Please see below- Conditions on your permit- #7 NO FINAL WITHOUT DRD REVIEW OF MITIGATION (on page 3)- contact Donna Frostholm 360-379- 4466 to review the mitigation area #6 NO FINAL WITHOUT GEOTECH LETTER(on page 3) Final Inspection on (permit-page) BEFORE FINAL INSPECTION- ASBUILT FROM PHILP CONRAD AT ADI CONFIRMING TRAM WAS INSTALLED PER PLANS Before we can perform the final inspections on this permit- the above conditions must be received and signed off. Donna has Condition#6&7 been completed? I do not see the as built nor the geotech letter in the approved building file-please check your MLA file. If these have been received please sign off the condition in our database. Finally- You have requested final inspection for this Wednesday 05/17/17. The good news is it is Monday- We have two day to meet the above conditions and have them signed off- If you have not contacted Donna to review the mitigation plan-please do so. Also if you could check the status of the as built and the final letter from the Geo tech that would be greatly appreciated. If these things are not signed off by Wednesday—we will not be able to perform the final inspections, until they have been complete. Let me know if you have any questions. I left a correction and spoke with Kim who is the representative for the contractor.The anchor bolts at the front of the form were not according to the plan.The anchor bolts for the motor mount were not set in the forms or onsite. She said they are to be epoxied in post pour and will provide a letter from the engineer.The tie back anchor are also not per plan and she will provide a letter that addresses that as well. I explained that she should have those letters in hand prior to placing concrete. She felt it would be no problem but will cancel the pour if she doesn't get the letters.They want to pour in early AM if possible. Her number is 360-979-6273. Left pictures on your desk.
