This Memorandum of Agreement (MOA) between the Quilcene School District (District) and Jefferson County Public
Health (JCPH) is entered into for the purpose of determining the feasibility of and planning and preparation for
establishing a School Based Health Center in the District.
WHEREAS, the District has expressed interest in establishing a School Based Health Center (SBHC) within its school; and
WHEREAS, JCPH has professional staff, expertise, and currently manages SBHCs in two other school districts in Jefferson
County; and
WHEREAS, it is important for the Quilcene community to learn about school based health centers and have
opportunities to ask questions and give input; and
WHEREAS, establishing a SBHC takes planning and preparation in addition to community outreach;
NOW THEREFORE, the parties agree to the following terms and provisions for community outreach, planning and
preparation for the establishment of a school-based health center in the Quilcene School District:
1. TERM OF AGREEMENT
This agreement begins July 1, 2019 and runs through December 31, 2019. Any work performed prior to full
execution of this Agreement is hereby ratified.
Both parties have the right to terminate this agreement at any time by giving ten (10) days written notice.
This Agreement may be amended by mutual written consent of both parties.
2. SERVICES AND SCOPE
JCPH will conduct community meetings in Quilcene to provide opportunities for residents to learn about school
based health centers and the health benefits associated with them. Community meetings will be in sufficient
numbers to assure optimal opportunity for community engagement. Community meetings will also be open to
residents of Brinnon.
JCPH will attend meeting(s) of the Quilcene School Board as recommended by the Quilcene School District
Superintendent and/or Quilcene School District Principal.
Concurrent with the community outreach meetings, JCPH will plan for the establishment of a SBHC including
ensuring adequate staffing, working with the District to determine day(s) and times the SBHC will be open for
students, determining the level of health care services to be provided, expanding the SBHC Advisory Committee
to include representatives from Quilcene, and any other preparatory activities necessary to establish a SBHC.

Upon final approval by the Quilcene School Board for establishing a SBHC, JCPH will formally put the SBHC in
place along with plans, mutually agreed to by JCPH and the District, for officially opening the SBHC.
3. PAYMENT
JCPH shall bill the Quilcene School District monthly for actual expenses. Invoices will include details of work
performed in support of incurred expenses.
Total compensation for the Outreach, Planning and Preparations (including any supplies deemed to be
necessary to carry out the Services and Scope of this MOA) shall not exceed $20,000.
Both parties agree that if total funding available is less than $20,000, JCPH work deliverables will be adjusted
accordingly so as not to exceed the funding available.

This Memorandum of Agreement contains all the terms and conditions agreed upon by the parties. No other
understanding, oral or otherwise, regarding the subject matter of this agreement shall be deemed to exist or to
bind any of the parties hereto.
5. INDEMNIFICATION
Parities to this MOA mutually agree to protect and hold harmless the other party and its elected and appointed
officials, officers and employees against all claims, losses, damages, suits and expenses except to the extent they
arise out of, or result from, the negligence or willful misconduct of the indemnitor or its elected or appointed
officials, officers, and employees in the performance of this Agreement.
6. MODIFICATION
This Agreement may be modified at any time by written agreement of all parties.

SUBJECT: Memorandum of Agreement with Quilcene School District
STATEMENT OF ISSUE: Jefferson County Public Health approached the Quilcene School District
to discuss the possibility of establishing a School-based Health Center (SBHC) in the District. The
response was strong support for establishing a SBHC. We were invited to talk with Quilcene
School District Board and the Board asked us to proceed with the process. Since JCPH has SBHCs
in the Port Townsend and Chimacum School Districts we have drawn upon that past experience to
plan for the creation of a new SBHC in Quilcene. One of the elements important to this project is
to ensure that the residents of the Quilcene and Brinnon areas have an opportunity to ask
questions, provide input and understand what a SBHC does and can provide.

ANALYSIS/STRATEGIC GOALS/PRO'S and CON'S: The Quilcene School District applied for
and received a grant from OSPI to assist us in our costs associated with community outreach and
other necessary items and steps needed to prepare for community meetings, work with
Superintendent Frank Redmon, and Principal Sean Moss and get ready to open a SBHC. They
have committed to remodeling a classroom for the clinic room and staff have been meeting all
along regarding the clinic specifications. Jefferson Health Care has donated some equipment and
committed additional funding to support the clinic once it is up and running. Community meetings
are being scheduled. A soft opening is in development and a plan for providing sports physicals as
the introduction to the SBHC is in the works.
Substantial work is involved in establishing the clinic. Once the preparatory work in the
community is done, the Quilcene School Board will make a final decision regarding opening a
SBHC in the Quilcene School District.

FISCAL IMPACT/COST BENEFIT ANALYSIS: The School District has received $10,000 from
OSPI and expects to receive another $10,000. The Memorandum of Agreement is ""not to exceed
$20,000" with a clause stating that if funding is less than $20,000, JCPH will adjust deliverables
accordingly.
RECOMMENDATIOS: JCPH is recommending approval of the Memorandum of Understanding
with the Quilcene School District to help offset costs to JCPH for the necessary preparation work.
