SUBJECT: Amendment to Memorandum of Agreement between JCPH and the
Quilcene School District extending the termination date
STATEMENT OF ISSUE:
Jefferson County Public Health (JCPH)entered into a Memorandum of Agreement(MOA)
that provides funding to JCPH for the community outreach work and other work necessary to
establish a School Based Health Center(SBHC). The current MOA is scheduled to end on
December 31,2019. It was written originally with the thought that the SBHC might open in
the Fall of the current 2019-2020 school year. The process for final school board approved
took longer than originally planned to ensure optimal opportunity for community and parent
input. Additionally,the construction of the clinic is not yet completed and all of the necessary
work by JCPH has not yet been completed.
ANALYSIS:
This request is to extend the MOA through July 31, 2020 to allow for getting the SBHC up and
operational and to cover any additional needs of the clinic discovered once the clinic has been
operational for a period of time. This is the only change to original MOA. Both the original
MOA and the Amendment are included with this request. The language change in the
Amendment is highlighted.
FISCAL IMPACT:
There is no fiscal impact associated with this Amendment.
RECOMMENDATION:
JCPH recommends approval of Amendment#1 to the Memorandum of Understanding
between Jefferson County Public Health and the Quilcene School District.

AMENDMENT#1
TO
MEMORANDUM OF AGREEMENT
Between
Quilcene School District
And
Jefferson County Public Health
Dated: July 1, 2019—December 31, 2019
The Purpose of this Amendment is to extend the Memorandum of Agreement (MOA)to July 31, 2020 for
ongoing work to establish and open a School Based Health Center (SBHC) within the Quilcene School District.
The MOA is hereby amended as follows:
Section 1. TERM OF AGREEMENT
This agreement begins July 1, 2019 and runs through July 31, 2020. Any work performed prior
to full execution of this Agreement is hereby ratified.
Both parties have the right to terminate this agreement at any time by giving ten (10) days
written notice.
This agreement may be amended by mutual written consent of both parties.
All others terms and conditions of the MOA remain in effect and the MOA and this Amendment represents the
sum total of the mutual agreement by and between the Quilcene School District and Jefferson County Public
Health.
