These services included extra days for pile driving inspection, additional engineering time to evaluate foundation capacity, and additional engineering time for bridge girder shop drawing review. SUPPLEMENTAL AGREEMENT The Local Agency of Jefferson County Public Works hereby amends the agreement with Shearer Design LLC ("the Agreement"), executed on October 27, 2014 (and later amended through Supplemental Agreement(s) 1 and 2, executed on May 23, 2016 and March 23, 2018.) All provisions in the Agreement remain in effect, except as expressly modified by this Supplemental Agreement. The changes to the Agreement are described as follows: II Section II, SCOPE OF WORK, is hereby amended by adding extra days for pile driving inspection, additional engineering time to evaluate foundation capacity, and additional engineering time for bridge girder shop drawing review. V Section V, PAYMENT, shall be amended as follows: Task No. 7, Construction Support, will have a revised maximum amount payable of$19,962.00. The new, total maximum amount payable will be $122,942.00, as shown below. Analysis/Strategic Goals/Pro's Et Con's: This supplement to the existing scope of work between Jefferson County and Shearer Design LLC is for construction support services for the Salmon Creek Bridge at West Uncas Road MP 0.804. At the request of Jefferson County, Shearer Design provided construction support services that exceeded the scope of the construction support services identified in Supplemental Agreement No. 2. These services included extra days for pile driving inspection, additional engineering time to evaluate foundation capacity, and additional engineering time for bridge girder shop drawing review. Fiscal Impact/Cost Benefit Analysis: The maximum amount payable for this supplement is $2,616.00. The maximum amount payable for the Shearer Design LLC agreement will increase from $120,326.00 to $122,942.00. This project is funded at 87% by the Salmon Recovery Funding Board, with the remaining 13% funded by a $100,000 National Fish Passage Program grant and the County Road Fund. Recommendation: The Board is requested to sign all three originals of the Supplemental Agreement with Shearer Design LLC, and return two originals to Public Works.
