DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim. Action was tabled pending further investigation. DECISION: Pending CLAIMANT: Qwest CLAIM #: C-33-03 DAMAGES: $95.35 ALLEGATION: Damage to Underground Service Wire. DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim finding it is the obligation of the claimant to bury their underground service equipment to proper depth standards. The Public Works department verified that the underground service line, which was damaged during routine road grading, was buried at a depth of 4 to 6 inches. The required minimum burial depth is 30 vertical inches as outlined in section 13.56.35(b) of the Jefferson County Code Title 13 Utilities. DECISION: Recommendation to deny claim. DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim. Action was tabled pending further investigation. DECISION: Pending CLAIMANT: Sheri Couch CLAIM #: C-35-03 DAMAGES: $92.00 ALLEGATION: Tripped on Courthouse Interior Stairs Causing Physical Injury to Right Knee. DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim for reimbursement of the ambulance transport fee. The Washington Counties Risk Pool has hired an independent adjuster to investigate this claim. Action was tabled pending further investigation. DECISION: Pending CLAIMANT: Rob Gruye', Middlepoint Associates, LLC CLAIM #: C-36-03 DAMAGES: $256.25 ALLEGATION: Window of Building was Damaged When Hit by a Baseball During an Adult League Baseball Game at Memorial Field Stadium. DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim. The Public Works Recreation Division verified that the damages resulted from an athletic activity which was sponsored by Jefferson County. DECISION: Recommendation to pay claim. Risk Management Committee Minutes - October 29, 2003 Page: 3 CLAIMANT: CLAIM #: DAMAGES: ALLEGATION: Bruce Glenn C-37-03 $685.18+ Vehicle Damage DISCUSSION: The Risk Manager and Deputy Prosecuting Attorney reviewed the above referenced claim. Based on the circumstances of the incident, Jefferson County is responsible to pay for the repair of the claimant's vehicle. DECISION: Recommendation to pay claim, pending the receipt of further documentation of expenses incurred. Meeting adjourned.
