ANALYSIS: Habitat for Humanity has requested to amend the scope of work(Exhibit A) to the Affordable Housing/Homelessness grant funding agreement that was approved by the Board on April 20, 2020. FISCAL IMPACT: None RECOMMENDATION: Approve and sign the amendment to agreement. REVIEWE D B `ilip M y, ounty Admini rator Date: Grant Agreement Amendment No. 1 by and Between Jefferson County and Habitat for Humanity For Affordable Housing/Homelessness Services Grant Funding This Amendment No. 1 is by and between JEFFERSON COUNTY (hereinafter known as "COUNTY") and HABITAT FOR HUMANITY (hereinafter known as "HABITAT"). WHEREAS, the COUNTY and HABITAT entered into an Agreement on April 20, 2020 for Affordable Housing/Homelessness Services Grant Funding; and WHEREAS, the parties desire to amend the terms of that Agreement; and NOW, THEREFORE, IT IS AGREED BETWEEN BOTH PARTIES AS NAMED HEREIN that the Agreement executed on April 20,2020 is hereby amended as follows: 1. EXHIBIT A—PROJECT DESCRIPTION is amended as follows: 1. At least two additional homes will be repaired. A scope of work will be prepared on each selected repair project addressing critical repairs that are emergency in nature; permits will be secured, work will be completed, and the homeowner will sign off on the completed project. Measures: Scope of Work is developed on each selected project; scope addresses all emergency critical repairs; permits are secured as necessary; homeowner signs off on completed project. 2. All other terms and conditions of the original Agreement executed on April 20, 2020 will remain the same.
