Can we get an extension for this permit ? We are a couple weeks from being complete but the permit expires on July
3oth.

Dear Rick,
Thanks for meeting with Department Staff. l've attached two invoices, two for Health and one for DCD. The DCD fees are
actually lower than what they truly are. I didn't start tracking case time until August 2016. So I totalled the time and
subtracted the fees that were already paid at intake (Consistency Review Fee and Shoreline Exemption base). The
difference is the sum of the time for Donna and myself.
The next invoice will be quarterly or through which finish, whichever comes first.
Here is a summary of our notes from the meeting today:
. Need accurate and consistent revised site plan for both BLD11-00223 and BLD15-00207
. The site plan should overlay the NTI survey and incorporate the notes and #'d site plan with photos that Susan
provided.
o Show downspout locations and flowpaths
. Differentiate the existing and revised water line, perhaps with color coding
r Show old faucet valves for old RV site.
. Show outhouse
o The size should be 11" x 17" using an engineer's scale.
. Septic designer needs to submit modification permit for existing drainfield
Dixie will submit a revised CASP. Donna will review this and charge hourly fees. Cunent rate is $9O/hour. DCD
recommends that the CASP revision be approved before submittal for a revised BLD11-00223 and BLD15-OO2O7

Discussed: possibility that lot consolidation versus restrictive covenant could provide preservation credit for the CASP
because it cannot be reversed unlike the restrictive covenant thereby providing a high level of protection. Donna will
research this. The reserve area that must be mitigated is '12' x 67'. We discussed the possibility of using the access
driveway to the old cabin as CASP mitigation planting. DCD is concerned that this option would block ingress/egress and
for construction equipment should the structure ever need repairs.

So I just spoke to management about your permit-
Because you can't really do any work on this permit BLDI I-00223 until BLD15-00207 is released and the
CASP stuff is worked out and approved by our department.
I think it makes since to just place renewal fee on hold for BLD1 l-00223 - Please do not pay this renewal fee

Rick,
I met with Linda Atkins today to discuss your proposal. We would agree to inspection for an
hourlyfee of 590/hour (includingtravel) of the water line as you requested provided thatthe
following is uncovered (refer to enclosed site plan also, upside down, sorry!);
1,. in the area of where it appears the water line and sewer line cross
2. at a point southwesterly along the water line to where the minimum 10'from the sewer
line can be found or
3. to where you intent to cut and extent the line into the proposed construction under
BLD15-00207 (unknown, not marked on the map)
4. an area to show where the water line was constructed to serve BLD11-00223 verifying
setback to the end of the drainfield. Be sure that the inspection ports on the drainfield
are available for inspection.
Upon satisfactory inspection we would allow BLD11-00223 to be finaled.
A condition of final for BLD15 -00207, when approved, would then require a septic designer to
conduct an inspection and submit a report verifying pumps and controls are set accurately and
submit a revised record drawing reflecting the final status of the site and components per
requirements in JCC8.15.110 (10):
