ALLEGATION: Delay in receiving a building permit for house construction.
DISCUSSION: In response to a letter from the claimant requesting reconsideration of the claim, the Risk
Manager and Deputy Prosecuting Attorney agreed to offer a settlement payment of a portion
of the claim in the amount of $8,100.
DECISION: The Risk Manager and Deputy Prosecuting Attorney recommend that a reduced amount of
$8,100 be paid.

DAMAGES:
$1,298.33
ALLEGATION: Damage to electrical equipment.
DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim
finding that road crew personnel was performing brush cutting operations in the area where
the incident occurred and verified the damages sustained.
DECISION: The Risk Manager and Deputy Prosecuting Attorney recommend the claim be paid.

ALLEGATION: Damage to electrical equipment.
DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim

incident occurred and verified the damages sustained.
DECISION: The Risk Manager and Deputy Prosecuting Attorney recommend the claim be paid.

Unnecessary expenses incurred due to Health Department staff's incorrect interpretation of
requirements.
DISCUSSION:
The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim and
determined that Health Department staff's incorrect interpretation of the requirements caused
unnecessary expenses and therefore, warrants partial payment of this claim.
DECISION: The Risk Manager and Deputy Prosecuting Attorney recommend that a reduced amount of
$2,046.05 be paid.

ALLEGATION: Damage to vehicle window.
DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim
finding Animal Services personnel was performing grounds maintenance in the area where the
incident occurred and verified the damages sustained.
DECISION: The Risk Manager and Deputy Prosecuting Attorney recommend the claim be paid.

ALLEGATION: Unlawful arrest and imprisonment
DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim
finding that Mr. Plachta was held in custody for a period of 5 days on warrants which had
expired. Therefore, a payment of approximately $500 for each day of confinement is
warranted.
DECISION: The Risk Manager and Deputy Prosecuting Attorney recommend that a reduced amount of
$2,500 be paid.

DAMAGES:

ALLEGATION: Improper towing of vehicle.
DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim
finding that partial payment of the claim is warranted.
DECISION: The Risk Manager and Deputy Prosecuting Attorney recommend that only the towing fee of
$61.51 be paid.
