— Concerned Citizens for Community Inclusion Services; July 1, 2018 —
June 30, 2019; Additional $10,300 for a total $20,837
STATEMENT OF ISSUE:
Jefferson County Public Health, Developmental Disabilities Division is requesting Board approval of
Amendment #1 to the Professional Services Agreement with Concerned Citizens, to provide Community
Inclusion Services; July 1, 2018 — June 30, 2019; Additional $10,300 for a total $20,837
ANALYSIS STRATEGIC GOALS/PRO'S and CON'S:
This amendment extends the Professional Services Agreement with Concerned Citizens for Community
Inclusion Services from June 30, 2018 to June 30, 2019 and adds an additional $10,300 to continue
services that are tailored to individual needs, interests, and abilities of adults with developmental
disabilities. These individualized services are necessary to help adults with developmental disabilities
connect and contribute to their community.
FISCAL IMPACT COST BENEFIT ANALYSIS:
Funding for the agreement with Concerned Citizens is through the Developmental Disabilities
Administration with DSHS and is identified in the budget under Community Inclusion Services.
RECOMMENDATION:
JCPH management requests approval of the Amendment #1 to the Professional Services Agreement —
Concerned Citizens for Community Inclusion; July 1, 2018 — June 30, 2019; Additional $10,300 for a total
$20,837

Contract Amendment #1

WHEREAS, Concerned Citizens (Subcontractor) and Jefferson County (County) entered into an agreement on
July 1, 2017 for Professional Services to be provided in connection with the provision of Community Inclusion
Services to Persons with Developmental Disabilities in Jefferson County.
WHEREAS, the parties desire to amend the terms of that agreement.
IT IS AGREED BETWEEN BOTH PARTIES AS NAMED HEREIN AS FOLLOWS:
The program title will be changed from "Community Integration Services" to "Community
Inclusion Services."
2. The term of the above referenced agreement ends June 30, 2019.
3. Subcontractor's contract will be increased by $10,300.00 in funding for services rendered during
the term of July 1, 2018 through June 30, 2019. Total compensation under this Agreement shall
not exceed $20,837.00 without express written amendment signed by both parties.
4. Work performed between July 1, 2018 and the execution of this Agreement that is consistent with
the provisions of this Agreement is hereby ratified.
5. All other terms and conditions of the agreement will remain the same.
