STATEMENT OF ISSUE:
Jefferson County Public Health is requesting Board approval of Amendment No. 1 to the Agency Agreement
—Division of Behavioral Health and Recovery (DBHR) with Washington State Health Care Authority (HCA),
July 1, 2019 — September 29, 2019; Additional $21,236 for a total of $128,281.
ANALYSIS/ STRATEGIC GOALS/PRO'S and CON'S:
This ongoing contract funds JCPH to coordinate and implement prevention programs designed to prevent or
delay the misuse and abuse of alcohol, marijuana, tobacco, and other drugs among youth up to age 18 and
adults ages 18-25. This amendment adds $21,236 to support Port Townsend Community Coalition activities
including coordination, prevention, and training based on evidence -based practices as identified in the
Excellence in Prevention Strategies List.
FISCAL IMPACT/COST BENEFIT ANALYSIS:
$3,236 is funded from the Partnerships for Success (CFDA#93.243) and $18,000 is from the State Opioid
Response (CFDA#93.288).
RECOMMENDATION:
JCPH management request approval of Amendment No. 1 to the Agency Agreement — Division of Behavioral
Health and Recovery with WA HCA, July 1, 2019 - September 29, 2019; Additional $21,236 for a total of
$128,281.

WHEREAS, HCA and Contractor previously entered into a Contract for obtaining Community
Prevention and Wellness Initiative (CPWI) Prevention Services in order to increase capacity to
implement direct and environmental substance use prevention services in high need communities
qualified to immediately implement identified evidence -based practices and programs to prevent and
reduce the misuse and abuse of alcohol, tobacco, marijuana, opioids, and other drugs and,
WHEREAS, HCA and Contractor wish to amend the Contract pursuant to Section 4.3 to award funding
to increase capacity for coalitions to implement prevention programming in their community as outlined
in the coalition's strategic/action plan;
NOW THEREFORE, the parties agree the Contact is amended as follows:
The Contractor shall implement all programs as approved by HCA/DBHR in the proposed action
plan.
2. Total consideration payable to the Contractor for this Amendment for timely and satisfactory
performance of the work is based on the following:
A. The source of the funds are:
1) Partnerships for Success 2018 (PFS 2018) CFDA#93.243
a. Period of Performance is July 1, 2019 through September 29, 2019

B. Funds shall be utilized according to the requirements and guidelines established for funding

C. Funds must be fully obligated and all costs incurred by September 29, 2019.
D. Final billing for work under this exhibit shall be submitted no later than December 12, 2019.
3. This Amendment will be effective July 1, 2019 ("Effective Date").
4. All capitalized terms not otherwise defined herein have the meaning ascribed to them in the
Contract.
5. All other terms and conditions of the Contract remain unchanged and in full force and effect.
The parties signing below warrant that they have read and understand this Amendment and have
authority to execute the Amendment. This Amendment will be binding on HCA only upon signature by
HCA.
