Location:Slab on grade and footings at BA-BC/B I.Antenna tower footing west side of building A. Level:Foundation. Plans Date: 12/30/13. Reinforcing Steel: Inspected the placement of ASTM A615 Grade 60 reinforcing steel into 5'3"x 5'3"x 4'deep antenna tower footing witin the locations I noted above. #7 at 12"o.c.each way,top and bottom. All reinforcing steel was of the proper size,grade,lap,spacing,location,clearance and secured prior to concrete placement. Concrete Placement: Observed the placement of 90 CY of mix design number PV40 into slab on grade and footings within the locations noted above. One set of 5 4x8 cylinders were cast for compressive strength test specimens.Time,temperature,slump and air tests were performed. Reference report numbers C39297 and F63930. Concrete was mechanically consolidated using a high cycle vibrator.Concrete was placed using a boom pump. 1 i Work performed that was visually inspected was found to be in conformance with the project plans and applicable code.
