STORMWATER POLLUTION PREVENTION PLAN
Project Description
The proposed project includes installation of approximately 20 miles of fiber optic cable
from the intersection of Highway 101 and Clearwater County Road to an existing cable on
the Hob/Clearwater Mainline near the Olympic Correction Center. Provided below is a
_ description of the fiber optic cable installation and a description of construction practices that
will occur along the alignment. A location map of the project is shown on attached Sheet 1.
- The fiber opticcable will cross four wetlands, three rivers and eleven creeks: Queets River,
Donkey Creek, Hurst Creek, Wildcat Creek, Hunt Creek, Waring Creek, Elkhorn Creek
' Shale Creek, Clearwater River, SOB Creek, Christmas Creek, Peterson Creek, Crook Creek,
- and Snahapish River. To avoid directly impacting these water bodies, the fiber optic cable
will be place overhead, through a conduit attached to an existing bridge, or placed above the
culvert in the road fill.
Table 1 lists each creek and river crossing as well as the cable placement across each water
_- body. Table 2 lists each wetland crossing. Attached Sheets 2 - 18 show the plan views and
cross sections of each waterway crossing. Silt fencing will be erected between the
construction and the wetland, creek bank or riverbank to prevent the introduction of silt or
... debris into the water. See attached Sheet 19 for a typical silt fence detail.

There are also several unnamed tributaries that will be crossed along the proposed route. In
these cases, the cable will be placed above the culvert in the road fill to avoid directly
impacting these water bodies. Silt fencing will be erected near the creeks to prevent the
introduction of silt or debris into the water.
The principal water quality and fishery concern for the fiber optic cable installation is the
-- release of sediments to downstream creeks and rivers. The temporary erosion and sediment
control plan is aimed at preventing sediment from entering downstream surface water bodies.
Construction
Several steps will be taken to install the proposed fiber optic cable along the Clearwater
County Road and the Hoh/Clearwater Mainline developed road right-of-way. The primary
steps include: staking; trenching; installation and backfilling; resurfacing and revegetation.
_ Construction is proposed to begin April 2002 and will last until approximately July 2002.

-- · Staking: The center line of the fiber optic cable will be clearly staked or otherwise
marked prior to construction. Stakes and markings will be maintained throughout
construction.
· Trenching, Installation and Backfilling: The fiber optic cable will be installed using a
~. self-propelled plow. The plow includes material and labor to trench, install and
backfill the fiber optic cable. The'cable will be covered with 42 inches of cover. The
trenching spoils will be hauled away. Any stockpiled trench spoils will be covered or
will be surrounded by a filter fabric fence, if appropriate. Standard erosion control
practices will be employed during trenching operations and construction activities.
Where trench dewatering is necessary, appropriate BMP's will be used to minimize
- erosion and prevent sediment from entering waterways. Free flow of water into
trenches will be'avoided. Immediately following trenching, the fiber optic will be
placed in the bottom of the trench. After the fiber optic cable is installed, the trenChes
-' will be backfilled with sand and gravel material as specified by the Washington
Department of Natural Resources and the Washington Department of Transportation.
No foreign substance such as brush, trees or refuse will be permitted in the backfill.
Any stockpiled backfill material will be covered or will be surrounded by a filter
fabric fence, if appropriate. See Sheet 19 for a typical trench detail.
· Resurfacing and Revegetation: In the last step of the process, all temporary
construction debris will be cleared and disposed of at an approved upland disposal
,- site, and erosion control measures will be implemented. Disturbed areas will be
either resurfaced or revegetated to original or better condition. Cleanup, resurfacing
and revegetation will follow guidelines as required by the Washington Department of
-- Natural Resources and Washington Department of Transportation. Details of these
requirements are discussed in the following section.
-- General Requirements For Erosion Control, Revegetation and Resurfacing
· Cover measures: No exposed, stripped soils may remain unworked, unstabilized,
unprotected, or uncovered for more than 7 days. Where feasible, no more than 500
feet of trench shall be open at one time. Mulch shall be used for stabilizing most
_ temporarily exposed soils. Trench spoils shall be removed from the construction site.
· Mulch: Mulch shall be usedfor immediate temporary erosion protection. Mulch shall
-- also be used as a pre-application before reseeding to conserve soil moisture and help
plants grow; hold fertilizer, seed, and topsoil in place; and moderate soil temperatures.
Mulch types approved for the project include: wood cellulose fiber bark or wood
-- chips, sawdust, vermiculite/perlite/pumice, tackifier compost, and soil amendments.
See the Washington Department of Transportation and the Department of Natural
Resources requirements for more detail.

-' · Hydro seeding: Hydro-seeding operations shall be performed within 14 days after the
soil was originally disturbed. Hydro seeding shall not be applied when the ground is
frozen, snow covered, or muddy. The Contractor shall coordinate the schedule with
the weather forecasts so that the hydro seeding has the greatest chance of survival.
Hydro seeding operations may only take place on rainless days when no rain is
_ forecasted for the following two days. If rainfall does occur on either of the two days
after an area has been seeded, the area shall be inspected by the Contractor for
damage. Hydro seed 'shall be reapplied to all damaged areas on the first appropriate
-- day after rainfall. If for any reason the hydro seed cannot be reapplied within the time
limits specified for bare soil, another cover measure shall be used during the interim
(i.e., mulch).
The hydro seed shall consist of seed, fertilizer, mulch, and tackifier as specified in the
Washington Department of Natural Resources and the Department of Transportation
-- erosion control requirements.
_ · Perimeter Protection: At each river and creek crossing, a silt fence shall be installed
downgradient of the trench. The silt fences shall be installed according to the details
shown on Sheet 19.
Silt fences shall be inspected each day and repaired as necessary to conform to the
design specifications or upon notification of deficiency. Repair shall be completed
- shortly after notification of a deficiency.
· Dust Control: Proper dust control measures shall be used by the Contractor to prevent
-' wind erosion and off-site sediment from wind. Mulch and grass seeding will suppress
dust generation from most areas. Frequent inspection of mulched and grass seeded
areas shall be performed by the Contractor to ensure that dust suppression is
occurring.
Water may be used by the Contractor for dust control. When water is applied to
suppress dust, the ground surface shall become wet but not sufficiently wet to
generate runoff. Oil is inappropriate for dust control.
· Maintenance: The Contractor shall maintain, repair, and replace all erosion and
sediment control devices such that the system is in good working condition for the
- duration of construction. The erosion and sediment control measures are to be
maintained in such a manner that the measures shall be viable for a reasonable period
of time after the project is complete.
· Restoration: The Contractor shall make every effort during construction not to harm
land and disturb vegetation or road surfaces. If either is disturbed so that it is not
capable of returning to its original condition, the Contractor shall to reseed or
resurface as necessary. To revegetate the disturbed area, the Contractor shall use the

-- appropriate seed mixture as specified by the Department of Natural Resources. To
resurface the disturbed area, the Contractor shall follow the standard specifications
required by the Washington Department of Transportation.
General Maintenance
Signs shall be maintained and/or replaced when damaged by the Owner of the fiber optic
cable. 'The Owner shall be responsible for wind and slide damage to the fiber optic cable.
_ The Owner shall also be responsible for relocating the fiber optic cable due to a road
relocation or culvert replacement.
