendment is effective retroactive to July 1, 2017
