No. 601508, 1801976 Statement of Issue: Authorization of Change Order #1 for the South Discovery Road Pavement Preservation Project, 1801976 with Lakeside Industries, Inc. in the estimated amount of $2,400.00. Analysis/Strategic Goals/Pros Et Cons: This change order is necessary to pay for costs for revising 3 private driveway approaches adjoining the project, by placement of Reclaimed Asphalt Pavement. Fiscal Impact/Cost Benefit Analysis: The cost of this change order is estimated to be $2,400.00 and 0 working days. Recommendation: Public Works recommends that the Board authorize Change Order No. 1 and return two signed originals of each to.Public Works for further processing. Li 5lgrmrore Uoie tjroi¢cr M4tM14 yte- Tide Consent given by Surely:(when required) Project Title South Discovery Road Pavement Preservation By Ailornev-In-lac, Dole You are ordered to perform the following described work upon receipt of an approved copy of this change order: Place and compact Reclaimed Asphalt Pavement(RAP)along the back edge of three driveway approaches as follows: Driveway#16,102+30 right,address 4372 Driveway 1:105'long Driveway 2: 30'long Driveway#11.96+35 right,address 4250 Driveway 3: 27'tong Total length: 162',165'rounded The RAP material shall be placed and compacted in a strip 3 feet wide along the back of the driveway approaches,from approximately 2"deep at the approach tapering down to approximately 0"deep at the 3' width point. This work is estimated to require 1.52 CY,or approximately 3.2 tons of RAP, Price to include all materials,labor,and equipment,and markups. Traffic control will be paid by the project traffic control bid items,and not included in this price. The total estimated increase in the contract amount for this change order is$2,400.00. This work will be paid as new Bid Item#25 Reclaimed Asphalt Pavement. All work materials and measurement to be in accordance with the provisions of the standard specifications and special provisions for the tvre of construction involved.
