Brown kandali M. Durant, R.S. A.M. O'Meara Phyllis Bee, P.H.N. Carroll M. Mercer, Chairman Joseph Fishnaller, M.D. Gael Stuart, Admin. MINUTES June 7, 1978 9:30A.M. Meeting called to order by Chairman Carroll M. Mercer with all members present with the exception of A.M. O'Meara who is on vacation. Minutes: The minutes of May 3, 2978, were read and approved as presented by mot on of B.G. Brown and second by C.M. Mercer. CORRESPONDENCE; Chairman Mercer stated he received a letter from Bob Hays complaining that Dr. Rowe had authorized the Health Nurse to administer his shot medication and Rowe isn't his doctor, and he has been refused further treatment from the Health Department. Dr. Fishnaller explained that some doctor had to approve the giving of a shot medication by a nurse, or the nurse puts her license on the line. Mrs. Bee said Dr. Rowe actually signed Mr. Hays' prescription. Dr. Fishnaller said he has stopped the practice of giving shots, i.e. 4 allergy shots, hormone shots, vitamin shots, etc., in both Clallam and Jeffer- son Counties because: 1) they are not a primary care facility; 2) legally he cannot avoid personal responsibility for patients he has never seen; and 3) the county is also responsible in so far as he is their agent. Dr. Fish- nailer will write a letter of explanation to Mr. Hays. FINANCIAL REPORT: Gael Stuart submitted a copy of the financial report update (attachment #1) . Items discussed were: 1. Small problem in operating supplies. 2. Overdraft in Home Health Aide column due to budget extension for CETA position not yet being included. 3. Overdraft in office equipment and operation equipment - part of over- draft due to MCH, WIC program. 4. $4,000 budgeted for malpractice insurance, and county is paying for it. In the future, they will not budget for this item. 5. Amount of money coming from the other Health Department is not reflected in the budget - supposedly approximately $45,000 will be coming from the Olympic Health District. 6. MCH Grant will be a plus financial operation because part of normal costs were picked up in grant. Mr. Stuart advised the budget has money in it that will not be expended, and there will be a surplus at the end of the year. OLp BUSINESS: 1. Pleasant Tides —Site evaluations and sewage disposal: Randy Durant reported that Steve Crane, Attny. for Austin Frazer, requested today's hearing be postponed until July meeting. 2. Olympic Health District property distribution: $960.00 has been received from the Olympic Health District, which is a partial distri- bution of funds. Mr. Stuart still has not received an outline of firm figures, but he feels there is approximately $19,000 to $20,000 due Jefferson County. Pros. Attny. Howard is to get the figures from both Clallam and Jefferson County Treasurers on amounts that hadn't been transferred, plus what was actually in the fund. NEW BUSINESS: ' 1. Applications are being received for a half-time public health nurse. The Board requests to be notified of time and date of interviews so they can attend if possible. 2. A .part time secretary has been hired to fill in for vacations. Salary is part of MCH grant. • Minutes of Board of Health Meeting June 7, 1978 page 2 REPORTS: 1. Public Health Nursing: Phyllis Bee A. Glenn Peck, from State MCH Department, audited their MCH program and was very pleased with their accomplishments in relation to their plans and objectives. Accomplishments to date: 1) Lack of adequate health care and supervision for children - service population projected for the year is 373 children; Well Child Clinics started in March, and 98 children have been seen - 95 new patients and 3 returns. Out of this number, 15 were referred to their private physican. 2) To provide adequate care for high risk newborns - 40 people (infants and parents) were projected for the year - 17 have been seen. 3) Home deliveries by an amateur - to provide adequate care • and follow up. A good job has been done here too. 4) Scoliosis Screening - planned to serve 280 school children and 203 have been served already. When the half-time public nurse starts working they will probably have to • hold more clinics. B. Most of the new equipment has arrived. • C. School nurse program is drawing to a close and annual reports will be ready as soon as they are typed up. D. VD business has picked up. E. Gael Stuart said they do not have contracts developed between the schools and the Department, or the Olympic Health Department. He and David Abel will work together to draft a common contract with the schools for the provision of school health services that will reflect costs, and also spell out services. F. Summer immunization clinics will be scheduled either on Saturdays or evenings in Port Townsend, Chimacum, and Quilcene, so parents can get their children immunized before school starts. 2. Environmental Health: Randy Durant A. Along with the Activity Report (attachment #2-A & 2-B) , Randy pointed out that almost all of their time is being spent with the sewage program. In the near future he will prepare a more detailed outline of their duties and activities comparing not only last year and this, but also, comparing Jefferson with comparable counties, especially Clallam. • B. They are now permitting grocery stores and will be inspecting at least semi-annually. C. Pleasant Tides - Randy met with David Roberts, and at that time, he basically agreed that he could not see a way to design systems that would function property on Mr. Frazer's three lots. Steve Crane requested they be placed on next months agenda. D. They are working on camping vehicle regulations - only have guidelines to work with at the present time. E. Food Program - restaurant g permits expire July 1, 1978. F. Thelma Clark property, Shine: Mr Wiggs advised Randy that he • had done six perculation tests and the results were very un- acceptable. He will respond to Mrs. Clark. 3. Health Officer: Joseph Fishnaller - A. Attended TB course in Everett on June 1 & 2, 1978, basically for points toward keeping his license in effect. 1 Minutes of Board of Health Meeting June 7, 1978 page 3 • B. Home Health Program: Having organization problems in Clallam County. Will know more about it in a month. 4. Health Department Administration: Gael Stuart A. Office of Ageing: 1. Has submitted a letter asking if we would be interested in establishing a "chore service" and "small home repair'', ' and if our CAP organization would run the program for the four counties. Mr. Stuart will discuss it with the CAP Board. 2. Health Screening Program - has foot care, but is not pay- ing for it. They are trying to set up a way that the Office On Ageing, through the State, will include foot care in their package. Mr. Stuart advised them that it would pose no problem here, provided it was paid fbr. B. It is absolutely essential that we develop policies regarding the actual activities of nurses in the health screening program. They should be written by the Health Officer in connection with the medical community. They are needed in environmental health, public health nurse areas, and as part of the school contracts. • ,1; ' C. Comment: Mr. Stuart said due to the competence of the Depart- ment Heads, he feels the need for an administrator should be examined. All concurred that Mr. Stuart is definitely needed. Director of Environmental Health Subject: Activities - Environmental Health for May, 1978 Field Work: Totals for sewage disposal permits, site evaluations, and plats attached. Fifty three sewage disposal permits were issued (approx. 52% increase). Ninety five site evaluations k were made (255% increase). Time involved in office has increased accordily. We have been unable due to the time anything spent in the sewage program to do much of else. If this continues I will be asking for more help and giving a 4 more detailed outline of our responsibilities.
